The tanks are situated inside a polymer lined steel containment dike area with hazard warning labels present. There is also an earthen berm around the production facility. Situated on the well pad is a large gas compressor. Addressing the well head the well is equipped with a lower A and upper B section with a production tree present. The production valves are in the open position. One 2 diameter riser pipe is present with a valve and gauge attached for annular pressure monitoring. The 9.625 well head section showed zero pounds pressure. Also noted the South East slope of the well pad does have an earthen slip that is being addressed by the Division Engineering section. The slip has required 10,000 to 12,000 cubic yards of material to be removed. The company is remediating the slip under emergency stabilization guidelines under the well construction rules. |

The material will be allowed to cure for 48 hours before being drilled out and pressure tested. |

1403554636 | 2014-06-23 | Completion Testing | No Violations | On this day I was on site from 9:00 am to 5:00 pm to observed the ongoing well control issue along with my supervisor Rocky King and Inspector Dave Ball, we were in contact with the well owner Mike Hall and discussed a secondary recovery plan of action in the event that well fluid product would escape the location boundaries, 3rd party environmental contractors are being brought in to monitor for any off site impact, I inspected the wet weater stream beds nearest to the well pad along township road 289 and no flowing or standing water was observed, Warren trucking is on site with a crane to disassemble the service rig and blow out preventer, once the equipment is removed Wild Well Control company has a plan of action to remover tubing snubbing unit and blow out preventer along with the damaged well head assembly and replace the well head assembly to gain well control back. Wild Well Control has established proper working perimeters and staging areas and is monitoring the well site gas levels. |

Note: Suggested to well owner that they notify local EMA of the situation and also talk to local Fire Departments and also inquire if that have the capability to spray foam in case of a fire at the site.The well owner would take suggestion into consideration and they also informed me that Wild Well control folks have been contacted and they were in route to well site. |

1403741349 | 2014-06-25 | Completion Testing | No Violations | On this day I visited the site at 9:00 am for approximately 30 minutes and returned at 2:20pm and was on site until 5:00pm to observe the current well control situation, Wild Well Control company is on site and has currently rigged up a 2flowline with no choke to the working pit that has been installed on location to help divert the well product into a controlled environment, they are also still pumping 2.5 barrels per minute of fresh water into the well head to help dissolve the gasses leaking from the Premium Valve company well head, Warren Drilling has a crane on location and is providing lifting and rigging support to disassemble the work over rig off the hole owned by Fortis well service, Due to the recent rain the dry weather defined ditch line that runs to the West of the location and passes under Bethel Township road 289 at a GPS location of EPE 15 -39.67347 X 81.24767 now has water running in it and per request of my supervisor Rocky King chloride tests were ran using both high and low level Quantab strip test and both levels read 640 parts per million. |
1403927085 | 2014-06-27 | Completion Testing | No Violations | On site inspection to witness the well control issue along with Supervisor Brent Bear, On this day Wild Well control company completed the following tasks- the well head was cut off just above the 5.5 casing outlet ports removing the BOP & snubbing unit- the bottom of the B section left remaining was then unbolted and lifted off leaving the lower base assembly remaining attached to the 9.625- a new ring seal was installed and new well head assembly was then set onto the lower half thus regaining well head control at 9:12 pm- just a little over 6 days and 12 hours after the initial call was received, Hall drilling crews are to remain on site until all product is recovered from the site surface, once the product is recovered the pit will be emptied and filled in. |

The water at this site tested at 4.8 (1000 PPM) on a high range Quantab. A second site 3000 southeast of the wellhead was taken from the Clear Fork of the Little Muskingum. The test yielded a .4 (below 32 PPM) on a low range Quantab. A third test site, 900 west of site one, tested at .4 (below 32 PPM) on a low range Quantab as well. |
