Anteros people were on site and made the needed corrections accordingly.

The drilling supervisor and myself did walk the location and review the recent slip on the North East side of the location and observed the company has made efforts to deter the surface water from further infiltrating the area.
0663299104 2018-03-07 Production Wells No Violations On this day, I accompanied ODNR Inspector William Stolarik. Please refer to his comments for a detailed report. The well is equipped with annular pressure monitoring devices. Concrete barriers surround the wellheads. The facility is gated with an electronic shutdown button on the right-hand side, prior to the pad. The front left of the pad is equipped with (8) 400 barrel steel tanks within containment and an electronic shutdown button. There are (6) gas processing units along the back of the pad, and a midstream facility behind the gas processing units.

1019705439 2014-02-21 Drill / Deepen / Reopen No Violations At 8:00 am I met Bill Stolarik, Conrad Baston, Tom Wince, Joe Honeycutt, and Josh Allred to discuss the slip that has developed. Conrad stated that the construction records from the site indicated that neither the rig nor any production equipment was jeopardized by the slip. Conrad agreed to submit to ODNR DOGRM a plan in writing as soon as it is finalized. The rig is currently drilling at a depth of 13459.
1078769701 2018-03-07 Production Wells No Violations Status check inspection accompanied by ODNR Inspector Tom Thomas to document the well status. The well is equipped with a casing and tubing well head assembly with (2) manually operated valves on the tubing well head and one emergency shut down valve on the tubing tree production flow line. The well head casing flow line is also tied into the production flow line however the valves are in the closed position. The 13.375 and 9.625 casing well head assemblies have flowlines with valves and gauges installed to monitor annular pressure. The 13.375 casing well head pressure gauge reads 700 pounds pressure and the 9.625 casing well head reads 700 pounds pressure. On pad total is (6) complete wells supported by (6) gas processing units. Also on location is a production fluid facility consisting of a total of (8) steel construction tanks with a capacity of 400 barrels of fluid each, the tanks are situated inside a polymer lined steel containment dike area. The well lease road entrance is gated and locked with signage posted at the entrance of the well pad and at each well head. The well pad has a stone surface along with the lease road. The well pad perimeter berm is of earthen material and in good repair.

1290355557 2014-02-20 Drill / Deepen / Reopen Violations Noted On the day of my inspection I witnessed that Patterson UTI Drilling Rig #342 was drilling an 8.5 inch diameter lateral hole at a depth of 12313 feet on 13.1# oil based drilling mud with an estimated td of 16824 feet. Site inspection accompanied by Company Man revealed a slip on the northeast corner of drill pad. Core samples have been taken and are awaiting results for a plan of action to address. I requested that Antero expedite a plan to address the slip as thaw has begun and the slip will only worsen. At the time of inspection simultaneous operation was taking place with the drilling and 2 wells producing. Repairs were being made to the drilling unit primary containment. Noble County Inspector Eric Conrad made call to Antero Construction Engineer and he stated that a plan would be emailed to Eric tonight. Monroe County Inspector Bill Stolarik will meet with them tomorrow.
1371592464 2013-06-16 Casing String Witnessed No Violations On this day I witnessed the setting of 85 of 26 conductor casing with 30 barrels of cement returns to surface, the 100 minimum per permit requirements could not be reached due to the initial site excavation removing 80 of cover down to solid bedrock. Division Deputy Chief Dave Claus was made aware of this issue on 06/14/13 by Dave Ball on 3411124374.
1385157101 2013-11-22 Casing String Witnessed No Violations Inspector on site to witness the cementing of the 18.625 surface casing, 413 of pipe was set inside of a 435 24 diameter bore hole and cemented with 849 sacks of class A 2% calcium 1/4# flake cement with 63 barrels of good cement returns witnessed to surface and the casing pressure tested to 500 psi over lift pressure upon the landing of the cement plug.

Upon inspection of the containment I reminded the company representative to closely monitor it as the snow melts. A substantial slip has developed on the east side of the pad. The company drilling representative stated that soil sampling was done and a plan developed to stabilize the slip.
1393002274 2014-02-20 Administrative Inspection No Violations I called Daniel Smith with Antero and asked him about the slip. Mr. Smith stated that Antero engineers were formulating a plan to stabilize the slip. I received a call from Conrad Baston, Antero Civil Engineering General Manager, at 3:42pm. He agreed to meet onsite and discuss the slip.
1393065583 2014-02-21 Surface Facility Construction No Violations On the day of my inspection I was accompanied by Inspector Eric Conrad and Anteros environmental construction personnel with the inspection and meeting purpose of reviewing a slip that has occurred on the North East corner of the drill site pad, On this day the company has presented a remedial plan of action to repair the slip along with a temporary plan of action to stabilize the area by the means of diverting the surface water infiltrating the area, protective boundaries will be established for safety reasons. Completed engineered drawings will be submitted immediately upon completion to the Division, The affected area was photographed and my direct supervisor Rocky King notified and sent pictures of the situation.

Inspection of the recent slip area found the slip has not progressed since my last inspection.
1393506060 2014-02-27 Administrative Inspection No Violations On this day following up on NOV-1290355557, I contacted Monroe County Inspector Bill Stolarik and he and Noble County Inspector Eric Conrad have met with Antero Resources Corp. Construction Engineer and a plan has been submitted to address slip. Temporary berms are in place to redirect surface water from impacting slip.
