0500753305 2015-05-11 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the following present on location- Stingray Pressure pumping services are on site along with Nine Energy wireline implementing the completion process on all 4 wells on location that have been drilled. All stimulation support equipment is on containment with the well pad berm serving as secondary containment with all site valves in the closed position during stimulation. The company does have personnel in place monitoring and testing any fluid that may accumulate before discharging takes place. Addressing the Preliminary Restoration the well pad construction phase is complete with adequate vegetation and erosion control being established in the affected areas, the well pad working surface shall remain as stone. 4-400 barrel production fluid storage tanks are present on location and all production flow lines are in place with some fine pluming connection to take place during the final completion phase. Preliminary Restoration passed on this day.

1211757048 2014-09-22 Drill / Deepen / Reopen No Violations Status check inspection on the well drilling process found Sidewinder rig 209 on location drilling the 8.5 diameter production borehole at a depth of 9,775 in the horizontal curve section at 67 degrees using 11.5# drilling mud with a true vertical well bore depth of 9,207 and directional azimuth of 158.83 degrees.
1410478941 2014-09-11 Casing String Witnessed No Violations On site inspection to witness the 13.375 x 355 surface casing running and cementing with Sidewinder rig 209 on the hole and CalFrac cement services cementing the casing using 457 sacks of class A 3% calcium .25 pound per sack Cello flake with 50% calculated open hole cement excess ran and 18 barrels of cement and 40 barrels of spacer returns to surface and the casing pressure tested to 700 psi upon the landing of the plug. It was also discovered on this day in the 2 site catch basins a bronze tinted fluid product that had the appearance other than freshwater. After evaluation of the product it was determined to have the characteristics of a Base Oil K50H used in the fluid drilling section of the well bore, the product appeared to be in the raw form and no signs of where the product was spilled on the drill site was discovered. Gulfport Drilling Supervisor Ricky Keith along with rig manager Lennie Cook and Drilling HSE Coordinator Willard Champagne were on site and agreed with the determination that this was possibly Base Oil K50H. Kris Anderson with ALL Consulting spoke with me on the phone and all parties involved were in agreement to remove the product out of the catch basing by the means of a vacuum truck and continue to monitor the situation and repeat the process as needed, hydro flushing will be administered at a later time if deemed necessary. 3 sets of samples were take 1 from each catch basin numbered SA-13-WJS & SA-14-WJS, along with a background sample of the product Base Oil K50H identified as sample SA-15-WJS, MSDS sheets of the product were received from Gulfport on this day.
1435107590 2015-06-23 Completion Testing No Violations Status check inspection of the well completion process found Mercer Well Service rig on location and has recently completed the post drilling well bore clean out run with 2.875 production tubing set to a depth of 9,141. Pro FlowCompany flow back was on site to provide well flow back operations and testing. The production well head equipment is now in place and the crews are mopping up and preparing to move over to the McLaughlin well pad.
1592091545 2014-06-18 Casing String Witnessed No Violations On the day of my inspection I witnessed Rocky Mountain Drilling Inc. drilled a 32 inch diameter hole to the depth of 110 feet and ran 110 feet of 26 inch 102.63#/ft. conductor pipe. Stingray Cementing was on site and gained circulation with 30bbls of fresh water. Stingray pumped 15bbls of gel, 10bbls of fresh water and cemented pipe with 273 sacks of class A cement with 2% CaCl and .25#/sack of Celluseal. Displaced cement with 65bbls of fresh water had good cement returns of 25bbls and cement stayed surface.
1594850269 2014-06-30 Casing String Witnessed No Violations The said well was spudded on 06/29/2014 with 112 of 26 conductor being set inside a 110 x 32 borehole on 06/30/14 with Stingray cement services cementing the casing using 272 sacks of class a 2% calcium .25 pound per sack flake 15.6# 1.21 yield cement with 50% cement excess ran.
1710996593 2017-02-13 Production Wells No Violations Status check inspection found the well in production and selling off the 2.875 production tubing tree. The well head did have annular monitoring gauges in place with zero pressure on the 13.375 and 500 psi on the 9.625 annular. The well is producing into a gas processing unit and selling into a main sales line. There is a tank farm consisting of a total of 4 tanks with the capacity of 400 barrels each with the tanks located inside a polymer lined steel containment dike area. Proper well identification is posted at the well heads and location entry point. The company has also installed emergency shut down devices at each well head along with at each gas processing unit and at the location entry point and production tank farm.
1724146240 2019-04-10 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (4) producing wells equipped with a plunger lift system, (1) gas processing unit, (4) 400-barrel steel production tanks within steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 600 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 0 psi. The production records are current. I spoke with Company Representative Steve Habig during the inspection.
