0424875601 2014-12-28 Drill / Deepen / Reopen No Violations Status check inspection on the drilling process found Sidewinder drilling rig # 54 on location with a borehole deviation plug being set on 12/26/14 and allowed to cure. The rig has ran in the hole with a tri cone bit and tagged the top of the cement plug @ 2,010 and dressed the plug off down to 2,159 and pulled out of the hole with the tri cone and they are currently picking up directional tools to run in the hole to begin a directional borehole nudge around the 12.250 lost hammer assembly @ 2,387.
0465454400 2018-03-02 Production Wells No Violations Status check inspection found the well pad entry point is gated and locked with signage posted. The lease road is graveled along with the well pad, both are in good repair. Addressing the well head - The well is equipped with a B section along with a 9.65 and 5.5 casing well head section and a tubing well head. The tubing well head has 2 manually controlled valves and one emergency shut down valve. Annular pressure monitoring is in place with 0 (psi) pound per square inch indicated between the 9.65 and 13.375 and 1,000 psi between the 5.5 and 9.625. Proper well identification is posted at the well head. Also on location is (4) gas processing units one for each well head on location. At the rear of the well pad is the production fluid storage area consisting of a total of 8 steel construction tanks all situated inside a polymer lined steel containment dike area. Emergency shut down devices are situated at the well pad entrance and at the tank battery loading site. 3 additional cellars were set on the pad but only one of the wells were spudded.
0559148463 2016-02-17 Preliminary Restoration No Violations Preliminary Restoration and status check inspection to document the well drilling and completion process is now complete with the well now in line and capable of production. The well is in the completion flow back testing stages and is being flowed through 2 vertical sand trap separators and a gas processing unit. Addressing the production fluid storage tank pad which is adjacent to the well pad, upon inspection of the production fluid facility a total of (8) Eight -400 barrel capacity tanks are present in a polymer lined metal containment dike area with proper identification posted. Addressing the Preliminary Restoration- upon inspection it was observed that the area affected by the well pad construction has proper vegetation established with erosion control in place.
0659463099 2019-02-07 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (4) producing wells, (1) with conductor only, (4) gas processing units, (8) 400-barrel steel production tanks within polymer-lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 1,000 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 0 psi.
0669449845 2015-06-30 Drill / Deepen / Reopen No Violations Status check inspection on the current well drilling status found Patterson Drilling rig # 340 on location with the 8.750 well bore total drilling measured depth recently reached at 16,661 in the directional azimuth of 332.37 degrees with a borehole inclination of 90.98 degrees and true vertical depth of 8,399.45. The rig is currently pulling the 5 drill string in preparation to run the 5.5 production casing.

0912295382 2014-12-16 Casing String Witnessed No Violations I am onsite to witness the surface casing installation and cementing operations. Sidewinder Rig 54 drilled 378 of 24 diameter hole. The rig crew then installed 358 of J-55 18.625 casing. I performed a test of the cement mix water with a low range Quantab, the test indicated that the total dissolved chlorides were present at a concentration of 32 parts per million. Allied Oil and Gas Services pumped 695 sacks of class A cement with .25 pounds per gallon flowseal and 2% CaCl into the casing with 45 barrels of the cement returning to surface. The plug was landed and the casing integrity test passed with only minimal bleed off.
1417820183 2014-12-05 Casing String Witnessed No Violations The said well was spudded on 12/04/2014 by Express Energys top hole rig with a 100 x 32 conductor borehole being drilled and Express Energys cement service on location to cement the casing using 330 sacks of class A 2% calcium .25 pps flake with circulation established prior to cementing and 100% open hole cement excess ran with good cement returns to surface. No fresh water was encountered during the drilling of the borehole.

Also noted the landowner-farmer has began to till the ground along the lease road in preparation of the planting process. Addressing the well head- The drilling and completion process is now compete and the well is in production with 4 dehydration units on location supporting each of the existing wells present. A total of 8-400 barrel capacity production tanks are present at a separate production pad at the rear and adjacent to the well pad.
1667876178 2015-01-04 Drill / Deepen / Reopen No Violations Status check inspection of the current drilling process found Sidewinder Drilling rig 54 on location drilling the 12.250 2nd intermediate well bore at a depth of 6,829. The rig has pulled the drill string and is making repairs to the top drive assembly.
2064381648 2014-12-26 Plug / Plug Back No Violations On site inspection @ 5:30 am to witness a deviation plug set in the 12.250 section due to a lost hammer bit assembly at a depth of 2,387, with the top of the lost assembly marked at 2,351. Sidewinder Drilling rig # 54 is on the hole and has spotted the 4.5 drill string at a depth of 1,894 with the borehole dry. Allied cement services will spot a 68.5 bbl 319 sack Class A 3% calcium plug and displace with 25.3 bbl of fresh water. The 13.375 is set to 2,059 and the company will drill the cement out from under the 13.375 and deviate around the lost 12.250 assembly.
