0048633452 2013-09-04 Drill / Deepen / Reopen No Violations On this day I picked up the cement tickets and casing tally from the 9.625 casing & cement job and entered the information in the RBDMS database, The Drilling Co Falcon drilling is skidding the drilling rig to the next well on the pad and will come back and drill the Horizontal section after completing all vertical well bores on the pad.
0223716783 2014-04-03 Work Over Reconditioning No Violations Status check inspection of the 2nd pressure squeeze job on the production casing, Halliburton cement service was on location and used 7 barrels of polymer type resin cement to try and repair the leaking well bore section. The well bore section was pressures up to 9,100 psi during the squeeze. The company will let the resin set for 48 hours and then drill it back out.
0232410455 2014-04-02 Work Over Reconditioning No Violations Notification was received on this day from the well owner Mike Hall that the recent squeeze job on the 5.5 production casing has been drilled out and pressure tested showing a 400 psi leak in 20 minutes, they are going to re-squeeze the leaking area again with a cement resin.
0289254683 2014-02-05 Completion Testing No Violations Status check inspection on the completion process found that during the initial pressure test the 5.5 production casing did not pass the pressure test pre stimulation test procedure with a 1,000 psi loss in 5 minutes, a bond log was attempted to be ran to evaluate the area of the production string that a external casing patch was set at a depth of 7,554 and the logging tools set down on a obstruction approximately 100 above the patched area. The well owner Mike Hall was on site and notified me of the situation and said there tentative plans are to clean out the obstruction believed to be cement and re evaluate the well by the means of cased hole logging using the following possible tools, a temperature- casing caliper and radial bond log. My supervisor Rocky King was notified of the situation on this day.

Wild Well is using water cannons to direct the flow during the rig down process. The service rig is being rigged down and moved off of site in preparation for the wellhead repair.
0560350888 2018-03-13 Production Wells No Violations On this day, I found the well equipped with annular pressure monitoring devices. The surface pressure gauge is reading 370 pounds per square inch, and the intermediate gauge is reading 360 pounds per square inch. There are (4) wells in production on this pad. All wells have concrete barriers surrounding them. The facility is gated with proper signage. There are (4) 400 barrel steel tanks within containment at the front right-side, prior to the pad, with an electronic shutdown button. There is a midstream facility at the front right-side of the pad, and (4) gas processing units beside it. The wellheads are not properly identified, but it will be noted that I spoke with Antero representative, Jonah Turner, during my inspection on another pad, and informed him to make sure that his operators have their wellheads identified with proper signage.
0790879572 2013-08-19 Casing String Witnessed No Violations Inspection to witness the surface casing pressure test, Casing was tested to 300 psi with no recorded pressure loss. We also reviewed the site containment and went over the basic well design plans with the new company man overseeing the drilling operations, The were given a list of phone numbers for inspectors.
0843365391 2018-10-04 Production Wells No Violations Status check inspection found (4) wells present on the pad with all wells found to be in production. The lease road surface is stone construction and in good repair. There is a gate across the lease road with owner and well pad information present. The production facility is also along the lease road and not on the well pad. The production facility consists of (4) steel construction tanks capable of holding 400 fluid barrels capacity each. The tanks are situated inside a polymer lined steel containment dike area with hazard warning labels present. There is also an earthen berm around the production facility. Situated on the well pad is a large gas compressor. Addressing the well head the well is equipped with a lower A and upper B section with a production tree present. The production valves are in the open position. One 2 diameter riser pipe is present with a valve and gauge attached for annular pressure monitoring. The 9.625 well head section showed zero pounds pressure. Also noted the South East slope of the well pad does have an earthen slip that is being addressed by the Division Engineering section. The slip has required 10,000 to 12,000 cubic yards of material to be removed. The company is remediating the slip under emergency stabilization guidelines under the well construction rules.
0895911211 2014-04-09 Brine Hauler No Violations Status check inspection of the completion well process found Fortis well service completing the well by drilling out the recent production casing squeeze job and running 2.375 tubing, upon my last inspection it was observed that several of the Hall Drilling water trucks did not have their proper UIC identification on the trucks and on this day I observed the trucks now have the proper UIC identification assigned to Hall Drilling.

We discussed having environmental firm take water samples from near by waterways to monitor for any potential hazards in the event of brine getting off site.

1082336731 2014-06-26 Completion Testing No Violations Status check inspection of the current well control situation found that the Fortis well service was removed this morning off the hole along with any other rig support equipment was removed from the well head area, the well control company Wild Well Control is moving the casing Hydro cut machine onto the location and staging it near the well head in preparation to cut the failed well head assembly off tomorrow morning, the water way chloride levels were check at 3 points near the well location and no readings were found over 500 parts per million.

Baker Hughes cement services was the cement contractor using a lead and tail Class A cement mix for a total of 2286 sacks of cement with the excess amount of 35% ran and calculated estimated of 108 barrels of cement returns to surface.

1384297961 2013-11-12 Casing String Witnessed No Violations Status check inspection found that during the running of the 5.5 production casing the casing parted at 7443 between joint 215 @ 216, the casing is parted approximately 200 up from the bottom of the 9.625 casing. Baker Hughes fishing service is on location to remedy the problem.
1385309695 2013-11-24 Drill / Deepen / Reopen No Violations Status check inspection on the recovery efforts to set a external casing patch and reattach the parted 5.5 production casing, The casing is current separated with the bottom section set in the hole to the well bore total depth of 16,545 and the top of the bottom section at 7,532.5. The company is currently running in the hole with the top section of 5.5 production with a National Oil Well Varco high pressure external casing patch and will be attempting to rejoin the separated casing string using the patch, The company will the set a retrievable type high pressure bridge plug at a point inside the 5.5 casing below the patch area and pressure test the casing consist to a pre approved workable pressure on fluid.
1385951269 2013-11-29 Casing String Witnessed No Violations Report to document the repair and pressure test of the parted 5.5 production casing string, The casing initially parted during the initial stages of the cement job on 11/08/13 and the cement was immediately circulated out of the hole, Over the course of the next several weeks efforts were made to rejoin the casing with the end results being a overshot casing patch set at 7,554, the casing was also pressure tested to 3,100 psi by the method of setting a retrievable bridge plug at 7680 and pressuring up on the internal side of the casing. A casing collar log was ran indicating the location of the patch with a record received.
1395863194 2014-03-26 Work Over Reconditioning No Violations Notification was received today at 1:27 pm from the company owner Mike Hall in regards to the issue with the production casing not pressure testing, the leak has been isolated to the external casing patch set in the production casing at a vertical depth of 7443 with 2 solid bridge plugs being set below the area suspected of leaking and a pressure test completed showing a 2000 psi leak in 10 minutes at a top pressure of 8000 psi, they are formulating a plan with Halliburton to use a resin type material to try and squeeze this material into the area of the casing that is leaking.
1396033855 2014-03-28 Work Over Reconditioning No Violations Status check on site inspection to witness the 5.5 production casing remedial repair work where a external casing patch was previously set during the well completion process and has been identified as leaking, the patch was installed by running it on the top section of the parted production casing string and stinging into the bottom section of parted production casing with a logged location depth of 7,532 to 7,537, a cast iron bridge plug was set below the patch inside the casing at 7,565 to isolate the patch, 2.375 tubing was ran in the production casing to a depth of 7,520 and 10 barrels of Halliburton well lock resin type well bore repair material was then spotted and squeezed under pressure into the leaking casing patch area with a initial squeeze pressure test showing a 1000 psi loss in 10 minutes and after displacing 4 barrels of well lock into the leaking area the last pressure test recorded before shutting the well in to allow the material to cure was 200 psi in 30 minutes. The material will be allowed to cure for 48 hours before being drilled out and pressure tested.
1396385052 2014-04-01 Work Over Reconditioning No Violations Status check inspection on the drill out of the resin type material used in the production casing squeeze job on 03/28/14, the material was tagged at a depth of 7,123 with the location point of the area squeezed at 7,532 to 7,537 and a solid bridge plug at 7,565 giving a calculated interior casing fill up of 442 divided by 43 coverage per barrel squeezed leave an approximate .38bbl calculated displacement into the annular area of the casing. The company shall drill out the resin to the top of the bridge plug and call me for the pressure test.
1403398650 2014-06-21 Completion Testing No Violations Notification was received from Mike Hall by phone at 9:52 am on this day that while drilling out the completion plugs a valve on the B section of the well head assembly that is attached to the 5.5 production casing had failed and the well was discharging gas and fluid uncontrollably, Mike informed me tubing was in the hole and Fortis well service was rigged up on the hole with the Blow out preventer in place and Wild Well Control Company was enroot from Texas, Mike also has notified a 3rd party contractor to begin mixing a mud solution to kill the well, I suggested the notify 911 dispatch, I called Inspector Dave Ball who was on duty to respond and he was on location by 11:00 am, I also immediately notified my supervisor Rocky King of the well control situation.
1403486917 2014-06-22 Completion Testing No Violations Administrative inspection to document the current well control situation status, at 11:17 am I made phone contact with Hall Drilling owner Mike Hall and was advised the attempts to kill the well with a mud pill was unsuccessful and they believe the tubing has a friction hole in it below the well head, Wild Well control is sending more men and equipment to the location and shall be rigging up water cannon monitors to divert the well product coming out of the well head in order for the work over rig to be moved and the tubing snubbing unit and blow out preventer to be disassembled, the tentative plan is to remove the well head and install a new well head, Mr. Hall also informed me that a fluid pit would be constructed on site.
1403554636 2014-06-23 Completion Testing No Violations On this day I was on site from 9:00 am to 5:00 pm to observed the ongoing well control issue along with my supervisor Rocky King and Inspector Dave Ball, we were in contact with the well owner Mike Hall and discussed a secondary recovery plan of action in the event that well fluid product would escape the location boundaries, 3rd party environmental contractors are being brought in to monitor for any off site impact, I inspected the wet weater stream beds nearest to the well pad along township road 289 and no flowing or standing water was observed, Warren trucking is on site with a crane to disassemble the service rig and blow out preventer, once the equipment is removed Wild Well Control company has a plan of action to remover tubing snubbing unit and blow out preventer along with the damaged well head assembly and replace the well head assembly to gain well control back. Wild Well Control has established proper working perimeters and staging areas and is monitoring the well site gas levels.
1403563056 2014-06-21 Completion Testing No Violations Received phone call from county inspector Bill Stolarik stating that he had received call from Hall Drilling that valve on BOP stack has failed and they have no control over well and he asked if I would go to site and get a visual on the problem and after arriving on site it was determined that there was gas and water spewing from faulty valve area and it was being contained on site at this time.

Note: Suggested to well owner that they notify local EMA of the situation and also talk to local Fire Departments and also inquire if that have the capability to spray foam in case of a fire at the site.The well owner would take suggestion into consideration and they also informed me that Wild Well control folks have been contacted and they were in route to well site.

1403638225 2014-06-24 Completion Testing No Violations My inspection today finds the well is still flowing. Wild Well Services is onsite to provide well control services. I spoke to Mike Hall owner of Hall Drilling. Mr. Hall stated that they were continuing pump two barrels a minute through the tubing. I observed the pit on location being lined.
1403741349 2014-06-25 Completion Testing No Violations On this day I visited the site at 9:00 am for approximately 30 minutes and returned at 2:20pm and was on site until 5:00pm to observe the current well control situation, Wild Well Control company is on site and has currently rigged up a 2flowline with no choke to the working pit that has been installed on location to help divert the well product into a controlled environment, they are also still pumping 2.5 barrels per minute of fresh water into the well head to help dissolve the gasses leaking from the Premium Valve company well head, Warren Drilling has a crane on location and is providing lifting and rigging support to disassemble the work over rig off the hole owned by Fortis well service, Due to the recent rain the dry weather defined ditch line that runs to the West of the location and passes under Bethel Township road 289 at a GPS location of EPE 15 -39.67347 X 81.24767 now has water running in it and per request of my supervisor Rocky King chloride tests were ran using both high and low level Quantab strip test and both levels read 640 parts per million.
1403927085 2014-06-27 Completion Testing No Violations On site inspection to witness the well control issue along with Supervisor Brent Bear, On this day Wild Well control company completed the following tasks- the well head was cut off just above the 5.5 casing outlet ports removing the BOP & snubbing unit- the bottom of the B section left remaining was then unbolted and lifted off leaving the lower base assembly remaining attached to the 9.625- a new ring seal was installed and new well head assembly was then set onto the lower half thus regaining well head control at 9:12 pm- just a little over 6 days and 12 hours after the initial call was received, Hall drilling crews are to remain on site until all product is recovered from the site surface, once the product is recovered the pit will be emptied and filled in.
1404079164 2014-06-29 Completion Testing No Violations Status check inspection to follow up on the clean up of the recent well control issue, the well has integrity restored at surface and the company has men on site hauling the fluid off location and recovering the standing fluid from inside the well pad containment area.
1404331651 2014-07-02 Completion Testing No Violations My inspection today finds the well is shut in. The other wells on the site are being turned on at the time of my inspection. I tested water in three locations that were previously tested. Test site one is approximately 700 from the wellhead, the water is running slowly at the time of the test. The water at this site tested at 4.8 (1000 PPM) on a high range Quantab. A second site 3000 southeast of the wellhead was taken from the Clear Fork of the Little Muskingum. The test yielded a .4 (below 32 PPM) on a low range Quantab. A third test site, 900 west of site one, tested at .4 (below 32 PPM) on a low range Quantab as well.
1405736762 2014-07-17 Completion Testing No Violations Status check inspection to follow up on the recent issue with the well where the well head had to be changed out due to a leaking flange on the B section of the well head found the company preparing to bring in contractors to finish drilling out the remaining 3 plugs in the well bore and recover the 2 tubing lost in the well bore with new containment to be put down prior to commencing the work over completion operation, the work over pit on location has been emptied and shall be closed in the next 48 hours.
1413422845 2013-11-15 Casing String Witnessed No Violations On site inspection to witness the remediation efforts to reattach the parted 5.5 production casing, The casing is separated down hole and the top joint looking up was removed by the Baker fisherman putting the top of the lower section at 7498.27, the company ran back in the hole with the top section string of casing and attempted to set the upper part of the casing string down on the lower section but was unsuccessful in getting the string to screw back together.
1424971457 2015-02-26 Preliminary Restoration No Violations Preliminary Restoration status check inspection found 4 wells total present on this pad at this time with the well head equipped with a production tree with the production valves in the open position selling into a gas processing unit equipped with a mechanical sand trap and USA Compression gas compressor set on the well location with a inline fluid separator and dryer system present. 4- 400 barrel production tanks are present near the well pad entrance supporting the wells on the pad, all located inside a mechanical dike area with proper well identification present at both the well head and tank battery area. Preliminary Restoration passed on this day.
1434628779 2015-06-18 Preliminary Restoration No Violations Preliminary Restoration follow up inspection found the restoration status is still showing up as active in the database and was signed off by myself on 02/26/2015. I once again have added the well GPS coordinates into the system also. Inspection of the well head found the following equipment present 5.5 production casing with the production valves open and selling into a 2 sales line. A total of 4 -400 barrel capacity tanks are present in a plastic lined metal constructed dike area. Chloride levels were tested at 4 random spots across the well pad with no levels showing up over 121 parts per million on a Quantab low level strip field test. Preliminary Restoration once again passed on this day.

1916746931 2013-11-20 Production Wells No Violations On site electrical inspection with Brad Sellers the Divisions inter agency coordinator and a inspector with the Ohio Department of Commerce- electrical inspector, On the day of inspection no deficiencies were found in the grounding system on the electrical services on site. The company did have their electrical inspector visit the site the next day and double check all the grounding on site.
1951379702 2013-08-26 Drill / Deepen / Reopen No Violations Status check inspection found the drilling rig waiting on the Nipple up crew to set the BOP on the well head, they will the complete a full pressure test of the BOP.
