0195278476 2018-03-07 Production Wells No Violations Status check inspection to document the well status. The well is equipped with a casing and tubing well head assembly with (2) manually operated valves on the tubing well head and one emergency shut down valve on the tubing tree production flow line. The well head casing flow line is also tied into the production flow line however the valves are in the closed position. The 13.375 and 9.625 casing well head assemblies have flowlines with valves and gauges installed to monitor annular pressure. The 13.375 casing well head pressure gauge reads 270 pounds pressure and the 9.625 casing well head reads 40 pounds pressure. On pad total is (8) complete wells supported by (8) gas processing units. Also on location is a production fluid facility consisting of a total of (8) steel construction tanks with a capacity of 400 barrels of fluid each, the tanks are situated inside a polymer lined steel containment dike area. The well lease road entrance is gated and locked with signage posted at the entrance of the well pad and at each well head. The well pad has a stone surface along with the lease road. The well pad perimeter berm is of earthen material and in good repair.
0272801811 2019-02-07 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (8) producing wells, (8) gas processing units, (4) 400-barrel steel production tanks within polymer-lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. This well is setup to be produced by means of a plunger lift system. The 9.625 inch diameter intermediate casing has a pressure of 0 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 280 psi. It will be noted that in 2018 a slip had occurred along the southern slope of the pad and western slope of the lease road, prior to the pad. The slopes have since been graded, seeded, and erosion control in place. Currently awaiting new vegetation.

They have nippled up and tested the Blow Out Preventer and drilled out from under the 13.375 and are sitting on bottom with directional tools awaiting directional drilling orders from Anteros management.

0889575595 2015-04-29 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling process found Patterson Drilling rig # 340 on location drilling the 8.750 production borehole at a depth of 14,085 in the directional azimuth of 149 degrees and inclination of 89.48 degrees with a true vertical depth of 8,484 using 14.03 mud weight and implementing managed pressure drilling controlling 1769 units of 3rd party gas and holding 282 psi annular pressure on the drill string at surface through the choke. The company has experienced several fluid gains and has contacted me regularly while working gas influx out of the well bore through flaring.
0982962373 2015-11-09 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction has proper vegetation and erosion control in place in accordance to ORC 1509.072. Addressing the well and production equipment- The well drilling and completion process is finished with 2.875 production tubing set to a depth of 8,348 with crews present hooking up the production flowlines and equipment. Crews are also present on location removing the containment used during the completion process and cleaning up the well heads and cellar. A total of 8 wells heads are present on location along with 8 gas processing units with 8- 400 barrel production tanks present in a polymer lined metal containment dike area.
1328166935 2014-12-11 Casing String Witnessed No Violations The said well was spudded on 12/10/2014 by Express Energys top hole rig with a 32 x 100 conductor borehole being drilled and 100 of 26 conductor pipe being set. Express Energys cement service is on location to cement the casing using 330 sacks of class A-2% calcium .50 pps flake with 100% open hole cement excess ran and circulation established prior to cementing and the cement displaced with 55 barrels of fresh water and good cement returns witnessed to surface.
1419002908 2014-12-19 Drill / Deepen / Reopen No Violations Status check inspection on the drilling process found Sidewinder Drilling rig 59 on location drilling the surface borehole @ 244 on air. Spud and standby notification was received on 12/08/14 by the company man for a surface casing cement job that evening. Efforts to contact the rig by phone and e-mail for a status update were not possible so I made a field visit for a status check @ 5:45 am.
1419175521 2014-12-21 Casing String Witnessed No Violations 12/20/14 The surface casing was ran and cemented to with Sidewinder Drilling rig 59 on location and Allied Cement Services cementing the casing using 932 sacks of class A 2% calcium .25 pps flake and 25 barrels of 6% gel ran ahead with 40 barrels of good cement returns documented to surface and the casing pressure tested to 500 psi upon the landing of the plug. The job was not witnessed due to having a migraine headache and all other inspectors being occupied.
1439388516 2015-08-12 Casing String Witnessed No Violations Status check inspection on the well completion process found Fortis Service Companies work over rig on location currently reaming a tight spot in the 5.5 23# production casing with a 4.375 taper mill at the depth of 10,920. No annular pressure is reported and US Well service is standing by to finish the well stimulation process.
1443789753 2015-10-01 Completion Testing No Violations Status check inspection on the well completion process found that a wireline gun and 12,000 of wireline has been lost in the well bore during the stimulation process. Fortis Company workover rig did attempt to recover the lost items unsuccessfully and now a coil tubing unit is being mobilized to fish on the lost items.
