1538401636 2018-10-01 Preliminary Restoration No Violations On this day, I am conducting a preliminary restoration inspection. The site meets ORC 1509.072 (A). The well is equipped with 4.5 inch diameter casing and 1.9 inch diameter tubing with the production valves open. It is produced by means of a pumping unit with rods and a gas engine. There is (1) 100 barrel steel tank, drip tank, vertical separator, and domestic meter all contained within an earthen dike. The tank is properly identified and the production records are current. It will be noted that the pumping unit is running during the inspection.
1751759887 2018-08-08 Preliminary Restoration No Violations On this day, I am conducting a follow-up inspection for the preliminary restoration, pertaining to a slip that occurred along the hillside, near the wellhead. Recent activity has been conducted and the hillside appears to be stabilized. Awaiting on vegetation to establish before passing the preliminary restoration.
1952525150 2018-03-19 Preliminary Restoration No Violations Remedy required to pass ORC 1509.072. Erosional issues and minor slips along lease road that will need to be maintained. The well was completed in 2012 and never had a preliminary restoration inspection. The issues appear recent, due to weather. Pictures were taken during the inspection. I spoke with Whitacre Enterprise pumper, Kevin Winland on 03/21/2018, to inform him of the issue, and he said there is plans to remediate the area once the weather breaks.
