0654366213 2014-02-07 Completion Testing No Violations Completion stage #4 being perforated while I was on site with the following observed, a solid completion plug was set at 17,765 and the casing was perforated with 60 shots broke down in 6 intervals of 10 shot bursts from 17,750 to 17,606, 2 wells are being completed simultaneously on this pad.
0856094488 2018-10-04 Production Wells No Violations Status check inspection found (4) wells present on the pad with all wells found to be in production. The lease road surface is stone construction and in good repair. There is a gate across the lease road with owner and well pad information present. The production facility is also along the lease road and not on the well pad. The production facility consists of (4) steel construction tanks capable of holding 400 fluid barrels capacity each. The tanks are situated inside a polymer lined steel containment dike area with hazard warning labels present. There is also an earthen berm around the production facility. Situated on the well pad is a large gas compressor. Addressing the well head the well is equipped with a lower A and upper B section with a production tree present. The production valves are in the open position. 2 diameter riser pipes are present with valves and gauges attached for annular pressure monitoring. The 13.375 well head section showed zero pounds pressure and the 9.625 well head section showed zero pounds pressure. Also noted the South East slope of the well pad does have an earthen slip that is being addressed by the Division Engineering section. The slip has required 10,000 to 12,000 cubic yards of material to be removed. The company is remediating the slip under emergency stabilization guidelines under the well construction rules.
0988666922 2014-02-12 Completion Testing No Violations Status check inspection on the completion process found Wise coil tubing services moving in to recover a lost wire line gun at a depth of 17,225. Direct instructions were given to the company man on site to test the coil tubing Blow out preventer function and pressure test before commencing work over operations.
1243555025 2015-06-18 Preliminary Restoration No Violations Preliminary Restoration follow up inspection found the restoration status is still showing up as active in the database and was signed off by myself on 02/26/2015. I once again have added the well GPS coordinates into the system also. Inspection of the well head found the following equipment present 5.5 production casing with the production valves open and selling into a 2 sales line. A total of 4 -400 barrel capacity tanks are present in a plastic lined metal constructed dike area. Chloride levels were tested at 4 random locations across the well pad with no levels showing up over 121 parts per million on a low level Quantab field strip test. Preliminary Restoration once again passed on this day.
1375109774 2013-07-29 Drill / Deepen / Reopen No Violations The said well was spudded on 07/27/13 and 100 of 24 conductor was set with no annular space. The drilling contractor Zigenfuss will be setting the surface casing also. On this day I took GPS readings of all 4 wells to be drilled on this location and confirmed the permit conditions were being followed.
1375668474 2013-08-04 Casing String Witnessed No Violations On site inspection to witness the cementing of 360 of 20 surface casing. The well lost returns 61 barrels into displacement and the cement supervisor shut the job down at 101 barrels due to high pump pressure. The cement crew was displacing at .4 barrels a minute and did not exceed 120 psi until late into the job. A grout sting will be run and cement will be tagged and grouted. 3 centralizers were ran.
1375970842 2013-08-08 Casing String Witnessed No Violations Inspection to witness the grouting of the surface casing. A 1 tubing string was ran in between the conductor and surface casing strings and the cement top was tagged at 67, the well was then grouted to surface with class A- 15.6#- 2 percent calcium cement by Baker and Hughes cement services.
1381159213 2013-10-07 Drill / Deepen / Reopen No Violations Status check inspection found the current drilling depth to be 6964 using a 12.250 hammer bit and drilling on air. I also spoke with Kyle Baldwin from the Divisions Hydrology department in regards to a fresh water impoundment in the area recently built by Hall Drilling referred to as the Yockey impoundment, Kyle informed me that the impoundment is not on a shale site and we do not regulate it or its construction.
1381191057 2013-10-06 Drill / Deepen / Reopen No Violations On site to inspect site at request of local inspector to check for compliance of storing brine on site, I informed company personnel that all brine storage tanks will need to be placed into containment to help ensure that no brine laden fluid gets into ground water aquifer.
1381357711 2013-10-09 Casing String Witnessed No Violations Status check on the running of the 9.625 2nd intermediate casing string, The 12.250 borehole total depth was drilled to 7826 and 7753 of 9.625 casing was ran and set with a casing shoe and float collar ran along with a 2 stage cement tool at 5148 along with proper centralizer placement. A casing tally was received. I was on site at 5:30 Am and witnessed the last 6 casing joints ran in. The cement crew was on a delayed undetermined arrival.
1387762355 2013-12-22 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling progress found the current drilling depth to be 9119 using a directional drilling assembly and 8.5 bit with the last directional survey being at 9,094 with a True Vertical well depth of 8,735 and a inclination of 63.52 and azimuth of 300.58 degrees putting them near the bottom of the directional curve and horizontal well position. I also discussed with the company man on site and the well owner Mike Hall the need for the contractors to do better on their housekeeping in regards to the oil based drilling mud.
1395085763 2014-03-17 Completion Testing Violations Noted Status check inspection on the completion drill out process found Fortis company service rig on the hole completing the production casing clean out, upon my arrival the company had drilled out the completion plugs and was pulling out of the hole with the last 2 joints of 2.375 tubing with the bottom hole drill out assembly attached when the rig operator pulled the cutting assembly up into the work over stripping rubber causing the well head blow out preventer to lift and begin leaking at the connecting point between the well head top base plate and blow out prevent bottom base plate, the well then began to discharge fluid upon the location surface at a undetermined rate for approximately 1 hour from between the 2 surfaces, my district supervisor Brent Bear was notified of the well head control situation. Once the tubing was removed and the well head shut in and under control directions were given to Jack Strictler the company representative on site to begin clean up by the means of using a vacuum truck to recover the lost fluid, recommendations were also given to ground all tanks on site used for the completion process and move all portable heaters back from the well head a minimum of 50 along with grounding all other equipment and sealing electric cords connections laying on the location surface in fluid. The product lost from the well bore was contained on site and chloride test taken showed 750 parts per million chlorides on low level chloride strip tests.
1521326243 2018-03-13 Production Wells No Violations On this day, I found the well equipped with annular pressure monitoring devices. The surface pressure gauge is reading 300 pounds per square inch, and the 9.625 gauge is reading 250 pounds per square inch. There are (4) wells in production on this pad. All wells have concrete barriers surrounding them. The facility is gated with proper signage. There are (4) 400 barrel steel tanks within containment at the front right-side, prior to the pad, with an electronic shutdown button. There is a midstream facility at the front right-side of the pad, and (4) gas processing units beside it. The wellheads are not properly identified, but it will be noted that I spoke with Antero representative, Jonah Turner, during my inspection on another pad, and informed him to make sure that his operators have their wellheads identified with proper signage.
1556908583 2013-09-30 Drill / Deepen / Reopen No Violations Status check inspection on the running and cementing of 2176 of 13.375 1st intermediate casing string. Notification and records were received and the casing has been set in the well head slips and the BOP rigging and testing company is on site.
1589402130 2015-02-26 Preliminary Restoration No Violations Preliminary Restoration status check inspection found 4 wells total present on this pad at this time with the well head equipped with a production tree with the production valves in the open position selling into a gas processing unit equipped with a mechanical sand trap and USA Compression gas compressor set on the well location with a inline fluid separator and dryer system present. 4- 400 barrel production tanks are present near the well pad entrance supporting the wells on the pad, all located inside a mechanical dike area with proper well identification present at both the well head and tank battery area. Preliminary Restoration passed on this day.
