0014139492 2014-11-03 Casing String Witnessed No Violations The said well was spudded on 11/01/2014 by Rocky Mountain Drillings top hole rig with a 90 x 32 conductor borehole being drilled. On 11/03/14 I granted a variance to set the conductor at 90 due to being in solid bedrock and 93 of 26 weld connection conductor was set and cemented by Allied Cement Services using 536 sacks of Class A cement 2% calcium .25 pps flake cement with a calculated 250 % cement excess ran due to being in an old limestone quarry and the location on backfill.
0356919706 2015-03-08 Drill / Deepen / Reopen No Violations Status check inspection on the current well drilling process found Patterson drilling rig 332 on location with the current 8.750 production borehole total drilling depth recently reached at a measured depth of 16,718 in the directional azimuth of 330.28 degrees with a 90.43 degree inclination. The rig was currently pulling the drill string out of the hole at a depth of 5,814 at the time of my inspection.
0799062941 2018-03-02 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well site meets ORC 1509.072. The well pad entry point is gated and locked with signage posted. The lease road is graveled along with the well pad, both are in good repair. Addressing the well head - The well is equipped with a B section along with a 9.65 and 5.5 casing well head section and a tubing well head. The tubing well head has 2 manually controlled valves and one emergency shut down valve. Annular pressure monitoring is in place with 200 (psi) pound per square inch indicated between the 9.65 and 13.375 and 350 psi between the 5.5 and 9.625. Proper well identification is posted at the well head. Also on location is (5) gas processing units one for each well head on location. At the left front corner of the well pad is the production fluid storage area consisting of a total of 8 steel construction tanks all situated inside a polymer lined steel containment dike area. Emergency shut down devices are situated at the well pad entrance and at the tank battery loading site.
1053630456 2019-02-07 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (5) producing wells, (5) gas processing units, (8) 400-barrel steel production tanks within polymer-lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 300 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 200 psi.

Addressing the well status, The well drilling and completion process is now complete and all (5) of the wells on the pad are capable of production and are currently in the flow back testing stages with a company on location identified as Solid Oak flow back services monitoring the well testing. There is 3 vertical sand trap separators and 5 gas processing units on location with the sand trap separators being evacuated into a 500 barrel tank. Addressing the production fluid storage area- A total of (8) 400 barrel capacity tanks are present inside a polymer lined steel containment dike area.

Patterson drilling rig 332 is the rig on location and has drilled a 24 x 357 surface borehole and ran 352 of 18.625 87.5# J-55 casing. Allied cement services is on location to cement the casing using 779 sacks of class A -2% calcium, .25 pps poly flake cement with circulation established prior to cementing and the casing pressure tested to 500 psi upon the landing of the plug.
1424986863 2015-02-26 Drill / Deepen / Reopen No Violations Status check inspection on the current well drilling process found Patterson Drilling rig # 332 on location with the current well drilling measured depth to b e 12,942 in the horizontal well bore section using a 8.5 directional drilling assembly with a current directional azimuth of 334 degrees at true vertical depth of 8,170 and inclination of 91.14 degrees. The company currently has experienced a well bore gas pressure influx of 1,100 psi at the choke and is implementing a 35# lcm well control sweep, the blow out preventer is in the closed position and the mud weight was at 13#. The company will continue to monitor and control the gas and flair as needed.

The company shall continue to condition the hole with 5 sacks of Baker Squeeze well bore conditioning material added to the current synthetic based drilling mud. Once well bore stability is reached drilling will proceed. The Blow out preventer is in the open position and they are circulating through the gas buster system.

1426420372 2015-03-09 Casing String Witnessed No Violations Administrative inspection to document the 5.5 production casing cement job issue with lost circulation returns during the production cement job. The company man on site notified me that the returns were lost during the production cement job. The company will be bond logging the well when the completion crews move in. And has been notified that no stimulation shall take place until the Division reviews the bond log and gives approval to move foreword.
1441071385 2015-08-31 Completion Testing No Violations Status check inspection on the well completion process found the well stimulation process complete and Fortis Well Service has completed the 5.5 production casing clean out run and set 8,338 of 2.875 production tubing in the well with the well now ready for the production flowlines and equipment to be installed. The rig is working on secondary polymer containment and has flushed all brine from the tubing prior to pulling the workover string.
1445225544 2015-02-27 Drill / Deepen / Reopen No Violations My inspection today finds the drilling rig, Patterson 332, is still on a choke with gas levels holding steady between 2500 units and 3200 units. The current mud weight is 13.0 pounds per gallon. The rig has lost 1000 barrels of oil based mud since encountering the kick. The current shut in differential pressure is 100 psi. Moving forward the rig will attempt to build mud weight by .3 pounds per gallon.

Addressing the well status-The well drilling and completion process is now complete and a production well head tree is in place and the well is on line and producing. At the production fluid storage area a total of 8 production tanks are present with a fluid capacity of 400 barrels each, all located inside a polymer lined metal containment dike area.
1498840939 2015-08-12 Completion Testing No Violations Status check inspection on the well stimulation process found US Well Service on location along with Horizontal Wireline Service. The pad is currently on shut down due to formation communication pressure with a neighboring well being drilled in the horizontal section (3411124579000) 30 of 47 total stages have been stimulated with the stimulation equipment being located on secondary polymer containment and the well pad earthen perimeter serving as the primary site boundary containment.
1603334132 2015-01-18 Drill / Deepen / Reopen No Violations Status check inspection on the drilling progress found Patterson Drilling rig 332 on location drilling the 12.250 2nd intermediate well bore section at a depth of 3,501 with the company implementing a 15 degree well bore nudge in the direction azimuth of 184 degrees with the last borehole deviation survey at 3,323. The rig is currently pulling the drill string back up inside the 13.375 and off bottom for mist pump repairs.

Allied Horizontal Wireline Service is on location and has perforated the well bore stage with 60 perforations in 6 clusters between the depths of 15,944 and 16,279. KLX Company well head is providing surface control with a hydraulic master control valve and 2 manual valves present in the well head stack. Allied Horizontal Wireline was advised to post proper identification for the wireline support Hazardous materials on site.
