0205740050 2015-01-14 Drill / Deepen / Reopen No Violations I am onsite at the request of Inspector Stolarik to investigate a drill cuttings spill on the lease road. Upon arrival I witnessed a Great Plains dozer and loader gathering contaminated soil. The company representative stated that the spill occurred at 9:15 PM 1-13-2015. The source of the spill was a Waste Management truck. The volume of the spill was estimated at 2 to 3 barrels. I instructed the company to haul all contaminated soil and gravel to a proper landfill.
0382018666 2015-11-24 Preliminary Restoration No Violations Status check inspection of the well head found the well equipped with a production tree and the well in line and producing into a gas processing unit. Proper annular pressure surface monitoring gauges are present at the well head. Also present on location supporting the 4 existing wells is a total of 4 gas processing units and a total of 4-400 barrel production tanks. Proper well identification is posted. Addressing the restoration, it was observed that the area affected by the well pad construction has been restored with erosion control and vegetation established.
1847156747 2018-02-26 Preliminary Restoration No Violations Preliminary restoration found the area affected by the well pad construction as described in the pre-site construction plans meets ORC 1509.072. The entrance of the lease road has signage posted identifying each of the 4 wells present on the well pad. The well pad entrance has an electrically controlled emergency shut down button on the right side of the lease road. The lease road is gravel and in good repair. As soon as you enter the well pad there are 4 steel tanks that are of steel construction and of 400 barrels capacity each. The tanks are situated inside a polymer lined steel containment dike area with an emergency shut down button at the loading site of the tanks. Situated beside the production facility is 4 gas processing units one supporting each well on the pad, each gas processing unit has an emergency shut down device present. In the center of the well pad is 4 well heads with the tubing valves open and annular pressure monitoring in place. Each well head is also equipped with an electrically controlled shut down device.
2009023626 2015-07-22 Completion Testing No Violations Status check inspection on the well completion process found Mercer Well Service workover rig on the location completing the Routh #2-B well with 4 total wells on the pad. The said well #1-A has had the completion plugs drilled out and the 2.875 production tubing ran into the well bore to a depth of 10,151 with the production well head now set in place on the well, upon completion of the 3 remaining wells on the pad the company will then put all 4 wells into production.
2141194409 2019-05-28 Production Wells No Violations On this day, I found the pad fenced and gated with signage at the entrance. An earthen berm surrounds the perimeter of the pad. There are (4) producing wells equipped with a plunger lift system, (4) gas processing units (GPU) with sand traps, and (4) 400-barrel and (1) 50-barrel steel production tanks within polymer lined steel containment. An Emergency Shutdown (ESD) button is present at the production facility, wellheads, and entrance. Additionally, concrete barriers surround the perimeter of the wellheads and GPUs. Identification and annular pressure monitoring devices are present at the wellheads. The intermediate casing has a pressure of 350 pounds per square inch (psi), and the surface casing has a pressure of 0 psi.
