The tanks are situated inside a polymer lined steel containment dike area with hazard warning labels present. There is also an earthen berm around the production facility. Situated on the well pad is a large gas compressor. Addressing the well head the well is equipped with a lower A and upper B section with a production tree present. The production valves are in the open position. 2 diameter riser pipes are present with valves and gauges attached for annular pressure monitoring. The 13.375 well head section showed zero pounds pressure and the 9.625 well head section showed zero pounds pressure. Also noted the South East slope of the well pad does have an earthen slip that is being addressed by the Division Engineering section. The slip has required 10,000 to 12,000 cubic yards of material to be removed. The company is remediating the slip under emergency stabilization guidelines under the well construction rules. |

1395085763 | 2014-03-17 | Completion Testing | Violations Noted | Status check inspection on the completion drill out process found Fortis company service rig on the hole completing the production casing clean out, upon my arrival the company had drilled out the completion plugs and was pulling out of the hole with the last 2 joints of 2.375 tubing with the bottom hole drill out assembly attached when the rig operator pulled the cutting assembly up into the work over stripping rubber causing the well head blow out preventer to lift and begin leaking at the connecting point between the well head top base plate and blow out prevent bottom base plate, the well then began to discharge fluid upon the location surface at a undetermined rate for approximately 1 hour from between the 2 surfaces, my district supervisor Brent Bear was notified of the well head control situation. Once the tubing was removed and the well head shut in and under control directions were given to Jack Strictler the company representative on site to begin clean up by the means of using a vacuum truck to recover the lost fluid, recommendations were also given to ground all tanks on site used for the completion process and move all portable heaters back from the well head a minimum of 50 along with grounding all other equipment and sealing electric cords connections laying on the location surface in fluid. The product lost from the well bore was contained on site and chloride test taken showed 750 parts per million chlorides on low level chloride strip tests. |
