0311162274 2019-01-17 Plug / Plug Back No Violations On this day, I am onsite to witness the plugging operation conducted by Stalnaker Energy Corporation and Formation Cementing. Antero Representative Barney Etcher is onsite. The well was drilled to a depth of 2,153 feet with the 13.375 inch diameter intermediate casing string set at a depth of 2,133 feet, and never drilled out. 9-Energy ran a Cement Bond Log on 1/16/2019 confirming the casing was cemented to surface, and tagged bottom at a depth of 1,872 feet. Stalnaker Energy has a service rig on the hole with 2.375 inch diameter upset tubing set at a depth of 1,857 feet within the 13.375 inch diameter intermediate casing. The bottom plug covers approximately 310 feet (1857--1547) and consists of 230 sacks Class A Cement (48 barrels total volume) with 1.18 yield and a weight of 15.6 pounds per gallon. It was displaced with 6 barrels of fresh water. Then, 43 joints of tubing were pulled to a depth of 490 feet. The bottom of surface plug covers approximately 250 feet (490-240) and consists of 184 sacks Class A Cement (39 barrels total volume) with 1.18 yield and a weight of 15.6 pounds per gallon. It was displaced with 1 barrel of fresh water. Prior to the surface plug being set, the wellhead is being broken down below grade. It is welded together and will be cut off, before cementing. I spoke to ODNR Supervisor Mike Moreland, and the surface plug will be unwitnessed.
1125063932 2014-09-18 Casing String Witnessed No Violations Status check inspection found Rocky Mountain drillings top hole rig has drilled a 32 x 80 conductor borehole and set 80 of 26 conductor in place with a variance granted by the Division to set 80 of conductor due to being in solid limestone bedrock at 60 freshwater was also encountered at 27. Allied cement services are on location and cementing the casing using 228 sacks of class A-2% calcium .25 pps flake with 30% open hole excess ran and 46.5 barrels of fresh water used for cement displacement with good cement returns to surface. The hole was spudded on 09/15/14.
1415580951 2014-11-09 Casing String Witnessed No Violations On 11/09/2014 Dallas Morris rig # 24 is now on the hole and has drilled a 24x 387 surface borehole and ran 372 of 18.625 87.5# casing. Allied cement service is on location to cement the casing using 718 sacks of class A 2% calcium .25pps flake with 100 % open hole cement excess ran and 25 barrels of 6% gel ran ahead with the casing pressure tested to 500 psi over lift pressure upon the landing of the cement plug and good cement returns to surface.
1547479064 2019-01-14 Plug / Plug Back No Violations On this day, I spoke with Antero Company Representative Jayson Lukes during the inspection. The producing wells on the pad (permit numbers 4489, 4490, 4492, 4494) are being prepped per Anteros SIMOPS plan that was submitted and reviewed by the ODNR DOGRM, before Stalnaker Energy rigs-up for the plug and abandonment of permit numbers 4491 and 4493. TekSolv is currently onsite setting the back pressure valve. The producing wells have been blown down, are having back pressure valves installed in the tubing hangers, steel cages placed over the wellheads, LEL monitors in place, and concrete Jersey barriers spotted around the perimeter of the wellheads. It will be noted that a 2-way check valve will not be placed in the tubing, as the SIMOPS plan originally stated. I spoke with Jayson Lukes and ODNR Inspector Ken Frye about the amendment to the plan.
1547820812 2019-01-18 Plug / Plug Back No Violations On this day, I found that Stalnaker Energy has removed the wellhead and Formation Cementing set the surface plug from 100 back to surface with 92 sacks Class A cement weighing 15.6 pounds per gallon, and a 1.18 yield. A steel marker plate will be placed on the top of the casing, and the cellar will be filled with pea gravel. I spoke with ODNR Supervisor Brent Bear, and discussed that the cellar will be removed during the final restoration of the pad, but the company will fill the cellars with pea gravel until that process occurs.
2025321835 2018-03-07 Production Wells No Violations Status check inspection found a night cap installed on the B-section of the well head. Records indicate the well was drilled to the point of the 13.375 1st intermediate with conductor and 18.625 surface being set also. The drilling permit expired on 01/26/2018.
2093379237 2019-03-01 Final Restoration No Violations On this day, I am conducting a final restoration inspection. The pad has existing wells in production, and the cellar remains, but has been filled with gravel. Site meets ORC 1509.072 (B).
