0562109049 2018-03-07 Production Wells No Violations Status check inspection found the well is equipped with a casing and tubing well head assembly with (2) manually operated valves on the tubing well head and one emergency shut down valve on the tubing tree production flow line. The well head casing flow line is also tied into the production flow line however the valves are in the closed position. The 13.375 and 9.625 casing well head assemblies have flowlines with valves and gauges installed to monitor annular pressure. The 13.375 casing well head pressure gauge reads 400 pounds pressure and the 9.625 casing well head reads 450 pounds pressure. On pad total is (8) complete wells supported by (8) gas processing units. Also on location is a production fluid facility consisting of a total of (8) steel construction tanks with a capacity of 400 barrels of fluid each, the tanks are situated inside a polymer lined steel containment dike area. The well lease road entrance is gated and locked with signage posted at the entrance of the well pad and at each well head. The well pad has a stone surface along with the lease road. The well pad perimeter berm is of earthen material and in good repair.
0969069852 2019-02-07 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (8) producing wells, (8) gas processing units, (4) 400-barrel steel production tanks within polymer-lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 100 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 300 psi. It will be noted that in 2018 a slip had occurred along the southern slope of the pad and western slope of the lease road, prior to the pad. The slopes have since been graded, seeded, and erosion control in place. Currently awaiting new vegetation.

1385337954 2013-11-24 Casing String Witnessed No Violations On 11/24/13 Patterson Drilling rig 323 is now on the hole and 383 of 18.625 surface casing was set and cemented to surface by Halliburton Cementing services with 100% excess of class A cement ran and 53 barrels of cement returns to surface, No cement wiper plug was ran and the casing shall be pressure tested before drilling out from under the surface pipe. The cement water was tested with no chlorides showing above unacceptable levels. Due to cold weather conditions and the last joint having to circulated in the job took longer than anticipated.

1591927398 2014-08-25 Completion Testing No Violations Status check inspection of the completion process found that during the recent stimulation process a leak was discovered and repaired in the well head and a solid retrievable bridge plug was set in the 5.5 casing at a depth of 7,250 in the horizontal 20 degree curve section of the well bore and the well head consist pressure tested, directly following a attempt to recover the retrievable bridge plug was unsuccessful using the wireline services and coil tubing was brought in and ran in the hole with a fishing overshot and hydraulic jars being used to grab ahold of the lost tools but unsuccessful in recovering the lost tools, a service rig is to be mobilized and set up on the hole and the coil tubing shall be cut off and stick tubing ran in the hole in order to fish out the lost tools and plug.
1631755422 2013-12-10 Drill / Deepen / Reopen No Violations Status check inspection on the drilling progress found the current drilling depth to be 6345 using a 12.250 drill bit with a projected well bore total depth for this intermediate well bore stage of 7337. I was notified that the company was running back in the hole with the drill string after a bit change trip and was approximately 1000 off bottom and attempted to circulate the hole and a down hole restriction caused a overpressure surface condition allowing the discharge line to fail emitting a 2% brine mist, I spoke with my supervisor and due to this being a lit mist discharge only with no reported quantity we are not requiring and surface material to be removed, the company will test the affected area using a 3rd party contractor for unwanted excessive contamination of the affected area.
1698886033 2014-06-19 Completion Testing No Violations Status check inspection found Fortis well service on the hole pulling the work over string out of the hole after completing a toe sleeve clean out run, perforations were shot from 19,261 to 19,263 to allow the company to pump down the wireline tools to begin the simulation process in the next 72 hours.
