0104527356 2014-12-10 Casing String Witnessed No Violations I am onsite to witness the casing installation and cementing operations of the conductor casing string. Express Energy Services drilled 100 of 32 hole with an auger rig. Express then ran 100 of 26 .375 wall weld casing in the well. The casing was cemented by circulating 330 sacks of class A cement with .5 pounds per sack flowseal and 2% CaCl into the annular space. I witnessed 18 barrels of cement return to surface and remain static. Three feet of cement was left in the cellar to ensure a liquid tight seal.

Addressing the well and production equipment- The well drilling and completion process is finished with 2.875 production tubing set to a depth of 8,600 with crews present hooking up the production flowlines and equipment. Crews are also present on location removing the containment used during the completion process and cleaning up the well heads and cellar. A total of 8 wells heads are present on location along with 8 gas processing units with 8 400 barrel production tanks present in a polymer lined metal containment dike area.

1424337016 2015-02-19 Casing String Witnessed No Violations 02/19/2015- Patterson Drilling rig # 340 is now n the hole and has drilled a 476 x 24 surface borehole and ran 476 of 18.625 87.5# casing with Allied cement service on location to cement the casing using 935 sacks of class A-2% calcium .125 pps Flowseal 100% open hole excess cement. With circulation established prior to cementing and the mix water tested for PH & Chlorides prior to cementing and the casing pressure tested to 500 psi over lift pressure upon the landing of the cement plug with good cement returns reported to surface, The job was not witnessed due to the sub zero temperatures and the rig freezing up, after 4 hours standing by on location I departed location at 9:30 am and the job did not go until after 3:00 pm. Inspector will not that the company has opted to leave the completed wells on the pad live and producing while drilling proceeds on the remainder of the wells to be completed.

The current true vertical depth is 4,416.84 and the company is following the proposed submitted drilling plan.

1438271686 2015-07-30 Completion Testing No Violations Status check inspection on the well stimulation process found Renegade Wireline Service on location perforating stage 27 out of 42 total stages to be completed. 60 perforations were shot in 6 clusters of 10 shots between the depths of 11,517 and 11,355 . Bass Well Control completion stack is utilized for well control with 2 manual and one hydraulic valve in place. US Well Service is the stimulation contractor on site with 15,000 psi rated iron in place.
1443342510 2018-03-07 Production Wells No Violations Status check inspection found the well is equipped with a casing and tubing well head assembly with (2) manually operated valves on the tubing well head and one emergency shut down valve on the tubing tree production flow line. The well head casing flow line is also tied into the production flow line however the valves are in the closed position. The 13.375 and 9.625 casing well head assemblies have flowlines with valves and gauges installed to monitor annular pressure. The 13.375 casing well head pressure is 400 psi and the 9.625 casing well head reads 20 pounds pressure. On pad total is (8) complete wells supported by (8) gas processing units. Also on location is a production fluid facility consisting of a total of (8) steel construction tanks with a capacity of 400 barrels of fluid each, the tanks are situated inside a polymer lined steel containment dike area. The well lease road entrance is gated and locked with signage posted at the entrance of the well pad and at each well head. The well pad has a stone surface along with the lease road. The well pad perimeter berm is of earthen material and in good repair.
1635624806 2015-08-31 Completion Testing No Violations Status check inspection on the well completion process found the well stimulation process complete and Fortis Well Service has completed the 5.5 production casing clean out run and set 8,601 of 2.875 production tubing in the well with the well now ready for the production flowlines and equipment to be installed. The rig is working on secondary polymer containment and has flushed all brine from the tubing prior to pulling the workover string.
1999776611 2019-02-07 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (8) producing wells, (8) gas processing units, (4) 400-barrel steel production tanks within polymer-lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. This well is setup to be produced by means of a plunger lift system. The 9.625 inch diameter intermediate casing has a pressure of 500 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 400 psi. It will be noted that in 2018 a slip had occurred along the southern slope of the pad and western slope of the lease road, prior to the pad. The slopes have since been graded, seeded, and erosion control in place. Currently awaiting new vegetation.
