0195307980 2019-04-10 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (4) producing wells equipped with a plunger lift system, (4) gas processing units, (4) 400-barrel steel production tanks within steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 450 pounds per square inch (psi). It will be noted that a riser pipe and gauge are not present for the 13.375 inch diameter casing string. Records indicate that the current casing string does not include the 13.375 inch diameter casing, as the original permit proposed.
0335116675 2014-09-22 Drill / Deepen / Reopen No Violations Status check in section on the drilling progress found Nomac Drilling rig 311 on the hole and the 8.750 production bore hole total drilling depth reached at 16,930 with a directional azimuth of 150 degrees and true vertical well depth of 9,582.
0379062851 2015-05-11 Preliminary Restoration No Violations Preliminary Restoration Status check inspection and well completion process update found a total of 4 wells have now been drilled on the Campbell well pad with all wells secured and awaiting a service rig to complete the toe prep clean out post-stimulation run. Upon completion the stimulation process will take place. The company has installed all well production flow lines and a tank battery has been constructed on location with a total of 4-400 barrel production tanks present. Upon review of the well pad perimeter it was observed that vegetation is established and no erosion issues noted as observed. The company has personnel in place monitoring any water that accumulates on location before discharging any fluid off site. Preliminary Restoration passed on this day.
0488273036 2014-08-25 Drill / Deepen / Reopen No Violations Status check inspection found Nomac Drilling rig 311 on location drilling the 12.250 1st intermediate well bore at a depth of 2,316 with a projected total depth to be drilled of 2,332.
0499852670 2014-08-22 Drill / Deepen / Reopen No Violations Status check inspection found the current drilling depth to be 1,828 in the 17.5 1st intermediate well bore section with a estimated total depth to be drilled of 2,332, the company has switched from air drilling to fluid drilling with 9.9# KCL due to hole conditions.

0703306291 2014-09-11 Drill / Deepen / Reopen No Violations Status check inspection of the drilling process found Nomac Rig 311 on location drilling the 8.750 production borehole at a depth of 9,677 in the horizontal curve section with a directional azimuth of 210.84 degrees using 10.70 # drilling mud.
0860603414 2015-09-02 Completion Testing No Violations Status check inspection on the well completion process found the well stimulation process is now complete, the well also has 2.875 production tubing set and is now on line and producing, with a company know as Pro-flow testing services on location, the company is currently on day 6 of 12 proposed days the well will be flow tested, the well is on line and selling gas.
1190451671 2014-09-24 Drill / Deepen / Reopen No Violations Status check inspection found the well drilling process complete with a total drilling depth reached of 16,930 and the 5.5 production casing has been ran and cemented to a depth of 16,833 with a directional azimuth of 150 degrees. The drilling rig is laying down the drill sting so the rig may be skidded. All records have been received and submitted.
1265786054 2014-09-15 Drill / Deepen / Reopen No Violations Status check inspection on the drilling progress found Nomac Drilling rig 311 on location drilling the 8.5 horizontal well bore section at a current horizontal depth of 13,302 using 12.5# drilling mud with 4,500 units of 3rd party gas encountered, the rig is currently pulling the drill string out of the hole at 6,141 to change out the down hole assembly & drill bit.
1279248515 2014-07-21 Drill / Deepen / Reopen No Violations Status check inspection found Zigenfoose drilling on location drilling the 24 diameter surface borehole to be drilled to 320.
1355136510 2015-07-22 Completion Testing No Violations Status check inspection on the well drilling process found the well drilling and stimulation process now complete with 4 wells on location awaiting the workover rig to drill out the completion plugs and run the production tubing. All completion work is being conducted on 2 levels of containment. The company has contractors on location finishing the installation of the 4 gas processing units and well production support equipment.
