1440605524 | 2015-08-25 | Preliminary Restoration | Violations Noted | Preliminary Restoration status check inspection found the following equipment present at the well head-4.5 production casing -1.5 tubing and pumping rods with the production valves in the open position and the well being produced by the means of a pumping unit. At the production fluid storage area on location near the well a single 210 barrel storage tank is present along with a single 100 barrel storage tank and mechanical separator along with a gas sales meter with a 16 day active sales chart showing current gas sales. A single 210 barrel storage tank is also present due to the steep grade to the well, the tank sits at the bottom of the hill along the lease road near the state route. Addressing the restoration, The producer has neglected to remove the waste water and close the pit within 2 months of the date the drilling was completed and has also not completed the site restoration within six months of the date the surface drilling was commenced. Instructions are as follows - Within 14 days -Remove the contaminated water from the pit utilizing proper disposal methods and close the drilling pit in. Complete the earthen restoration. I was requested by Monroe County Inspector Bill Stolarik to inspect this site. No vegetation has been established throughout site where earth has been disturbed. No seed or mulch has been applied. The dirtwork has not been completed to finished grade. Pieces of pit liner are visible in the immediate area of the well. The tank battery on top of the hill is holding water due to recent rain events. It is approximately 1.5-2 ft. from the top. There is no evidence of contamination on the water (ie. an oily sheen). The lease road is very steep. Grade is 1H:1V (45 degree angle) to 1.5H:1V (33.8 degree angle). It has gouges in it 1.5-2 ft. deep. The silt and sediment from those gouges has collected at the bottom of the road and there is evidence that it has entered the creek via channels cut by the runoff from water running off of the lease road. There is a dozer parked at the bottom of the hill where the lease road is and it has collected silt and sediment from the runoff on one side where the lease road crosses the creek. My recommendations are: for the lease road are re-grade and fill gouges in, install waterbars every 15-18 ft. and drain them to a rock-lined ditch constructed on the E side of the lease road using #1 and #2 limestone. For the area on top of the hill, finish grading, drain low lying areas and remove trash and pieces of plastic from pit liner. Establish vegetation on all disturbed areas. |
