On this day the well was observed to be production off the production tree with (2) manual production valves present along with a single (ESD) Emergency Shut Down valve. The ESD has remote shut down buttons located at the well pad entrance along with the production facility and the gas processing units. The well is equipped with steel lines on the 13.375 and 9.625 casing well heads with valves and gauges present to monitor annular pressures. The 13.375 well head section was observed to have 0 pounds pressure on it at surface and the 9.625 section was observed to have 1,500 pounds on it at surface. In conversation with Anteros Production Supervisor Doug Stebbins all annular pressures are being monitored and evaluated. The well pad has an earthen berm established. The production fluid facility is located on a adjoining site pad and consists of (8) steel construction tanks of 400 barrels fluid capacity each. The tanks are situated inside a polymer lined steel containment dike area. |

On this day it was reported at 10:26 am by Sam Myers that they has spilled approximately 500 gallons of diesel fuel on location and into a pond. Inspector Eric Conrad responded initially and I arrived on location at approximately 5:45 pm. Upon arrival it was observed and reported that a majority of the product has been cleaned up and removed from the main water. I was informed of the companys intention to burn the remaining fuel from the pond area. At that point I was also informed by Aaron Wolfe with the Ohio EPA that the pond was outside the Limits of Disturbance of the well pad and he had authorized the burning. I at that point asked Sam Myers with Antero and Aaron Wolfe with the EPA to please let me run it through the Divisions Management before any fires were lit. I did question this practice for both safety reasons and I did not want a fire lit in close proximity to a active shale pad without my management being aware of the situation first. I proceeded to a area off site to gain phone service and all parties processed to move foreword without respecting my wishes or authority. ODNR Deputy Chief Dave Claus and Regional Supervisor Brent Bear were made aware of the situation. |

The company has completed the thermal degradation activities around the pond located outside of the permitted limits of disturbance, as agreed upon by OEPA, Monroe County EMA, Antero, Patterson Drilling, EMS, and myself in the incident command meeting on 3-15-2015, EMS is utilizing vacuum trucks to skim the remaining sheen off of the pond. Myers Well Service is removing the contaminated soil at the direction of Hulls Environmental. At the time of inspection over 780 yards of soil and stone have been removed from the site and hauled to an approved landfill. At the last inspection I requested additional engineering controls including a secondary containment to fill the fuel tank that was overfilled, those controls are now in place. |

The producing wells on the pad (permit numbers 4489, 4490, 4492, 4494) are being prepped per Anteros SIMOPS plan that was submitted and reviewed by the ODNR DOGRM, before Stalnaker Energy rigs-up for the plug and abandonment of permit numbers 4491 and 4493. TekSolv is currently onsite setting the back pressure valve. The producing wells have been blown down, are having back pressure valves installed in the tubing hangers, steel cages placed over the wellheads, LEL monitors in place, and concrete Jersey barriers spotted around the perimeter of the wellheads. It will be noted that a 2-way check valve will not be placed in the tubing, as the SIMOPS plan originally stated. I spoke with Jayson Lukes and ODNR Inspector Ken Frye about the amendment to the plan. |

Then bumped the plug and took the pressure up to 500 psi over the lift pressurand held for 5 minutes. The casing test passed. |

Upon arrival I witnessed Myers Trucking and Antero employees installing curtain booms and oil soak booms in a farm pond adjacent to the site. After investigating the scene it was determined that the source of the release was a Patterson employee who walked away while filling the fuel tank allowing it to overflow. Measurements of the primary fuel tank indicate that a total volume of 603 gallons of diesel fuel were released. The spill was responded to by five Antero personnel, fourteen Myers personnel with eight water trucks and four EMS personnel. No diesel or sheen was found below the pond. Remove all contaminated soil and filter sock and haul to an approved landfill. Replace all soil removed, as well as, erosion and sediment controls. |
