0180972574 2019-01-29 Production Wells No Violations On this day, I found Antero progressing with the SimOps procedure in preparation for the plug and abandonment of permit number 4446. TekSolv is onsite installing back-pressure valves in the tubing hanger on the (6) producing wells on the pad, prior to installing the steel cages over the wellheads. They are currently attempting to install the valve on this well, but having issues with the threads on the well. They will not proceed until the well is isolated. Fortis Energy is also onsite installing containment and preparing to rig-up, once the SimOps procedures have been completed. I spoke with Antero Representative Barney Etcher during the inspection.
0211604977 2014-04-09 Drill / Deepen / Reopen No Violations Notification was received on 04/09/014 from the company man on site that during the cementing of the 9.625 2nd intermediate well bore section that the casing pressured up during cement displacement, upon inspection on this day I was notified that the top of the cement inside the 9.625 casing was at 1550 and the estimated top of cement on the annular side was approximately 3,300 and once the company finished drilling out the cement from the inside the casing a bond log shall be ran and the cement job repaired accordingly.
0364117210 2019-01-24 Production Wells No Violations On this day, I am conducting an inspection to document the SIMOPS progress for the plug and abandonment of permit number 4446. I spoke with Antero representative Barney Etcher, and he stated the SIMOPS process is being delayed due creating a standard operating procedure between departments within Antero Resources. Containment is being set in place and steel cages for the producing wells are onsite. The well in question currently has a nightcap in place and no gauges are present. I found the pad gated with signage at the entrance. There are (6) producing wells, (6) gas processing units (GPU), (4) 400-barrel steel production tanks within polymer lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Additionally, concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 70 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 1,150 psi.
0806627959 2018-03-07 Production Wells No Violations Status check inspection accompanied by ODNR Inspector Tom Thomas to document the well status. The well is equipped with a casing and tubing well head assembly with (2) manually operated valves on the tubing well head and one emergency shut down valve on the tubing tree production flow line. The well head casing flow line is also tied into the production flow line however the valves are in the closed position. The 13.375 and 9.625 casing well head assemblies have flowlines with valves and gauges installed to monitor annular pressure. The 13.375 casing well head pressure gauge reads 80 pounds pressure and the 9.625 casing well head reads 1,300 pounds pressure. On pad total is (6) complete wells supported by (6) gas processing units. Also on location is a production fluid facility consisting of a total of (8) steel construction tanks with a capacity of 400 barrels of fluid each, the tanks are situated inside a polymer lined steel containment dike area.

1165376058 2014-04-10 Casing String Witnessed No Violations On 04/08/14 I was notified by the company man that the 9.625 cement job locked up during displacement and they would be drilling out the cement and bond logging to find the top of cement (TOC). On 04/09/ 14 the TOC was tagged at 1,550 and the remainder of the cement began to be drilled out down to the casing float collar. On 04/10/14 the hole was bond logged and a solid bridge plug set in the 9.625 casing at 4,515 and the casing perforated from 4,502 to 4,498 with 16 perforations, circulation was then broke and the hole circulated bottoms up 4 times, a cement retainer was then set and the drill string stung into the retainer and the casing cement squeeze job applied by Allied Cement services using 1594 sacks of Class A cement with a 15.6# weight and 1.8 yield with no additives and 50% cement excess ran with no cement returns to surface. The cement retainer and bridge plug shall be drilled out and the well bond logged.
1294998221 2015-07-30 Work Over Reconditioning No Violations Status check inspection on the well workover operation found Fortis Service Companies workover rig on location running the 2.875 production tubing back into the well bore to a proposed total depth of 7,786. The rig has completed a clean out run of the 5.5 production casing down to the total depth with Cudd Well Service providing the workover well head control at surface and ROC company providing the fluids pressure flow control. The well control was maintained using 9.8 pound per gallon brine.
1390841073 2014-01-27 Drill / Deepen / Reopen No Violations Status check inspection on the drilling progress found the current well 24 bore surface hole has been drilled to a depth of 418, the company is running a borehole deviation survey and will then condition the hole in preparation to run the 18.625 surface casing.
1391050869 2014-01-28 Casing String Witnessed No Violations On 01/28/14 Falcon Drilling rig # 20 is now on the hole and 418 of 18.625 surface casing was set and cemented to surface using 850 sacks of class A 2% calcium 1/4 pps flake with cement returns to surface @ 70 barrels into total displacement, the cement weight was verified at 15.6# and the mix water checked for chlorides, the total job on site time was 10 hours due to the -20 degree temperatures.
1391525554 2014-02-04 Casing String Witnessed No Violations On 02/04/14 the 1st intermediate casing string was set and cemented to surface by Allied Cement services using 1841 sacks of class A- 2% calcium-1/4# flake cement with 80 barrels of cement returns to surface and the plug landed at 890 psi followed by the casing being pressure tested to 1410 psi with no recorded pressure loss.
1396551055 2014-04-03 Drill / Deepen / Reopen No Violations Status check inspection of the drilling process found Patterson Drilling rig 342 drilling the 2nd intermediate 12.250 borehole on air at a depth of 4,227 with a projected total depth of 6,900.
1397266353 2014-04-11 Casing String Witnessed No Violations Notification was received from the company rig supervisor James Harvey that the 9.625 remedial squeeze job that we initiated the previous night was being bond logged and a top of cement was indicated at 2,400 with several cement stringers up the hole into the bottom of the 13.375 previously set casing string at 2,100. We will meet on location and review all records and logs on the next working day, the drilling rig will skid to drill the 2nd intermediate on the next well on the pad. Notification was made to my supervisor Brent Bear and further remediation work will not be required.

A proper functioning work over blow out preventer is in place and has been function tested and charted.

There a quantity of 8- 400 barrel production tanks on location supporting the 6 producing wells now located on the well pad. Preliminary Restoration was passed on this day.

The company has recently added additional earthen containment around the well pad perimeter. Addressing the well status the well drilling and completion process is complete with the well in production with additional gas processing equipment located on the well pad in a separate location from the producing wells, there is also a total of 8-400 barrel capacity production fluid tanks on location located inside a plastic lined metal dike area with proper well identification posted at the well head.

The well is equipped with annular pressure monitoring devices. Concrete barriers surround the wellheads. The facility is gated with an electronic shutdown button on the right-hand side, prior to the pad. The front left of the pad is equipped with (8) 400 barrel steel tanks within containment and an electronic shutdown button. There are (6) gas processing units along the front right side of the pad, and a midstream facility along the front right side, as well.
