0904375901 2015-08-11 Preliminary Restoration No Violations Prelimminary Restoration status check inspection found the area affected deplicted in the pre site construction has erosion control and vegetation in place. There are a total of (4) wells on this pad all named the Howell. Addressing the status of the wells all 4 wells are capable of production with a total of 4 gas processing units in place on locatin with one each supporting each well. Near the entry point of the location located on the well pad the production fluid storage facility is located with a total of 4 -400 barrel production tanks located in a poly lined steel containment dike area. Preliminary Restoration meets 1509.072 and is passed on this day.
1395303167 2019-05-28 Production Wells No Violations On this day, I found the pad fenced and gated with signage at the entrance. An earthen berm surrounds the perimeter of the pad. There are (4) producing wells equipped with a plunger lift system, (4) gas processing units (GPU) with sand traps, and (4) 400-barrel and (1) 50-barrel steel production tanks within polymer lined steel containment. An Emergency Shutdown (ESD) button is present at the production facility, wellheads, and entrance. Additionally, concrete barriers surround the perimeter of the wellheads and GPUs. Identification and annular pressure monitoring devices are present at the wellheads. The intermediate casing has a pressure of 550 pounds per square inch (psi). No gauge is available for the surface casing. On 5/28/2019, I spoke with Company Representative Steve Habig to inquire about the presence of a surface string gauge. It will be noted that slight erosion has begun at a stockpile on the east side of the lease road, just prior to the pad. I spoke with ODNR Engineering Technician Andy Thomas to notify him.
1414024151 2014-10-22 Casing String Witnessed No Violations The said well was spudded on 10/21/14 by Express Energys top hole rig with a 105 x 32 conductor borehole being drilled. On 10/22/14 Stingray cement service cemented the 26 x 108 of conductor pipe using 273 sacks of Class A 2% calcium .25 pps flake cement with the cement weight verifyied and mix water tested for chlorides and Ph level, good cement returns to surface.
1421962480 2015-01-22 Casing String Witnessed No Violations 01/22/15 Patterson Drilling rig 538 is now on the hole and has drilled a 345 x 17.5 surface borehole and ran 343 of 13.375 surface casing with a guide shoe and float collar on the casing along with proper centralizer placement. Stingray Cement service is on location to cement the casing using 465 sacks of class A- 2% calcium .25 lb/sk celluseal cement with 47 barrels to displacement and the casing pressure tested to 500 psi over lift pressure upon the landing of the plug with good cement returns to surface.
1422377866 2015-01-27 Drill / Deepen / Reopen No Violations Status check inspection on the drilling process found Patterson Rig 538 on location drilling the last of 4 wells permitted on the well pad with the current drilling status found to be at a depth of 2,810 using a 8.750 bit and drilling on air in the vertical well bore section with a directional 3 degree nudge in the directional azimuth of 80.66 degrees with a total vertical depth to be 9,404 of this section before beginning to drill the horizontal well bore section.
