1348699837 2019-01-14 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (8) producing wells, (8) gas processing units within steel containment, (8) 400-barrel steel production tanks within polymer lined steel containment, and an Emergency Shutdown (ESD) button is present. Additionally, concrete Jersey barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 30 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 20 psi.

1399296999 2014-05-05 Casing String Witnessed No Violations Status check inspection on the cementing of the production casing, Allied Cement services is on location cementing the 16,404 of 5.5 23# casing with a float shoe and collar ran along with a trigger toe release sub and proper centralizer placement demonstrated, proper bore hole gas migration control was gained prior to cementing by increasing the drilling mud weight to just under 15 # and conditioning the bore hole with it thus bringing the 3rd party gas down from 4,900 units to under 1,500 units.
1399424922 2014-05-06 Casing String Witnessed No Violations Notification was received on this day at 9:48 am from the rig supervisor James Harvey that the production casing cement job that took place on 05/04/2014 had encountered problems during displacement and the cement circulation locked up during the displacement stage of the process with 70 barrels left to displace out of 257 barrels total, a bond log was ran and a top of cement was logged at 7,315 placing the top of cement 135 above the 9.625 casing shoe with the horizontal well bore kick off point 7,550 and total depth of the well bore at 16,407. Discussion was made with both Division Deputy Chiefs and my direct Supervisors and notification was made back to Mr. Harvey that they needed 1000 of cement above the kick off point as described in ORC 1501:9-1-08 (7).
1438783698 2015-08-05 Preliminary Restoration No Violations Preliminary restoration status check inspection finds the affected area limits of disturbance indicated on the pre-site plans has been properly restored under ORC 1509.072 with adequate vegetation - erosion & sedimentation control in place. Addressing the well pad working area there is 3 completed wells in production on location with a Patterson Drilling rig 343 on location drilling additional wells on the pad with a primary earthen containment berm in place to control the surface fluids by the use of perimeter drainage sumps and valves with the drilling rig and support equipment being all placed on secondary polymer containment. Located on the North side of the well pad is the production fluid storage tank battery with a total of 8-300 barrel tanks located in a poly lined metal containment dike area. On the Southern side of the well pad the gas processing and monitoring units are present. Preliminary Restoration passed on this day.
1448300704 2015-11-23 Production Wells No Violations Production well status check inspection found the well is now equipped with a production well head and is on line and producing through a gas processing unit. There is a total of 8-400 barrel capacity production tanks on location located inside a polymer lined steel containment dike. 8 well total are present on this well pad (Monroe North).
2104180292 2014-01-27 Drill / Deepen / Reopen No Violations The said well was spudded on 01/24/14 by Rocky Mountain Drilling with 100 of 26 weld joint 154.5# pipe being cemented to surface on 01/25/14 by Allied Cement services using 336 sacks of class A 2% calcium 1/4# flake cement with 10 barrels of cement returns to surface and the bottom of the tin horn sealed at the same time.
