Historically the well bore was lost and the well plugged and abandoned, however the 13.375 casing well head section was building 140 psi pressure. I shall speak with Anteros drilling supervisor James Harvey and ask them to formulate a plan to subside the pressure and finish the well plugging and abandonment process. There is 6 producing wells on the pad.

0725783792 2019-01-31 Plug / Plug Back No Violations Status check inspection on the re plug process accompanied by ODNR Inspector Ryan Armstrong. A service rig is rigging up on the well owned by Fortis Energy. A 5,000 pound blow out preventer will be installed on the well head before the rig goes in the hole with a 8.625 tri cone drill bit and 3 to 6 2.875 drill collars to drill out the top abandonment plug set inside the 9.625 casing, the blow out preventer will consist of a set of blind rams and set of shear rams and a annular circulating head with a flow cross present in the stack. On location being set up is (3) 500 barrel horizontal tanks, the tanks will be situated inside polymer containment. The rig is set up on polymer containment. The other 5 existing wells on location do have 2 way check valves set in the tubing hangers and VR plugs set in the flow cross sections, cages are present over each existing well. The rig is scheduled to commence drill out tomorrow morning 02/01/2019.

1389420911 2014-01-11 Casing String Witnessed No Violations On 01/11/14 Falcon Drilling rig 20 is now on the hole and 377 of 18.625 surface casing has been set and cemented to surface by Allied cement services with Class A-2% calcium-1/4# flake- 100% excess of cement ran with circulation established prior to cementing and no cement returns to surface along with the casing being pressure tested upon the landing of the plug with no recorded pressure loss. The well was pressure grouted with 20 barrels of cement from surface.

1402000618 2014-06-05 Plug / Plug Back No Violations On 05/19/14 the 8.5 production borehole was cemented through the drill string in place at a depth 13,673 with the drill string and down hole assembly left in the borehole. On 05/20/14 Blue Dot pipe recovery services ran a free point tool indicating the top of cement on the annular side of the drill string at 4,500- the drill string was then backed off at 4,250 and a 400 plug set by Allied cement services with the top of the plug tagged at 3852- a 300 surface plug was then set. On 05/30/14 during a random site inspection it was reported that there is annular gas migration between the 13.375 & 9.625 casing strings with the company reporting they are going to monitor the situation and discuss the situation with the Divisions management.
1437499001 2015-07-21 Plug / Plug Back No Violations Status check follow up inspection on the well bore annular pressure issue present between the 13.375 and 9.625 casing strings. On this day the well was found to have 140 psi present measured by a mechanical gauge. The pressur was blown down through a gauge port valve in a time frame of 15 seconds and allowed to buid back up taking 10 minutes to reach 140 psi. A third party contractor know as Tripple J pressure testing was on site and applied pressure wsing freshwater to the annular side of the 9.625 to the anount of up to and not exceeding 1,500 psi with the capaciy not accepting a volume of more than 3 gallons total and no rete could be established. At this time my recommendation was to leave the surface control equipment in place on the well until the Division Management could access the situation, Antero Company Drilling Supervisors Joe Honeycutt and James Harvey were on location to witness the testing.
1549381994 2019-02-05 Plug / Plug Back No Violations Status check inspection found a service rig owned by Fortis Energy on site attempting to drill out the previously set surface plug. The rig has pulled the workover string out of the hoe due to a plugged string float. The total depth achieved is 240. The intention of the work is to drill out the surface plug and check the pressure of the well bore below it and log the 9.625 casing string.
1549473191 2019-02-06 Plug / Plug Back No Violations Status check inspection on the well re plugging and evaluation process with a derrick type service rig on the hole owned by Fortis Energy. The rig has drilled out the cement surface plug set inside the 9.625 casing during June of 2014. The well was found after that to be building pressure between the 9.625 and 13.375 diameter casing strings. The rig has drilled out the surface plug set inside the 9.625 with the plug bottom being at 323 . The rig ran the workover string into the well bore to a measured depth of 3,783 and at that point sat down on the top of the next plug previously set in the 9.625 casing. The well bore was circulated clean with 150 barrels of oil based drilling mud circuited off the top of the plug at 3,783. There was zero pressure in the well bore below the surface plug. The rig is preparing to run a casing scraper on the 9.625 casing before bond logging the 9.625 diameter casing. Operations are currently halted while the (3) 500 barrel workover fluid tanks are cleaned.
1549813847 2019-02-10 Plug / Plug Back No Violations Status check inspection on the well re plugging process found crews from a service company identified as New Park on location and digging out the material from inside the tin horn cellar so the well head can be cut off and removed. A service rig is on location owned by Fortis Energy and recently attempted to pull the 9.625 casing up out of the well head, casing jacks were also used with a total of 400,000 pound upward force applied to the 9.625 with no results in lifting the casing. A radial bond log was ran with the top of cement on the 9.625 showed at 2,064.

1549910556 2019-02-11 Plug / Plug Back No Violations On site inspection to witness the well re plugging process accompanied by ODNR Inspector Tom Thomas. A service rig owned by Fortis Energy is rigged up on well. The company has attempted to pull the 9.625 casing up out of the well head slips unsuccessfully with a total reported pulling force of 400,000 by the use of casing jacks. The company has removed the well head and then once again unsuccessfully attempted to pull up on the 9.625 diameter casing. I spoke with ODNR Supervisor Mike Moreland and then called Antero Completion Representative Tim Neil. It was discussed between both parties the options of stopping the migrating pressure between the 9.625 and 13.375 diameter casing strings. The company has supplied the Division with a Bond Log of the 9.625 casing string and been informed that they have 2 options in stopping the migrating pressure by the means of either perforating and squeezing the 9.625 annular space or cutting segments of the 9.625 diameter casing out. The company has submitted a proposed plan and will re visit the plan and re submit it. KLX Energy is re installing the workover blow out preventer.
1549994712 2019-02-12 Plug / Plug Back No Violations On this day I was on site to witness the perforating of the 9.625 diameter casing string and the setting of a cement retainer in attempt to squeeze off the 9.25 x 13.375 annular space to stop migrating gas pressure. A service rig is on the hole owned by Fortis Energy Nine Energys Wireline Service is on location and has perforated the said well with (2) clusters of 10 shots between the depths of 1,895 to 1,898 and 1,915 and 1,918. The well was attempted to be circulated unsuccessfully through the bottom set of perforations. A cement retainer will be set at 1,910 And perforations shot above the retainer.
1550068961 2019-02-13 Plug / Plug Back No Violations On site inspection accompanied by ODNR Inspector Tom Thomas. Service providers on location include Formation Cementing along with a service rig owned by Fortis Energy. On the previous day(02/12/2019) 2 series of perforations were shot in the 9.625 casing with the lower set of perforations between the depths of 1,918 and 1,915. Than a upper set of perforations were shot between the depths of 1,898 and 1,895 a cement retainer was set at 1,910. A 2.875 work over string was then ran into the well bore and stung into the cement retainer and circulation established. On this day 02/13/2019 The service rig company Fortis Energy pumped hole capacity before turning the job over to Formation Cementing who pumped a Class A Neat Cement plug through the 2.875 diameter workover string into the annular space between the 9.625 and 13.375 casing strings. A 65 sack 13.6 barrel volume plug was pumped and displaced with 9.5 barrels of fresh water. The rig then pulled 4 tubing joints out of the hole and reverse circulated the well. The operation was then shut down for the day to allow the cement to cure and to monitor annular pressure.
1550150090 2019-02-14 Plug / Plug Back No Violations On site inspection follow up to witness the well re plugging process along with ODNR Inspector Tom Thomas. A service rig owned by Fortis Energy is rigged up on the well. Upon arrival we checked the well head Blow Out Preventer flow cross pressure to confirm the isolation squeeze plug set the previous day stopped the annular gas migration between the 9.625 and 13.375 casing strings. The well head blow out preventer was shut in overnight and the flow cross confirmed no pressure by gauge, following the conformation by gauge we opened the flow cross to the atmosphere and check for any pressure migration by the use of installing a plastic bag on the flow cross outlet. No pressure movement was observed and the well was observed while the rig was pulling the 2.875 workover string. Before the rig pulled the workover string the top of cement on the plug set the previous day was tagged at 1,756 indicating 159 of cement fill up over the top perforation in the bottom set of perforations below the cement retainer. Once cement top was confirmed the rig pulled the workover string and removed the Blow Out Preventer stack in order to visually confirm the annular between the 9.625 and 13.375 had no migrating pressure. Once the visual check was complete the tubing was ran back in to 210 and a final surface plug re- set in the 9.625 casing. The final surface plug was set by Formation Cementing and consisted of 80 sacks of class A cement with cement to surface. The 9.625 was reported to attempt to be pulled up out of the casing slips during the workover process on two different occasions. The casing was pulled to 400,000 pounds which was 320,000 over string weight of the 9.625 diameter 40 pound per foot set at 2,000. The company reported the casing would not pull stretch.
1567487218 2014-04-28 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling progress found the drilling rig Patterson 342 has completed drilling the 12.250 2nd intermediate well bore stage and the 9.625 casing has been run to a depth 7,123 with a guide shoe and casing float collar ran along with proper casing centralizer placement demonstrated, Halliburton cement services were on location beginning the cement job.
1760118583 2014-05-21 Plug / Plug Back No Violations On site to set 400 plug on top of drill pipe which was backed off@freepoint(4252) Allied cementing pumped a 149 sack plug with a weight of 16 ppg Type H cement and displaced it with 61 bbl of displacement fluid.

Note: Tagged plug@3850 rig crew then pulled drill pipe up to 300 and then set top plug once cement top was confirmed rig crew started to nipple down BOP stack and once removed welder will place required steel plate on top of casing once it is cut down 30 below ground level.

1770486625 2019-01-29 Plug / Plug Back No Violations On this day, I found Antero progressing with the SimOps procedure in preparation for the plug and abandonment of this well (permit number 4446). TekSolv is onsite installing back-pressure valves in the tubing hanger on the (6) producing wells on the pad, prior to installing the steel cages over the wellheads. Fortis Energy is also onsite installing containment and preparing to rig-up, once the SimOps procedures have been completed. I spoke with Antero Representative Barney Etcher during the inspection.
1822533357 2014-05-15 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling progress found the well control blow out preventer still in the closed position with the drill string still on bottom at 13,673 with the company working to increase the mud weight from its current 16.6# weight to 16.9# with a recent attempt to open the well control choke valve showing an increase in fluid volume and pressure from under 1000 gas unit to up over 4000 gas units, the choke valves were reduced and the well allowed to balance back out.

1839678806 2014-05-17 Plug / Plug Back No Violations Notification was received from John Mcevers at 2:45 pm today that the well gas influx has been controlled with 17.1 pound mud to a point the blow out preventer has been opened and the casing string has been attempted to be pulled unsucessfully with the drill string still sitting 20 off bottom at 13,673, they are proposing cementing the drill string in place through the directional drilling down hole assembly to a point of 250 up inside the 9.625 casing string and setting a 9.625 bridge plug on top of the cement. Notification was made to Deputy Chief Bob Worstall and District supervisor Brent Bear with the understanding a pre job proposal will be submitted before the cementing takes place and inspectors will be notified and on site to witness the job.
1980716696 2019-01-24 Plug / Plug Back No Violations On this day, I am conducting an inspection to document the SIMOPS progress for the plug and abandonment of the well. I spoke with Antero representative Barney Etcher, and he stated the SIMOPS process is being delayed due creating a standard operating procedure between departments within Antero Resources. Containment is being set in place and steel cages for the producing wells are onsite. The well in question currently has a nightcap in place and no gauges are present. Mr. Etcher also stated that Fortis Energy will be conducting the plugging operations.
1982584898 2019-02-04 Plug / Plug Back No Violations Status check inspection on the well re plugging process found a service rig on the hole owned by Fortis Energy drilling out the top plug previously set inside the 9.625 casing. The current drilling depth is 240 with a 8.625 tri cone drill bit. The rig will drill this drill collar down and clean the hole up for the evening and commence operations the following morning. They company man on site and myself discussed drilling this top plug out and running a casing scraper on the inside of the 9.625 casing prior to bond logging he casing.
2082223987 2014-05-12 Drill / Deepen / Reopen No Violations Status check inspection on the drilling progress found the current drilling depth to be 13,700 in the 8.5 horizontal well bore section with a directional azimuth of 151.90 degrees. Due to a down hole high influx of gas pressure the well head blow out preventer was closed on 05/10/2014 at 1:30pm, well bore circulation was maintained and the mud weight was increased to 15.6# on the inlet with a decrease down to 15.4# on the outlet and a total fluid gain of 100 barrels overall, at the time of my inspection the gas has subsided to 4,038 units and the company was increasing the mud weight to 15.7# once well control is totally established they will see if the drill string is free.
