0206489509 2014-09-19 Casing String Witnessed No Violations Status check inspection found Rocky Mountain drillings top hole rig has drilled a 32 x 80 conductor borehole and set 80 of 26 conductor in place with a variance granted by the Division to set 80 of conductor due to being in solid limestone bedrock at 60 freshwater was also encountered at 27. Allied cement services are on location and cementing the casing using 228 sacks of class A-2% calcium .25 pps flake with 30% open hole excess ran and 46.5 barrels of fresh water used for cement displacement with good cement returns to surface. The hole was spudded on 09/18/14.
0447239641 2018-03-07 Production Wells No Violations On site inspection accompanied by ODNR Inspector Tom Thomas. On this day the well was observed to be production off the production tree with (2) manual production valves present along with a single (ESD) Emergency Shut Down valve. The ESD has remote shut down buttons located at the well pad entrance along with the production facility and the gas processing units. The well is equipped with steel lines on the 13.375 and 9.625 casing well heads with valves and gauges present to monitor annular pressures. The 13.375 well head section was observed to have 75 pounds pressure on it at surface and the 9.625 section was observed to have 200 pounds on it at surface. In conversation with Anteros Production Supervisor Doug Stebbins all annular pressures are being monitored and evaluated. The well pad has an earthen berm established. The production fluid facility is located on a adjoining site pad and consists of (8) steel construction tanks of 400 barrels fluid capacity each. The tanks are situated inside a polymer lined steel containment dike area.

0725503109 2015-01-16 Drill / Deepen / Reopen No Violations Status check inspection on the current well control issue on this day @ 7:00 am it was observed that the company has built their drilling mud weight up to 15.4# with the well now at a static pressure point. The blow out preventer has been opened up and the drill string found to be free and off bottom a distance of 100. Upon working the drill string a 1334 unit pressure increase was observed along with a slight circulation fluid loss. The company will continue to rotate and circulate the drill string at this point until the drilling mud weight is equal going in and coming out of the wellbore.
0739376302 2015-06-22 Completion Testing No Violations Status check inspection on the well completion process found the well stimulation process has been completed and Bass Company well head has installed their well head blow out preventer. Cudd Services will be providing a tubing snubbing unit and Mercer well Service will be the service rig. The proposed plan is to drill out the completion plugs and run the production tubing.
1052632504 2018-03-07 Production Wells No Violations On this day, I accompanied ODNR Inspector William Stolarik. Please refer to his comments for a detailed report. The well is equipped with annular pressure monitoring devices. Concrete barriers surround the wellheads. The facility is gated with an electronic shutdown button on the left-hand side, prior to the pad. The back of the pad is equipped with (8) 400 barrel steel tanks within containment. There are (4) gas processing units along the front right side of the pad, and a midstream facility along the back right side.
1334042353 2019-01-14 Production Wells No Violations On this day, I spoke with Antero Company Representative Jayson Lukes during the inspection. The producing wells on the pad (permit numbers 4489, 4490, 4492, 4494) are being prepped per Anteros SIMOPS plan that was submitted and reviewed by the ODNR DOGRM, before Stalnaker Energy rigs-up for the plug and abandonment of permit numbers 4491 and 4493. TekSolv is currently onsite setting the back pressure valve. The producing wells have been blown down, are having back pressure valves installed in the tubing hangers, steel cages placed over the wellheads, LEL monitors in place, and concrete Jersey barriers spotted around the perimeter of the wellheads. It will be noted that a 2-way check valve will not be placed in the tubing, as the SIMOPS plan originally stated. I spoke with Jayson Lukes and ODNR Inspector Ken Frye about the amendment to the plan.
1415153434 2014-11-04 Casing String Witnessed No Violations On 11/04/2014 Dallas Morris rig # 24 is now on the hole and has drilled a 22 x 377 surface borehole and ran 362 of 18.625 casing with a Texas style guide shoe and float collar ran along with proper centralizer placement demonstrated. Allied Cement Services is on location to cement the casing using 707 sacks of class A- 2% calcium .25 pps flake with 100% open hole excess ran and the casing pressure tested to 500 psi over lift pressure with good cement returns to surface.
1415386536 2014-11-07 Drill / Deepen / Reopen No Violations Status check inspection on the drilling process found Dallas Morris rig 24 on location with the 17.5 1st intermediate borehole total drilling depth reached at 2,136 with the company rigging up the casing crew to run pipe.
1421246255 2015-01-14 Drill / Deepen / Reopen No Violations Status check inspection of the current drilling progress and well control issue along with Inspector Eric Conrad. It was reported by the Company Man on site Tony Shuster that on 01/13/2015 at 4:30 pm while drilling the Horizontal well bore section at a depth of 13,379 and directional azimuth of 330 degrees using 14.3# synthetic oil based drilling mud the company experienced a well bore gas pressure influx and increase. The Blow out preventer annular well control bag and pipe rams were closed and began to flare the excess gas with a initial choke pressure reported of 2,600 psi. As of the time of my inspection at 6:30 am on 01/14/15 the company was circulating at 41 strokes a minute with a mixture of KCL @ oil based drilling mud with a reported mud weight of 14.3# going into and 13.9# coming out of the well bore with the annular pressure now decreased to 180 psi. The proposed plan is to continue to circulate and lightly build mud weight in small increments up to 14.3# the 14.8# if needed and then initiate a pressure controlled drill string recovery. Once the drill string is out of the hole the company will make repairs to the blow out preventer and well flair stack igniter and monitor well bore pressure.
1793671148 2015-01-29 Drill / Deepen / Reopen No Violations Status check inspection on the drilling process found Patterson Drilling rig # 343 on location with the 8.5 production borehole total drilling depth reached at 17,876 at a directional azimuth of 328.4 degrees and a true vertical depth of 8,341. The rig is currently conditioning the well bore while pulling the drill string out of the hole at a depth of 11,000.
1969250111 2015-01-14 Drill / Deepen / Reopen No Violations I am onsite in response to a well control issue. Upon arrival I witnessed a substantial flare. Company representative Tony Shuster informed me that at 4:30 PM the previous day the rig took a kick. A top pressure of 2650 psi was encountered. The drilling mud fluid weight was 14 pounds per gallon at the time of the kick. The mud weight is currently 14.3 pounds and circulation has been maintained. Backside pressure is currently 50 psi.
2027704687 2015-11-09 Preliminary Restoration No Violations Preliminary Restoration status check inspecting found the affected area by the well pad construction has properly been restored with vegetation and erosion control in place. Addressing the well head- The well drilling and completion process is now complete and the well is on line and producing along with 3 other wells on the pad. Supporting the well is a gas processing unit and 8-400 barrel production tanks located in a polymer lined metal containment dike area. Proper well identification is posted at the well head.
