0156915331 2015-05-05 Casing String Witnessed No Violations Status check inspection to witness the cementing of the 369 of 18.625 surface casing inside a 24 borehole with CalFrac cement service on location to cement the casing using 616 sacks of class A-2% calcium .25 pps lcm cement with 50% open hole cement excess ran and circulation established prior to cementing with 6% gel ran ahead and good cement returns witnessed to surface with the casing pressure tested to 500 psi over lift pressure upon the landing of the cement plug.
0278756150 2019-02-07 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (5) producing wells, (5) gas processing units, (8) 400-barrel steel production tanks within polymer-lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 1,650 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 120 psi.
0569941269 2015-05-12 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling process found Patterson Drilling on location drilling the 12.250 2nd intermediate well bore section at a current depth of 5,204 and implementing a 15 degree borehole nudge in the direction of 73 degrees azimuth with the company using a tri cone drill bit and misting on air with a proposed total depth to be 7,540 for this section being drilled.
0849972733 2015-05-15 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling process fount Patterson Drilling rig # 332 on location drilling the 12.250 2nd intermediate well bore section at a current depth of 7,486 with the company currently conditioning the hole on air. The current borehole position is 70.04 degrees azimuth with a 16 degree inclination.
0970204103 2015-05-26 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling process found Patterson Drilling rig # 332 on location drilling the 8.750 production borehole at a depth of 14,809 in the horizontal well bore section. The company is currently practicing managed pressure drilling using 12.9# mud weight and holding back 110 pounds pressure on the annular side of the drill string at surface.
1280431380 2015-05-19 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling process found Patterson drilling rig 332 on the hole with the 9.625 2nd intermediate casing recently set to a depth of 7,564. The company has ran in the hole with the directional drilling assembly and is preparing to begin drilling the 8.750 production borehole curve and lateral, a Formation Integrity Test will be performed on the casing shoe.
1424729509 2015-02-23 Casing String Witnessed No Violations The said well was spudded on 02/20/2015 and due to hole conditions a 36 x 40 liner string was set at surface, this was due to the well pad being a built on a old limestone quarry and the conductor hole collapsing. 02/23/2015 Express Energys top hole rig is on location and as drilled a 32 x 94 conductor borehole and ran 94 of 26 conductor pipe. Express Energy cement service is on location to cement the casing using 330 sacks of class A-2% calcium .125 pps flake with 100% open hole cement excess ran and the cement water tested for chlorides and PH prior to cementing with good cement returns witnessed to surface.
1455736825 2016-02-17 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction meets ORC 1509.072 with vegetation established and erosion control in place. Addressing the well status, The well drilling and completion process is now complete and all (5) of the wells on the pad are capable of production and are currently in the flow back testing stages with a company on location identified as Solid Oak flow back services monitoring the well testing. There is 3 vertical sand trap separators and 5 gas processing units on location with the sand trap separators being evacuated into a 500 barrel tank. Addressing the production fluid storage area- A total of (8) 400 barrel capacity tanks are present inside a polymer lined steel containment dike area.
1519998364 2018-03-02 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well site meets ORC 1509.072. The well pad entry point is gated and locked with signage posted. The lease road is graveled along with the well pad, both are in good repair. Addressing the well head - The well is equipped with a B section along with a 9.65 and 5.5 casing well head section along with a tubing well head. The tubing well head has 2 manually controlled valves and one emergency shut down valve. Annular pressure monitoring is in place with 100 (psi) pound per square inch indicated between the 9.65 and 13.375 and 900 psi between the 5.5 and 9.625. Proper well identification is posted at the well head. Also on location is (4) gas processing units one for each well head on location. At the right front corner of the well pad is the production fluid storage area consisting of a total of 8 steel construction tanks all situated inside a polymer lined steel containment dike area. Emergency shut down devices are situated at the well pad entrance and at the tank battery loading site.
