Cudd Well control tubing snubbing unit is on location and will utilized on the well on top of Bass Well Service well control blow out preventer. ROC Company will conduct the well bore flow back operation with all operations to be conducted utilizing the pressure maintaining process by keeping the well bore static while the completion plugs are drilled out. All well pad perimeter drains are in the closed position and the company has a 3rd party contractor identified as Deepwell on location with their vacuum truck recovering any fluid on location while completion operations are being conducted.

There is a gas processing unit on location to support the well along with 8 steel 400 barrel capacity production tanks located inside a polymer lined steel containment dike area. The well site meets ORC1590.072.
1076528030 2019-01-14 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (8) producing wells, (8) gas processing units within steel containment, (8) 400-barrel steel production tanks within polymer lined steel containment, and an Emergency Shutdown (ESD) button is present. Additionally, concrete Jersey barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 1,100 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 140 psi.

1380298515 2015-01-12 Casing String Witnessed No Violations I am onsite to witness the casing installation and cementing operations of the conductor casing string. Rocky Mountain Drilling drilled 100 of 32 hole with an auger rig. Rocky Mountain then ran 100 of 26 .375 wall weld casing in the well. The cement mix water was tested with a low range Quantab and registered 82 parts per million total chlorides. The casing was cemented by Allied Oil and Gas Services circulating 330 sacks of class A cement with .25 pounds per sack flowseal and 2% CaCl into the annular space. I witnessed 15 barrels of cement return to surface and remain static. Three feet of cement was left in the cellar to ensure a liquid tight seal.
1439711895 2015-08-16 Casing String Witnessed No Violations I am onsite to witness the surface casing cementing operations. Patterson Drilling rig 343 drilled a 17.5 borehole to a depth of 1434 and ran 13.375 casing to 1412. CalFrac cemented the casing in place with 292 barrels of class A cement at a weight of 15.6 pounds per gallon. Circulation was lost at 30 barrels into displacement and was not regained. The casing integrity test was passed by pressuring the casing up to 2100 psi and holding it for five minutes after landing the plug at 1600 psi. The company was instructed to find the cement top and cement from there to surface.
1439819546 2015-08-16 Casing String Witnessed No Violations Status check inspection to witness the annular space top out job on the 13.375 surface casing. The casing string was set to depth of 1,411 and the initial cement job was witnessed by Inspector Eric Conrad who notified me at 7:15 am on this day that the company did not get cement returns to surface and indicated they would be conducting a top out job in the afternoon hours. I arrived at 14:00 pm and witnessed Cal Frack top off the annular space with 86 barrels of class A cement, the cement was pumped through a 1 tubing string set to a depth of 130 in the annular space. I verified good 15.6# cement to surface by weight of mud scale. Off location at 18:00 pm.
1614851765 2016-04-06 Completion Testing No Violations On site inspection to witness the well bore stage 41 of 43 stimulation process with Allied-Horizontal wireline service perforating the stage with 60 perforations in 6 clusters staggered out between the depths of 9,046 and 8,897, a composite completion plug was set at 9,060. US Well service is providing the pressure pumping service with Bass Well Head surface control in place supported by ROC flow back service utilizing 15,000 psi working pressure stimulation iron. The well bore TVD id 8,350 and there is 2 stages left to stimulate. The annular pressure is being monitored electronically. All stimulation equipment is placed on polymer containment with the well pad perimeter valves in the close position acting as the primary well pad perimeter containment.
