0022578775 2019-01-24 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (5) producing wells, (1) additional well that has had only the conductor drilled and set, (5) gas processing units (GPU), (8) 400-barrel steel production tanks within polymer lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Additionally, concrete barriers surround the perimeter of the wellheads. Identification and annular pressure monitoring devices are present. The 9.625 inch diameter intermediate casing has a pressure of 2,200 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 20 psi. I contacted Doug Stebbins with Antero Resources, to notify him of the pressure.

Addressing the well status-the drilling process is now complete and the well is awaiting stimulation. A total of 8-400 barrel production tanks are in place inside a poly lined metal containment dike area. A total of 5 gas process separation units are in place to support each of the 5 existing wells on the pad.

0593089014 2016-01-04 Completion Testing No Violations Status check inspection on the well stimulation process found Baker & Hughes stimulation service currently stimulating stage 53 of 56 total stages to be completed with Horizontal Wireline Service on location conducting the perforating for this stage with 60 shots in 6 clusters of 10 shots administered on this stage between the depths of 9,349 and 9,498 with a solid completion plug set first at 9,512. The 5.5 production casing annular pressure is being monitored electronically during stimulation.
0693015416 2014-11-30 Casing String Witnessed No Violations The said well was spudded on 11/29/14 by Express Energys top hole rig with a 100 x 32 conductor borehole being drilled with the fresh water strata encountered at 76. 11/30/14-100 of 26 weld connection conductor was set and cemented by Express Energys cement service using 330 sacks of class A 2% calcium .25pps flake cement with 100% open hole cement excess ran and the cement displaced with 67 barrels of fresh water and good cement returns to surface. The cement mix water was tested for PH & Chlorides.
0711875249 2018-03-07 Production Wells No Violations On this day, I accompanied ODNR Inspector William Stolarik to witness a controlled annular pressure release into a tank. Please refer to his comments for a detailed report. We spoke with Antero company man Doug Stebbens about the process. It took approximately 15 seconds to bleed off the annular pressure. The well is equipped with annular pressure monitoring devices. Concrete barriers surround the wellheads. The facility is gated with an electronic shutdown button on the left-hand side, prior to the pad. The front of the pad is equipped with (8) 400 barrel steel tanks within containment. There are (5) gas processing units along the back of the pad, and a midstream facility along the left side.
0775537045 2016-01-21 Production Wells No Violations Status check inspection to document the well head working pressures. The 2.875 production tubing has 5,600 psi, the 5.5 casing has 6,185 psi internally and zero externally, the 9.625 has 300 psi shown as a constant pressure for the last 3 days.
1236968512 2015-01-14 Casing String Witnessed No Violations I am onsite to witness the surface casing installation and cementing operations. Dallas Morris Rig 24 drilled 395 of 24 diameter hole. The rig crew then installed 395 of 18.625 J-55 casing with a Texas shoe and a float collar. I performed a test of the cement mix water with a low range Quantab, the test indicated that the total dissolved chlorides were present at a concentration of 82 parts per million. Allied Oil and Gas Services pumped 775 sacks of class A cement with .125 pounds per sack flowseal and 2% CaCl into the casing with 59 barrels of the cement circulating back to surface. I verified the cement weight at 15.8 ppg and 15.6 ppg during the job. The plug was landed and the casing integrity test passed with no bleed off.
1270468778 2017-08-25 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction meets ORC 1509.072 with vegetation established and erosion control in place. The lease road is gated and identification signs are in place. There is emergency shut down devices at the well pad entry point and tank battery and also at the electrical service point for the gas processing units. The well is equipped with a casing and tubing head with flow lines attached to each well head outlet. A primary master valve is in place on the tubing head with (2) master valves present down stream in the production flow line. The casing head also has (2) master valves present. A gas processing unit is present on the well pad along with units supporting the other 4 wells on the pad. At the production facility a total of (8) steel construction 400 barrel capacity tanks are present inside a polymer lines steel containment dike area. Proper well head identification is posted at each well. Annular pressure monitoring gauges are present showing 3600 psi on the 9.625 diameter casing annulus and 1500 psi on the 13.375 . Doug Stebbins with Antero was notified.
1421600585 2015-01-18 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling progress found Dallas Morris rig # 24 on the hole with the 13.375 1st intermediate recently set to a depth of 2,219. The rig is currently picking up directional tools to run in and begin grilling out from under the 13.375 and begin their 12.250 section and well bore deviation.
1511069585 2018-03-07 Production Wells No Violations On site inspection along with ODNR Inspector Tom Thomas and Antero employee Doug Stebbins. The intention of the inspection is to discuss and review annular pressures. The said well was observed to be in production and to have annular pressure gauges present on the 13.375 casing well head section and 9.625 casing well head section. The pressure between the internal of the 13.375 and external of the 9.625 was 50 psi. The pressure between the internal of the 9.625 diameter casing and the external of the 5.5 diameter production casing is 1,500 psi. The pressure on the 9.625 well head section was blead off on this day down to a static point in less than 20 seconds. The company will monitor and record the pressure every 24 hours in the proceeding days. The 9.625 diameter casing well head pressure was blead down from 2,200 psi to a static level on 03/05/2018 and had built up 1,500 psi in 48 hours.
