0034634408 2015-02-14 Drill / Deepen / Reopen No Violations Notification was received by the company man on site Brandon Jeffcoat that Express Energys top hole rig has spudded the said well and is drilling the conductor borehole. They will notify me of the upcoming cement job once total depth has been reached and they are running casing.
0182334573 2018-03-02 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well site meets ORC 1509.072. The well pad entry point is gated and locked with signage posted. The lease road is graveled along with the well pad, both are in good repair. Addressing the well head - The well is equipped with a B section along with a 9.65 and 5.5 casing well head section and a tubing well head. The tubing well head has 2 manually controlled valves and one emergency shut down valve. Annular pressure monitoring is in place with 100 (psi) pound per square inch indicated between the 9.65 and 13.375 and 900 psi between the 5.5 and 9.625. Proper well identification is posted at the well head. Also on location is (5) gas processing units one for each well head on location. At the left front corner of the well pad is the production fluid storage area consisting of a total of 8 steel construction tanks all situated inside a polymer lined steel containment dike area. Emergency shut down devices are situated at the well pad entrance and at the tank battery loading site.
0310321978 2016-02-17 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction meets ORC 1509.072 with vegetation established and erosion control in place. Addressing the well status, The well drilling and completion process is now complete and all (5) of the wells on the pad are capable of production and are currently in the flow back testing stages with a company on location identified as Solid Oak flow back services monitoring the well testing. There is 3 vertical sand trap separators and 5 gas processing units on location with the sand trap separators being evacuated into a 500 barrel tank. Addressing the production fluid storage area- A total of (8) 400 barrel capacity tanks are present inside a polymer lined steel containment dike area.
0317225092 2015-04-14 Casing String Witnessed No Violations 04/14/2015- Patterson Drilling rig 332 is now on the hole and has drilled a 24 x 396 surface casing borehole and ran 400 of 87.5# J-55 surface casing with a Texas Style guide shoe and float collar ran. CalFrac cement service is on location to cement the casing using 705 sacks of class A-3% calcium .125 pps LCM cement with 70% open hole cement excess ran and the casing pressure tested to 500 psi upon the landing of the cement plug with good cement returns witnessed to surface 56 bbl into displacement.
0516902196 2015-06-08 Drill / Deepen / Reopen No Violations Status check inspection on the well drilling progress found Patterson Drilling rig 332 on location drilling the 8.750 production borehole at a current depth of 14,770 using managed pressure drilling holding 417 psi annular pressure with 4,119 psi stand pipe pressure and a current drilling mud weight of 15.1# and viscosity of 77. The proposed well bore depth is 16,500.
0672450051 2015-04-21 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling process found Patterson Drilling rig 332 on location drilling the 12.2502nd intermediate wellbore at a depth of 5,109. The company is implementing a 15 degree borehole nudge in the direction of 72 degrees azimuth.
0914490741 2015-02-20 Casing String Witnessed No Violations I am onsite to witness the conductor casing string installation and cementing operations. Express Energy Services drilled 100 of 32 borehole and ran 100 of 26 casing. My test of the cement mix water with a Quantab indicated chloride levels of 193 parts per million. Due to failing to circulate cement to surface on the other wells on the pad 200% excess cement was pumped. I witnessed 30 barrels of cement return to surface and circulation was maintained through out the job. Three feet of cement was left in cellar to provide a liquid tight seal.
1255531894 2019-02-07 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (5) producing wells, (5) gas processing units, (8) 400-barrel steel production tanks within polymer-lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 1,100 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 0 psi.
1433183231 2015-06-01 Drill / Deepen / Reopen No Violations Status check inspection on the well drilling status found Patterson Drilling rig 343 on location drilling the 8.750 production borehole at a current depth of 8,715 in the directional azimuth of 327.78 degrees with a inclination of 89.78 and 6.69 degree offset.
1707111132 2015-04-29 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling process found Patterson Drilling rig 332 on location and the issue resolved with the 12.250 drill bit and bottom hole assembly being stuck in the hole at a depth of 6,224. The rig is running in the hole at 3,885 with the drill string to resume drilling operations on the 12.250 2nd intermediate wellbore.
1768110077 2015-06-05 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling progress and reported gas influx issue. Patterson Drilling rig # 343 is on location and has drilled the 8.750 production bore to a measured depth of 14,320 and has pulled the drill string off bottom to 14,296 with a borehole directional azimuth of 328.93 degrees and inclination of 90.71 degrees, the company is currently using 14.5 mud weight and circulating gas out of the borehole with a recorded measurement of 8,921 units of 3rd party gas and no recorded fluid loss only gas borehole influx.
