0464277342 2015-11-09 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction has proper vegetation and erosion control in place in accordance to ORC 1509.072. Addressing the well and production equipment- The well drilling and completion process is finished with 2.875 production tubing set to a depth of 8,465 with crews present hooking up the production flowlines and equipment. Crews are also present on location removing the containment used during the completion process and cleaning up the well heads and cellar. A total of 8 wells heads are present on location along with 8 gas processing units with 8 400 barrel production tanks present in a polymer lined metal containment dike area.
0814518886 2019-02-07 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (8) producing wells, (8) gas processing units, (4) 400-barrel steel production tanks within polymer-lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. This well is setup to be produced by means of a plunger lift system. The 9.625 inch diameter intermediate casing has a pressure of 250 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 50 psi. It will be noted that in 2018 a slip had occurred along the southern slope of the pad and western slope of the lease road, prior to the pad. The slopes have since been graded, seeded, and erosion control in place. Currently awaiting new vegetation.
1066392929 2015-01-18 Drill / Deepen / Reopen No Violations Status check inspection on the drilling progress found Sidewinder Drilling rig 59 on location drilling the 12.250 2nd intermediate well bore section at a depth of 3,018 with a 12 degree directional nudge being implemented in the direction of 184 degrees azimuth.
1181022871 2018-03-07 Production Wells No Violations Status check inspection found the well is equipped with a casing and tubing well head assembly with (2) manually operated valves on the tubing well head and one emergency shut down valve on the tubing tree production flow line. The well head casing flow line is also tied into the production flow line however the valves are in the closed position. The 13.375 and 9.625 casing well head assemblies have flowlines with valves and gauges installed to monitor annular pressure. The 13.375 casing well head pressure gauge reads 125 pounds pressure and the 9.625 casing well head reads 200 pounds pressure. On pad total is (8) complete wells supported by (8) gas processing units. Also on location is a production fluid facility consisting of a total of (8) steel construction tanks with a capacity of 400 barrels of fluid each, the tanks are situated inside a polymer lined steel containment dike area. The well lease road entrance is gated and locked with signage posted at the entrance of the well pad and at each well head. The well pad has a stone surface along with the lease road. The well pad perimeter berm is of earthen material and in good repair.
1420980034 2015-01-10 Casing String Witnessed No Violations On site at the Urban Pad to witness surface pipe being run and cemented in place Sidewinder rig 59 ran 483 of 18.625 and it was cemented in place with 788 sacks of Class A cement and they had 55 bbl of good returns.
1431711050 2015-05-15 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling process found Patterson Drilling rig # 340 on location drilling the 8.750 production borehole with the horizontal curve section recently completed at a depth of 8,854 with a true vertical depth of 8,425 using 13.8# oil based drilling mud and practicing managed pressure drilling holding back 80 psi annular pressure. The borehole direction is 90.06 degrees inclination and 151.27 degrees azimuth.
1537366707 2014-12-10 Casing String Witnessed No Violations I am onsite to witness the casing installation and cementing operations of the conductor casing string. Express Energy Services drilled 100 of 32 hole with an auger rig. Express then ran 100 of 26 .375 wall weld casing in the well. The casing was cemented by circulating 330 sacks of class A cement with .5 pounds per sack flowseal and 2% CaCl into the annular space. I witnessed 14 barrels of cement return to surface and remain static. Three feet of cement was left in the cellar to ensure a liquid tight seal.
