0119332069 | 2013-10-21 | Casing String Witnessed | No Violations | On 10/20/13 the 9.625 was set and the bottom section section below the DV tool at 4680 was set and cemented, the float did not hold and the cement head was shut in until the cement could cure. The plug did land properly. |
0504589089 | 2013-10-21 | Drill / Deepen / Reopen | No Violations | Accompanied Inspector Bill Stolarik on inspection. Went to site with the intention of witnessing a temperature log that was supposed to take place at 6:00 am. The logging truck was not there. Mr. Audi Miller said that it would be there at 8:00 am. Bill and I returned to site after checking other drilling sites, and the logging truck still wasnt there. We were then informed that it would be there by 12:00 pm. |
0506642754 | 2015-06-18 | Preliminary Restoration | No Violations | Preliminary Restoration follow up inspection found the restoration status is still showing up as active in the database and was signed off by myself on 02/26/2015. I once again have added the well GPS coordinates into the system also. Inspection of the well head found the following equipment present 5.5 production casing with the production valves open and selling into a 2 sales line. A total of 4 -400 barrel capacity tanks are present in a plastic lined metal constructed dike area. Chloride levels were tested at 4 random locations across the well pad with no levels showing up over 121 parts per million on a low level Quantab field strip test. Preliminary Restoration once again passed on this day. Also noted the well API # was superseded by 3411124414-01-00. |
0557706335 | 2014-01-06 | Drill / Deepen / Reopen | No Violations | Status check inspection found Falcon Drilling rig #17 has skidded back on the hole and is preparing to drill out from under the 9.625 casing with a current bit depth of 9093. |
0829274093 | 2014-01-10 | Drill / Deepen / Reopen | No Violations | Status check inspection on the current drilling progress found the current drilling depth to be 10,300 in the 8.5 horizontal well bore section. The horizontal kick off point was 8,029 and the bottom of the curve was 9,290. |
0836883178 | 2018-03-13 | Production Wells | No Violations | On this day, I found the well equipped with annular pressure monitoring devices. The surface pressure gauge is reading 100 pounds per square inch, and the 9.625 gauge is reading 0 pounds per square inch. There are (4) wells in production on this pad. All wells have concrete barriers surrounding them. The facility is gated with proper signage. There are (4) 400 barrel steel tanks within containment at the front right-side, prior to the pad, with an electronic shutdown button. There is a midstream facility at the front right-side of the pad, and (4) gas processing units beside it. The wellheads are not properly identified, but it will be noted that I spoke with Antero representative, Jonah Turner, during my inspection on another pad, and informed him to make sure that his operators have their wellheads identified with proper signage. |
0936179148 | 2015-02-26 | Preliminary Restoration | No Violations | Preliminary Restoration status check inspection found 4 wells total present on this pad at this time with the well head equipped with a production tree with the production valves in the open position selling into a gas processing unit equipped with a mechanical sand trap and USA Compression gas compressor set on the well location with a inline fluid separator and dryer system present. 4- 400 barrel production tanks are present near the well pad entrance supporting the wells on the pad, all located inside a mechanical dike area with proper well identification present at both the well head and tank battery area. Preliminary Restoration passed on this day. |

1304575248 | 2014-03-28 | Production Wells | No Violations | Status check inspection to document that all completion plugs have been drilled out and 8,918 of 2.375 production tubing has been ran into the well bore and the well is now capable of production with 3rd party contractors on site completing the installation of the production equipment. |
1336777605 | 2013-10-17 | Drill / Deepen / Reopen | No Violations | Total depth at the time of inspection was 7741 ft. Spoke with company man Mr. Audie Miller. They were getting close to running their 9.625 in pipe. |
1381870400 | 2013-10-15 | Drill / Deepen / Reopen | No Violations | Status check inspection on the current drilling progress found the company currently drilling the 2nd intermediate bore hole using a 12.250 drill bit with a current depth of 3194. In conversation with the company man on site it was discovered that they intended to drill their vertical test section on this current well, I informed Him they were not correctly permitted and spoke with the Division permitting section to confirm my suspicions. |
1382917485 | 2013-10-27 | Plug / Plug Back | No Violations | Plug back over verticle test section, plug set through the drill pipe with 550 of 2.375 tubing hung underneath. Frontier Drilling was the plug contractor but is not in the database. |
1391650422 | 2014-02-05 | Completion Testing | No Violations | Status check inspection on the completion process found the 1st stage to be completed being pressure tested and perforated on this day with the casing tested to 6,900 psi and the toe sleeve then opened followed by 50 perforations being deployed in 5 intervals between 18,045 and 17,942. |
1392257285 | 2014-02-12 | Completion Testing | No Violations | Status check inspection on the completion process found Wise coil tubing services moving in to drill out a bridge plug set at 4,845 due to the plug setting down and becoming lodged in the casing, the plug was deployed and the perforating gun pulled out of the hole, Direct instructions were given to the company man on site to test the coil tubing Blow out preventer function and pressure test before commencing work over operations. |
1538670803 | 2018-10-04 | Production Wells | No Violations | Status check inspection found (4) wells present on the pad with all wells found to be in production. The lease road surface is stone construction and in good repair. There is a gate across the lease road with owner and well pad information present. The production facility is also along the lease road and not on the well pad. The production facility consists of (4) steel construction tanks capable of holding 400 fluid barrels capacity each. The tanks are situated inside a polymer lined steel containment dike area with hazard warning labels present. There is also an earthen berm around the production facility. Situated on the well pad is a large gas compressor. Addressing the well head the well is equipped with a lower A and upper B section with a production tree present. The production valves are in the open position. 2 diameter riser pipes are present with valves and gauges attached for annular pressure monitoring. The 13.375 well head section showed 80 pounds pressure and the 9.625 well head section showed zero pounds pressure. Also noted the South East slope of the well pad does have an earthen slip that is being addressed by the Division Engineering section. The slip has required 10,000 to 12,000 cubic yards of material to be removed. The company is remediating the slip under emergency stabilization guidelines under the well construction rules. |
1595648646 | 2014-01-21 | Drill / Deepen / Reopen | No Violations | The well bore total drilling depth to be drilled has been reached at 18,165 using a 8.5 bit. The drilling rig is running the production casing and should be cementing in the next 24 hours. No company man was present to get additional information, but called me later in the day. |
1743344788 | 2014-02-06 | Completion Testing | No Violations | Universal well services stimulating the well bore with a slick water fracture and Baker Hughes providing the wire line services with a solid frac plug set at 17,940 and 60 perforations shot in 6 intervals of 10 shots each. |
