Inspector did also look over the excavation area of the freshwater impoundment with the following observed, no grades steeper than 3 to 1 and silt fence properly erected at the bottom of the grades. The company representative Mark Hall stated that He will be have the final excavation done and the total area seeded and mulched within the next week .

1358198628 2013-01-14 Drill / Deepen / Reopen No Violations Inspector on site for a status update on the stimulation completion process. Baker Hughes stimulation and Baker Atlas wire line Companies are currently stimulating the 3rd of 15 stages to be completed. A electric type Big Mouth completion plug was set @ 12,923 and the casing was perferated in intervals from 12,873 to 12,654 with a total of 60 perforations. With the initial break pressure of 6,874 PSI with an average treating rate of 75 BBL per minute with a total of 10,00 BBL per stage. Production casing annular pressure is being monitored and has remained static.

1400264887 2012-09-25 Drill / Deepen / Reopen No Violations Inspector stopped by drill site for an update on drilling progress and to inspect the condition of the 9.625 casing to be run into the well bore. Status is as follows- Current bore hole was TD @ 7613 and casing crew is on site preparing to start running 9.625 casing into the well bore. Casing is new with all manufacture ratings clearly identifiable.
1433784279 2015-06-08 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well site has sufficient vegetation and erosion control, there is one producing well on the pad and 3 additional well that have been spudded and only the surface casing set. There is a additional water impoundment at the rear of the well pad. The production fluid storage are is located at the bottom of the hill along County Road 16 and consists of 2-400 barrel capacity tanks located in a metal containment dike with a plastic liner. Preliminary Restoration passed on this day.
1491024011 2012-10-06 Drill / Deepen / Reopen No Violations Inspector received a call at 4Pm on 10-05-12 from a Ruth Partin who lives at 37224 Sycamore Valley Ohio- contact Ph# 1-740-934-2811, Who stated she followed a Hall Drilling tank truck along Ohio Sr 260 & 145 that appeared to be leaking a clear product from the back of the truck. Mrs. Parton stated that She aggressively passed the transport vehicle and completely blocked the road in efforts to advise the driver. And also stated that She notified the Ohio State Highway Patrol and Ohio EPA, ODNR Oil& Gas and a local Sheriff. My investigation proceeded as follows, Immediate contact was made with the Ohio EPA- Who received a call at 2:13pm on 10-04-12 and did not respond but stated the time Mrs.Partin saw this was 9:30 Am, Then to the Ohio State Highway patrol- who never received a call, and to both the Noble and Monroe co Sheriffs offices who were not notified by Mrs. Partin either, as She had stated. Notification was made to Mike Hall the operations owner and Vaughn Staples the drilling superintendent on duty and no waste product was shipped from the site on the day of 10-04-2012, A truck containing Bio-Degradable drilling fluid did arrive on site and was unable to unload due to the fact that the unload area was inaccessible and was asked to return at a later date. At this time it is my understanding that due to the extreme grade going into the site that most likely some of the residue water laying in the open trough divided overflow pans of the tanker was forced by gravity to another holding area and as the truck traveled this water was being dispersed as designed to do so. I also drove the road claimed by Mrs.Partin to be affected at 8am the following morning and found no sign of product on the road.
1565808783 2018-03-15 Production Wells No Violations On this day, I found the well equipped with annular pressure monitoring devices. The surface pressure gauge is reading 0 pounds per square inch, and the intermediate gauge is reading 0 pounds per square inch. There is (1) well in production, and (3) wells that have surface casing set, but were never completed. The inactive wells have sealed casing at the surface. All wells have concrete barriers surrounding them. The facility is gated with proper signage. There is a north pad and a south pad. The south pad contains the wells, along with (2) 400 barrel steel tanks within containment, on the back right-side. The north pad contains the midstream facility, (1) gas processing unit within containment, and (1) 400 barrel steel storage tank, also within containment. There are (2) additional 400 barrel steel tanks within containment, along County Road 16, near the lease road entrance. (Lat. 39.70921 Long. -81.25219) It will be noted that I spoke with Antero representative, Jonah Turner, during my inspection, and informed him to make sure that his operators have their wellheads identified with proper signage.

1754686432 2012-09-26 Drill / Deepen / Reopen No Violations Inspector on site to witness the last 4 joints of 9.625 casing being run into the well bore and the 2 stage cement job, Halliburton well svc on site to cement casing. Inspector did not witnes any cement returns on either of the 2 procedures completed both had adequate lift pressure and circulation prior to and during the cementing process. Post job discussion with the company engineer concluded with the resolution of Bond Logging to confirm adequate casing string tie according to ORC.
1944148856 2012-09-27 Drill / Deepen / Reopen No Violations Inspector was on site to witness the bond logging of the 9.625 to confirm the tie in between the 13.375 and the 9.625, and witness the pressure testing of the BOP and 9.625. The bond log showed proper cement placement. The Blow out preventer tested good, and the 9.625 casing passed proper pressure testing.
2021642461 2013-10-07 Preliminary Restoration No Violations Status check on the Preliminary restoration found the well bore drilling and stimulation process complete and the company installing the production flow line and setting the tank battery up. Proper identification need posted at the well head and tank battery. I will contact the company and ask the to post proper identification. The site initial restoration meets ORC 1509.072 with proper erosion control and vegetation established, I will note that a fresh water impoundment is located on location with fencing in place and a 14 water level observed.
