There are (8) producing wells, (8) gas processing units within steel containment, (8) 400-barrel steel production tanks within polymer lined steel containment, and an Emergency Shutdown (ESD) button is present. Additionally, concrete Jersey barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 0 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 20 psi.

1437045467 2015-07-15 Casing String Witnessed No Violations Status check inspection in regards to the surface casing cement top out job completed on 07/15/2015. During the initial cement job the cement truck experienced a mechanical failure during displacement and the cement was displaced with the rig mud pump and no plug was dropped. Upon my request the company verified the cement was displaced to depth by running in the hole and tagging up on the float collar, and then bond logging the casing with a verified cement bond up to 120 of surface. The casing was pressure tested to verify integrity and good cement bond at the shoe. We then topped the annular side off through a 1tubing string with 200 sacks a class A 15.6# 1.19 yield cement with good cement to surface.

1521002208 2015-07-13 Casing String Witnessed No Violations Patterson Drilling rig # 343 is now on the hole and has drilled a 17.5surface borehole to a depth of 1,412 and ran 1,412 of 13.375 54.5# casing with a float collar and guide shoe ran. Calfrac cement service is on location to cement the casing using 1389 sacks of class A 2% calcium .125 pps flake with 50% open hole cement excess ran, during cement displacement the cement truck failed and the cement was displaced with the rig pumps and no cement plug was dropped, no cement returns were seen to surface, the company shall wait the required 8 hrs and bond log.
1600140316 2015-01-12 Casing String Witnessed No Violations I am onsite to witness the casing installation and cementing operations of the conductor casing string. Rocky Mountain Drilling drilled 100 of 32 hole with an auger rig. Allied Oil and Gas Services then ran 100 of 26 .375 wall weld casing in the well. The cement mix water was tested with a low range Quantab and registered 100 parts per million total chlorides. The casing was cemented by circulating 334 sacks of class A cement with .5 pounds per sack flowseal and 2% CaCl into the annular space. I witnessed 17 barrels of cement return to surface and remain static. Three feet of cement was left in the cellar to ensure a liquid tight seal.
