0207551198 2019-02-07 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (4) producing wells, (4) gas processing units, (8) 400-barrel steel production tanks within polymer-lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 100 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 100 psi.
0257527430 2017-08-28 Preliminary Restoration No Violations Preliminary Restoration accompanied by ODNR Inspector Tom Thomas found the area affected by the well pad construction meets ORC 1509.072 with erosion control in place and vegetation established. Signage is posted at the well pad entrance and the location has a gated and locked entrance. Upon inspection of the well head the well was observed to be equipped with both a casing and tubing well head with a production flow line attached to the tubing well head. Also present is pressure gauges on the 5.5 casing well head and 9.625 casing B section of the well head with 220 psi showing on the 5.5 casing head and 190 psi showing on the 9.625 casing well head B section. At the production fluid storage facility a total of (8) tanks are present that are steel construction and 400 barrels total fluid capacity each. The tanks are situated inside a polymer lined steel containment dike area with a location master emergency shut down electric device located at the tank battery. Also present is (4) gas processing units one each servicing the (4) existing wells on location. A low pressure recovery system is also present with a total of (2) vertical burner unit present.
0454546378 2015-07-01 Drill / Deepen / Reopen No Violations My inspection today finds Calfrac and Patterson Drilling have circulated 1 steel tubing to a depth of 230 and they are unable to advance further. After consulting with Brent Bear the decision was made to allow the company to cement from that point to surface. Calfrac circulated cement from 230 to surface with 400 sacks of class A cement.
0461452231 2016-02-09 Completion Testing No Violations Status check inspection on the well completion process found US Well Service has completed stimulating the last of 51 stages to be complete and is rigging up on the Roxie# 2-H to begin the stimulation process. Bass Well head services is providing the completion well head control while Horizontal wireline services is providing the well completion perforating and plug setting. The stimulation equipment is located on polymer containment while the well site perimeter earthen dike serves as the primary well site boundary control. In conversation with the company men on site I did advise them of 1501:9-1-08 of the well construction rules, section (N) Annular pressure -paragraph (5) addressing annular pressure, The company men were notified annular pressures are to be monitored during stimulation.
0512365239 2018-03-02 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well site meets ORC 1509.072. The well pad entry point is gated and locked with signage posted. The lease road is graveled along with the well pad, both are in good repair. Addressing the well head - The well is equipped with a B section along with a 9.65 and 5.5 casing well head section and a tubing well head. The tubing well head has 2 manually controlled valves and one emergency shut down valve. Annular pressure monitoring is in place with 150 (psi) pound per square inch indicated between the 9.65 and 13.375 and 200 psi between the 5.5 and 9.625. Proper well identification is posted at the well head. Also on location is (5) gas processing units one for each well head on location. At the left front corner of the well pad is the production fluid storage area consisting of a total of 8 steel construction tanks all situated inside a polymer lined steel containment dike area. Emergency shut down devices are situated at the well pad entrance and at the tank battery loading site. 4 additional cellars were set on the pad but the wells never spudded.
0693136476 2017-08-28 Preliminary Restoration No Violations On this day I accompanied ODNR Inspector William Stolarik on site for a Preliminary Restoration inspection with four (4) producing wells on location. We found that the area affected by the well pad meets ORC 1509.072 with erosion control in place and vegetation established. Signage is properly posted at the well pad entrance and is gated and locked. There is an emergency shutdown switch located prior to entering the storage facility. The production equipment is made up of eight (8) 400 barrel steel tanks confined within steel dikes, four (4) gas processing units with one each per well head, two (2) low pressure vapor recovery units, and the tanks are properly identified.
1099819468 2015-07-30 Drill / Deepen / Reopen No Violations Status check inspection on the well drilling process found Patterson Drilling rig# 332 on the location currently drilling the 8.750 production borehole at a depth of 14,012 at a 91.002 degree inclination in the directional azimuth of 329. degrees. The well bore true vertical depth is 8,202 and the company is implementing managed pressure drilling using 12.7# mud weight holding back 3,414 psi stand pipe pressure with 7,250 units of 3rd party gas present.

The company is currently circulating out a gas influx at the circulating rate of 6 barrels per minute or 71 strokes per minute on the mud pump count with 9550 units of 3rd party gas present and 1,209 psi annular pressure and 2,559 psi stand pipe pressure present. The mud weight is 15.5# going in and 15.1# out of the well bore with the company just completing its 2nd circulation cycle.

1433194476 2015-06-01 Casing String Witnessed No Violations The said well was spudded on 05/31/2015 by Well Service Groups top hole rig service with a 100 x 30 conductor borehole being drilled and 100 of 30 conductor being set. 06/01/2015 Express Energy Cement Service cemented the casing using 235 sacks of class A -2% calcium .50 pound per sack loss circulation material and 100% open hole excess cement with good cement returns witnessed to surface and circulation established prior to cementing.
1435559801 2015-06-29 Casing String Witnessed No Violations Status check on site inspection at 12:00 AM to witness the 13.375 surface casing cement job. Patterson Drilling rig 332 is now on location and has drilled a 17.5 surface borehole to a depth of 1,198 and set 13.375 54.4# casing to bottom. CalFrac Well Service is on location to cement the casing using 1,195 sacks of class A cement with circulation established to surface prior to cementing and 20 barrels of gel spacer ran ahead of the cement. During cement displacement at a point 30 barrels into displacement a increase in displacement pressure was encountered and the casing began to lift out of the slips. The company men on site were advised that they would have to bond log and remediate accordingly.

With the 12.250 1st intermediate well bore drilled to a depth of 4,714 and the company currently running in the hole at a depth of 3,773 with 150 of 11.750wash over pipe to try and recover 148 of the bottom hole drilling assembly lost in the well bore. The rig is circulation the wash over assembly in the hole with 3% KCL water and the borehole direction is 16.92 degrees inclination in the directional azimuth of 232 degrees with the top of the fish at 4,714.

A kick plug was set on 07/12/2015 to allow the company to deviate around a lost 12.250 drilling assembly lost in the hole at a depth of 4,714 at a inclination of 15.91 degrees and directional azimuth of 229.88 degrees, with a top of the original drill pipe lost at 4,714 and the wash pipe lost at 4,652. A 535 sack class A plug was set through the drill string and was tagged up on at a depth of 3,717.
1461758585 2016-04-27 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction has been restored according to ORC 1509.072. Addressing the well status, the well drilling and completion stage is now complete and Warrior Energy Service is on location flowing the well back. With the following well support equipment present on location, 4 gas processing units one each for each well present on the pad, 8- 400 barrel capacity production tanks located inside a polymer lined steel containment dike area.
1746592994 2015-06-30 Casing String Witnessed No Violations Status check inspection on the current evaluation process of the recently set 13.375 surface casing string that encountered difficulties during the cement displacement process. On this day the cement was drilled out of the inside of the casing from the top of cement at 245 down to the casing float collar at 1,150. The well was then bond logged and the approximate cement top marked at 475, due to clarity issues with the casing bond log it was decided to run a 1 tubular string down the annular side of the 13.375 until the point of refusal or a minimum of 300 was reached. ODNR Supervisor Brent Bear was notified of the proposed action prior to commencement.
1827299725 2015-06-30 Drill / Deepen / Reopen No Violations My inspection today finds the drilling operator is attempting to wash in 1 steel tubing between the conductor and the 13.375 surface casing with CalFrac providing the pump services. CalFrac is pumping 2.3 barrels per minute. Remediation is necessary due to the backside of the 13.375 casing being filled up with cement cuttings from drilling out cement from inside the casing after a failed attempt to circulate cement to surface. At the time of my departure the 1 tubing has been circulated to a depth of 125.
1852737486 2015-08-12 Casing String Witnessed No Violations Status check inspection on the well completion process found Patterson Drilling rig 332 on location with the 8.750 production borehole total measured depth to be drilled recently reached at 17,625. The drill string is out of the hole and the rig is running the 5.5 production casing at a depth of 5,411. The company is controlling gas influx by keeping the well bore loaded with 17.1# drilling mud.
