1417276929 2014-11-29 Casing String Witnessed No Violations The said well was spudded on 11/28/14 by Express Energys top hole rig with a 100 x 32 conductor borehole being drilled. 11/29/14 the conductor was set and cemented using 330 sacks of class A 2% calcium .25pps flake cement with 100% open hole cement excess ran and the cement displaced with 67 barrels of fresh water.

1447089049 2015-11-09 Preliminary Restoration No Violations Preliminary Restoration status check inspection fount the area affected by the well pad construction has been restored with proper erosion and vegetation control in place. Addressing the well status-the drilling process is now complete and the well is awaiting stimulation. A total of 8-400 barrel production tanks are in place inside a poly lined metal containment dike area. A total of 5 gas process separation units are in place to support each of the 5 existing wells on the pad.
1450179949 2014-12-16 Casing String Witnessed No Violations I am onsite to witness the surface casing installation and cementing operations. Dallas Morris Rig 24 drilled 400 of 24 diameter hole. The rig crew then installed 393 of J-55 18.625 casing. I performed a test of the cement mix water with a low range Quantab, the test indicated that the total dissolved chlorides were present at a concentration of 44 parts per million. Allied Oil and Gas Services pumped 777 sacks of class A cement with .25 pounds per gallon flowseal and 2% CaCl into the casing with more than 70 barrels of the cement returning to surface. The plug was landed and the casing integrity test passed with only minimal bleed off.
1503661800 2017-08-25 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction meets ORC 1509.072 with vegetation established and erosion control in place. The lease road is gated and identification signs are in place. There is emergency shut down devices at the well pad entry point and tank battery and also at the electrical service point for the gas processing units. The well is equipped with a casing and tubing head with flow lines attached to each well head outlet. A primary master valve is in place on the tubing head with (2) master valves present down stream in the production flow line. The casing head also has (2) master valves present. A gas processing unit is present on the well pad along with units supporting the other 4 wells on the pad. At the production facility a total of (8) steel construction 400 barrel capacity tanks are present inside a polymer lines steel containment dike area. Proper well head identification is posted at each well. Annular pressure monitoring gauges are present showing 100 psi on the 9.625 diameter casing annulus and 575 psi on the 13.375 . Doug Stebbins with Antero was notified.
1520430233 2018-03-07 Production Wells No Violations On site inspection along with ODNR Inspector Tom Thomas and Antero employee Doug Stebbins. The intention of the inspection is to discuss and review annular pressures. The said well was observed to be in production and to have annular pressure gauges present on the 13.375 casing well head section and 9.625 casing well head section. The pressure between the internal of the 13.375 and external of the 9.625 was 475 psi. The pressure between the internal of the 9.625 diameter casing and the external of the 5.5 diameter production casing is 100 psi. I have asked the company to address the 475 psi on the 13.375 well head section and record the time it takes to blead the pressure off to a static level. The company will monitor and record the pressure every 24 hours in the proceeding days.
1520430475 2018-03-07 Production Wells No Violations On this day, I accompanied ODNR Inspector William Stolarik to witness a controlled annular pressure release of permit number 2-4551, into a tank. Please refer to his comments for a detailed report. We spoke with Antero company man Doug Stebbens about the process. The well is equipped with annular pressure monitoring devices. Concrete barriers surround the wellheads. The facility is gated with an electronic shutdown button on the left-hand side, prior to the pad. The front of the pad is equipped with (8) 400 barrel steel tanks within containment. There are (5) gas processing units along the back of the pad, and a midstream facility along the left side.

1672048189 2019-01-24 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (5) producing wells, (1) additional well that has had only the conductor drilled and set, (5) gas processing units (GPU), (8) 400-barrel steel production tanks within polymer lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Additionally, concrete barriers surround the perimeter of the wellheads. Identification and annular pressure monitoring devices are present. The 9.625 inch diameter intermediate casing has a pressure of 120 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 0 psi.
