0302950186 2017-12-29 Production Wells No Violations Status check inspection found the well head equipped with a casing and tubing well head with both the casing and tubing well head production valves open and feeding into a production sales line that leads to a gas processing unit on location near the well head. Annular pressure monitoring gauges are present on the well head. There is a total of 4 completed wells on the location along with 4 gas processing units and 8 -400 barrel capacity steel construction production tanks. The well pad has a earthen berm perimeter and the well pad surface is of stone construction. The lease road is gravel construction and in good repair.
0304793002 2015-10-21 Completion Testing No Violations My inspection today finds Fortis Well Service has drilled out plug number 43 and is tripping pipe down to plug number 44. The Antero representative stated that he anticipates finishing drilling out plugs tonight and moving the 3H tomorrow.
0539395798 2015-06-10 Drill / Deepen / Reopen No Violations Status check inspection of the drilling process. On this day I was accompanied by MRI-1 Austin Gutridge with the following observed. Patterson Drilling has moved back on the hole with the 9.625 casing previously being set. The rig has began drilling the horizontal well bore curve section at a depth of 7,889 and is currently building curve at a 6 degree inclination in the directional azimuth of334.92 degrees using a 12.3 # mud weight.
0840991098 2019-01-24 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (4) producing wells, (4) gas processing units (GPU), (2) 400-barrel steel production tanks within polymer lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Additionally, concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 0 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 140 psi.
0992249882 2015-05-11 Drill / Deepen / Reopen No Violations Status check inspection on the drilling process found Patterson Drilling rig #340 on location drilling the 12.250 intermediate well bore at a depth of 2,824. The company is implementing a 12 degree deviation in the directional azimuth of 187 degrees.
1277497173 2015-06-17 Drill / Deepen / Reopen No Violations Status check inspection on the well drilling process found Patterson rig 342 on location with the 8.750 production well bore total drilling depth recently reached on 06/15/2015 at a total measured depth of 16,090 in the directional azimuth of 330 degrees, the rig is currently running the 5.5 production casing at a depth of 16,090 and Halliburton Cement Service is scheduled to cement the casing this evening.
1427219629 2015-03-24 Casing String Witnessed No Violations The said well was spudded on 03/24/2015 by Rocky Mountain Drillings top hole rig with a 32 x 100 conductor borehole being drilled and 100 of 26 .375 wall weld connection conductor pipe being set. Express Energy Cement service is on location to cement the conductor with 330 sacks of class A-2% calcium .125 pps lcm 100% open hole excess cement with circulation established prior to cementing and good cement returns to surface.
1431098509 2015-05-08 Casing String Witnessed No Violations
1431693490 2015-05-15 Completion Testing No Violations Status check inspection on the drilling process found Patterson drilling rig # 342 on location with the 12.250 intermediate well bore drilled to a depth of 7,532with the borehole landed at a .35 degree inclination in the direction of 81.91 degrees azimuth, the rig currently is landing the last joint of 40# 9.625 casing to a depth of 7,532, Halliburton cement service is on location to cement the casing.
1448293422 2015-11-23 Preliminary Restoration No Violations Preliminary Restoration status check inspection finds the area affected by the well pad construction has been restored with erosion control and vegetation being established and in place. Addressing the well head-Upon inspection of the well it was observed that the drilling and well completion process is now complete with the well being flow tested and capable of production. The company has erected a tank battery consisting of a total of 8 tanks of 400 barrel capacity located inside of a polymer lined metal containment dike area. Proper well identification is posted.
1514722845 2017-07-17 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well site meets ORC 1509.072 with erosion control and vegetation established in the affected area of the well pad. There are (4) producing wells on the pad with the productin valves open and the wells selling to gas processing units with (4) units on location. Supporting the gas processing units are (8) steel production tanks that are 400 barrel cepacity with the tanks all situated inside a polyment lined steel containment dike area.
