0207203499 2014-12-07 Casing String Witnessed No Violations The said well was spudded on 12/06/2014 by Express Energys top hole rig service with a 32 x 100 conductor borehole being drilled. On 12/06/2014 I arrived on location @ 22:00 hours to witness the cementing of the 26 x 100 conductor pipe with Express Energy Cement Service on location to cement the casing using 330 sacks of class A-2% calcium .50 pps flake 100% open hole cement excess to be used, upon inspection of the mix water it was discovered that mist water had been put into the fresh water tank and a Quantab test showed over 700 parts per million chlorides. I then required the company to bring in fresh water and not use the water in that tank. The casing was then cemented with good cement returns witnessed to surface.
0353267194 2015-02-24 Drill / Deepen / Reopen No Violations Status check inspection on the drilling process found Patterson Drilling rig 340 on location with the 17.5 1st intermediate well bore total drilling depth reached at 2,215 and the rig completed pulling the drill string out of the hole. Wyoming casing service is on location to begin running the 13.375 casing. This well pad has well in production with the current drilling taking place. Proper containment is in place and all perimeter drains are in the closed position.
1025176196 2015-02-11 Drill / Deepen / Reopen No Violations Status check inspection on the drilling process found Sidewinder Rig #54 on location drilling the 12.250 2nd intermediate wellbore at a depth of 5,823 with a 11 degree directional nudge implemented in this section in the directional azimuth of 184 degrees.
1234474306 2018-03-02 Production Wells No Violations Status check inspection found the well pad entry point is gated and locked with signage posted. The lease road is graveled along with the well pad, both are in good repair. Addressing the well head - The well is equipped with a B section along with a 9.65 and 5.5 casing well head section and a tubing well head. The tubing well head has 2 manually controlled valves and one emergency shut down valve. Annular pressure monitoring is in place with 0 (psi) pound per square inch indicated between the 9.65 and 13.375 and 600 psi between the 5.5 and 9.625. Proper well identification is posted at the well head. Also on location is (4) gas processing units one for each well head on location. At the rear of the well pad is the production fluid storage area consisting of a total of 8 steel construction tanks all situated inside a polymer lined steel containment dike area. Emergency shut down devices are situated at the well pad entrance and at the tank battery loading site. 3 additional cellars were set on the pad but only one of the wells were spudded.
1377018632 2015-11-05 Completion Testing No Violations Status check inspection on the well completion process found US Well Service on location with 17 stages currently stimulated. Horizontal Wireline Service is on location and currently perforating the 18th stage with a completion plug set at 13,471 and 60 perforations shot in 6 clusters of 10 shots between the depths of 13,455 and 13,308. KLX Energy is the well head surface control company.
1422661096 2015-01-30 Casing String Witnessed No Violations 01/30/15 Sidewinder drilling rig #54 is now on the hole and has drilled a 386 x 24 surface borehole and ran 358.95 of 18.625 87.5# ppf J-55 casing with a guide shoe and float collar ran. Calfrac cement service is on location to cement the casing using 625 sacks of class A 3% calcium .25 pps flake with 70% open hole cement excess ran and the casing pressure tested to 500 psi over lift pressure upon the landing of the cement plug with good cement returns to surface and the cement mix water temperature-ph @ chlorides verified prior to cementing.
1437054231 2015-07-16 Drill / Deepen / Reopen No Violations Status check inspection on the well drilling process found Patterson Drilling rig # 340 on location with a 8.75 production borehole drilled to a depth of 7,543 with the previous 9.625 casing set to 7,343. Upon implementing a formation integrity test the company has discovered the previous 9.625 casing point was insufficient in depth per their well control protocol, with the formation integrity test to not allow a mud weight greater than 13#. To remedy the situation the company has CalFrac cement service on location and is spotting a 158 sack cement plug through the 5drill string in the open hole formation to seal off the formation.
1461247848 2016-04-21 Preliminary Restoration No Violations Upon inspection it was observed that the area affected by the well pad construction has erosion control and vegetation established and the site meets ORC 1509.072. Also noted the landowner-farmer has began to till the ground along the lease road in preparation of the planting process. Addressing the well head- The drilling and completion process is now compete and the well is in production with 4 dehydration units on location supporting each of the existing wells present. A total of 8-400 barrel capacity production tanks are present at a separate production pad at the rear of the well pad.
1646370668 2019-02-07 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (4) producing wells, (1) with conductor only, (4) gas processing units, (8) 400-barrel steel production tanks within polymer-lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 500 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 0 psi.
1925413390 2015-02-03 Drill / Deepen / Reopen No Violations My inspection today finds the drilling rig, Sidewinder 54, drilled a 17.5 borehole to a total depth of 2101. After pulling the drill pipe the rig intends to run 13.375 casing to bottom with bow spring centralizers.
2038153177 2016-02-17 Preliminary Restoration No Violations Preliminary Restoration and status check inspection to document the well drilling and completion process is now complete with the well now in line and capable of production. The well is in the completion flow back testing stages and is being flowed through 2 vertical sand trap separators and a gas processing unit. Addressing the production fluid storage tank pad which is adjacent to the well pad, upon inspection of the production fluid facility a total of (8) Eight -400 barrel capacity tanks are present in a polymer lined metal containment dike area with proper identification posted. Addressing the Preliminary Restoration- upon inspection it was observed that the area affected by the well pad construction has proper vegetation established with erosion control in place.
