0043271632 2018-03-07 Production Wells No Violations On site inspection accompanied by ODNR Inspector Tom Thomas. On this day the well was observed to be production off the production tree with (2) manual production valves present along with a single (ESD) Emergency Shut Down valve. The ESD has remote shut down buttons located at the well pad entrance along with the production facility and the gas processing units. The well is equipped with steel lines on the 13.375 and 9.625 casing well heads with valves and gauges present to monitor annular pressures. The 13.375 well head section was observed to have 0 pounds pressure on it at surface and the 9.625 section was observed to have 900 pounds on it at surface. In conversation with Anteros Production Supervisor Doug Stebbins all annular pressures are being monitored and evaluated. The well pad has an earthen berm established. The production fluid facility is located on a adjoining site pad and consists of (8) steel construction tanks of 400 barrels fluid capacity each. The tanks are situated inside a polymer lined steel containment dike area.
0167340668 2018-03-07 Production Wells No Violations On this day, I accompanied ODNR Inspector William Stolarik. Please refer to his comments for a detailed report. The well is equipped with annular pressure monitoring devices. Concrete barriers surround the wellheads. The facility is gated with an electronic shutdown button on the left-hand side, prior to the pad. The back of the pad is equipped with (8) 400 barrel steel tanks within containment. There are (4) gas processing units along the front right side of the pad, and a midstream facility along the back right side.
0273561202 2015-03-22 Drill / Deepen / Reopen No Violations Compliance Notice # 14264424445 issued on 03/15/15 in regards to diesel fuel being spilled and allowed to migrate into a pond next to the drilling rig. Upon inspection it was observed that as a precautionary measure a 3rd party contractor known as Environmental Management Service were still using a skimming method to recover fluid from the pond surface by the means of a vacuum truck, with no visible sheen present on the day of my inspection. Any possible contaminated soil has been removed and the soil in affected area has tested by a 3rd party contractor for any sign of remaining product. In regards to the drilling process Patterson rig 343 is milling debris found in the wellbore at a depth of 7,595 with several metallic objects found in the well bore. The have beginning drilling their curve section at a inclination of 10.26 degrees with a directional azimuth of 330 degrees.
0362599418 2019-01-14 Production Wells No Violations On this day, I spoke with Antero Company Representative Jayson Lukes during the inspection. The producing wells on the pad (permit numbers 4489, 4490, 4492, 4494) are being prepped per Anteros SIMOPS plan that was submitted and reviewed by the ODNR DOGRM, before Stalnaker Energy rigs-up for the plug and abandonment of permit numbers 4491 and 4493. TekSolv is currently onsite setting the back pressure valve. The producing wells have been blown down, are having back pressure valves installed in the tubing hangers, steel cages placed over the wellheads, LEL monitors in place, and concrete Jersey barriers spotted around the perimeter of the wellheads. It will be noted that a 2-way check valve will not be placed in the tubing, as the SIMOPS plan originally stated. I spoke with Jayson Lukes and ODNR Inspector Ken Frye about the amendment to the plan.
0407051247 2015-03-31 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling status found Patterson Drilling on location with the well bore drilling process total drilling depth recently reached at a total measured depth of 18,147 in the direction azimuth of 330.53 and inclination of 90.751 degrees. The rig has about 300 of drill string to pull and they will then prepare to begin running the production casing. This is the last well on the pad to be drilled.
0751603609 2014-10-09 Drill / Deepen / Reopen No Violations Status check inspection on the drilling process found the 12.250 2nd intermediate well bore total drilling depth has been reached at 7,292 and the company is conditioning the borehole in preparation of pulling the drill string and running the 9.625 casing in which a pre job casing tally was received and reviewed with 7,290 of 40# HC-110 casing being shown to be ran with a float shoe and collar used along with proper centralizer placement.
1145156064 2015-03-25 Drill / Deepen / Reopen No Violations Status check on site inspection of the current drilling process. On this day I was accompanied by ODNR Regional Supervisor Brent Bear with the following found upon inspection. Patterson rig 343 is on location and drilling the 8.750 production borehole at a measured depth of 11,069 with a directional azimuth of 329.33 and Inclination of 91.08 degrees and well bore true vertical depth of 8,421.
1409052874 2014-09-30 Drill / Deepen / Reopen No Violations Status check inspection on the drilling progress found Dallas Morris rig # 24 on location drilling the 2nd intermediate 12.250 well bore on air at a depth of 3,971 with the company implementing a 10 degree directional nudge with a 222 degree azimuth.
1410910598 2014-09-16 Casing String Witnessed No Violations On 09/16/14 permission was granted by my supervisor to set the conductor at 80 due to the top hole rig having trouble drilling in solid bedrock, the freshwater was hit at 27, Well Service group has a Soilmec drilling top hole rig on location and has drilled a 32 x 80 conductor borehole and set 80 of 26 conductor with Allied Cement services on location to cement the pipe using 232 sacks of Class A-25 calcium .25 pps flake 50% open hole excess cement with good cement returns to surface.
1411565059 2014-09-24 Casing String Witnessed No Violations On site inspection to witness the surface casing running and cementing found Dallas Morris rig 24 is now on location and has drilled a 24 x 396 surface borehole and set 374 of 18.625 87.50# casing with a float shoe and collar ran along with 5 centralizers, Allied Cement service is on location to cement the surface casing using 740 sacks of class A 2% calcium .25 pps flake with 100% open hole calculated excess ran and 102 barrels H2O to displace cement and land the plug with the casing pressure tested to 500 psi over lift pressure and good cement returns to surface.
1412614181 2014-10-06 Drill / Deepen / Reopen No Violations Status check inspection on the 12.250 2nd intermediate well bore section drilling process found Dallas Morris rig # 24 on location with a current well bore depth of 4,153using a 12.250 directional drilling assembly and implementing a 6.67 degree directional deviation in the direction of 250 degrees and time drilling until the proper build rate and angle is achieved. A recent kick plug was set over lost 12.250 directional tools with a total depth being reached of 4,767 and a 553 sack class H plug set by Allied Cement services at 4,200. The plug was tagged at 4,100 and dressed off to a depth of 4,200 before starting the directional deviation.
1426695255 2015-03-18 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling status found Patterson Drilling rig 343 on location beginning to drill the 8.750 production borehole. A formation integrity test was done to a equivalent of 14.9# drilling mud and 910 psi at the 9.625 casing shoe. The drilling plan shows the well bore to deviate in the direction of 330 degrees azimuth. The company is also implementing a controlled drilling method holding 150psi annular pressure on the well bore using 12.9# drilling mud. In regards to the recent diesel spill that migrated off location the company has HULL Consulting conduction soil analysis of the affected area, along with fluid skimming taking place on the pond surface with no signs of product present upon my inspection. Some housekeeping issues were addressed with the Company Safety hand and Antero rig Manager.
1426875328 2015-03-20 Drill / Deepen / Reopen No Violations Compliance notice # 1426442445 issued on 03/15/2015 follow up inspection. On this day it was observed that the area affected by the Diesel fuel spill on 03/15/2015 has had any affected soil removed and the pond adjoining the well pad has had any material removed from the water surface. As a precautionary measure the company continues to fun surface fluid skimmers. Concerning the current drilling process Patterson rig 343 is on location drilling the 8.70 production borehole using 12.5# drilling mud at a 10.6% inclination at a current measured depth of 7,542.
1447087186 2015-11-09 Preliminary Restoration No Violations Preliminary Restoration status check inspecting found the affected area by the well pad construction has properly been restored with vegetation and erosion control in place. Addressing the well head- The well drilling and completion process is now complete and the well is on line and producing along with 3 other wells on the pad. Supporting the well is a gas processing unit and 8-400 barrel production tanks. Proper well identification is posted at the well head.
