Universal Cement Services cemented the casing using 670 sacks of Class A 3% calcium .25 pps flake 100% open hole excess with circulation established prior to cementing. Due to a wooden wiper plug being ran the casing was not pressure tested upon the landing of the plug and shall be pressure tested once the cement sets up.

0297125461 2018-10-10 Drill / Deepen / Reopen No Violations On this day, I am conducting an inspection to follow-up on the current SIMOPS procedure. I spoke with EdgeMarc Production Superintendent Mark Toplovich, and he indicated that he met with ODNR Inspector Ken Frye on 10/09/2018. The well is currently isolated downhole, the production wellhead has been removed, and a steel cage and concrete barriers surround the well, protecting it from any potential incidents. It will be noted that Mr. Toplovich indicated that a back-pressure valve has been installed, instead of a 2-way check valve, which was approved through ODNR Inspector Ken Frye.

The location has master emergency shut down buttons in place along with a 8 chain link per miter site security fence. Addressing the restoration- The area indicated on the pre site plans has proper vegetation established and erosion control in place.

1413746666 2014-10-19 Casing String Witnessed No Violations Status check inspection found HAD Drilling rig 690 on location drilling the mouse hole for the drilling rig at a depth of 33 using 16 casing. The rig is a double rotary and drills the casing in with a cutting shoe on the bottom of the casing. At a depth of 33 the 16 casing which is 20 length weld connection at the seams parted leaving the 1st joint in the earth with the top of the casing 16 from surface, the company is going to attempt to fish the lost casing joint out and pull it back up to surface. The conductor has not been set for this well.
1415872680 2014-11-12 Casing String Witnessed No Violations On this day I arrived on site at 16:00 pm for a surface casing cement job call out, upon arrival I found H&P Drilling rig 521 on location and the 24 surface borehole drilled to a depth of 430. The company was having difficulty in getting the Buttress thread casing connections to thread together and after 2 hrs I left location and will stand by for a call out once the casing string is on bottom and they are ready to cement.

1418078424 2014-12-08 Plug / Plug Back No Violations Inspection to document Vertical strata test bottom hole kick plug. 8.5TD was @ 9,850 with 2 plugs set on 12/05/2014 through the drill string-1st plug @ 9,816 to 9,027 drill string pulled up to 9,027 and second plug set 9,027 to 8,367. Drill string pulled up and hole circulated clean. Drill string pulled out and company then ran in and tagged cement@ 8,367 and dressed kick plug off down to 8,600. Company then pulled out of hole with dress bit and picked up directional tools to begin drilling horizontal well bore section.

1430265053 2015-04-28 Completion Testing No Violations Status check inspection on the well stimulation process found Weathorford Completion Service on location stimulating the 13 of 32 stages to be completed. Upon my inspection the company was just starting the 13 stage with an initial breakdown pressure of 9,500 psi with Rush wireline service on location and the stage perforated between 13,082 and 12,991 with 4 clusters of 12 shots shooting 6 shots per foot, the total water volume for this stage shall be and estimated 8,500 bbl of H2O and 450,000 lbs. of sand. An assessment was done of the location to find the well head Blow out preventer accumulator placed beside the completion control trailer and the entire stimulation support equipment to be on containment. The well pad secondary containment is in the closed position.
1434544275 2015-06-16 Production Wells No Violations Preliminary Restoration status check inspection found the well location has had vegetation established and erosion control in place. Addressing the status of the well, The well drilling and stimulation process has been completed the well is shut in with no production lines hooked up or present and no production equipment present on location. A call was placed to Edgemarks main office for a status update and to establish a point of contact.

Annular pressure monitoring devices are installed with 800 pounds per square inch on the 9.625 inch diameter casing, and 560 pounds per square inch on the 13.375 inch diameter casing. The company will be notified of the pressures.

2019113199 2019-05-24 Production Wells No Violations On this day, I found the pad fenced and gated with signage at the entrance. An earthen berm surrounds the perimeter of the pad. There are (3) producing wells, (3) gas processing units (GPU), and (3) 500-barrel and (1) 100-barrel steel production tanks within plastic lined steel containment. An Emergency Shutdown (ESD) button is present at the production facility, wellheads, and entrance. Additionally, concrete barriers surround the perimeter of the wellheads. Identification and annular pressure monitoring devices are present at the wellheads. The 9.625 inch diameter intermediate casing has a pressure of 1,100 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 300 psi. On 5/24/2019, I contacted EdgeMarc Production Superintendent Mark Toplovich to notify him of the pressures.
