Also present is pressure gauges on the 5.5 casing well head and 9.625 casing B section of the well head with 1500 psi showing on the 5.5 casing head and 550 psi showing on the 9.625 casing well head B section. At the production fluid storage facility a total of (8) tanks are present that are steel construction and 400 barrels total fluid capacity each. The tanks are situated inside a polymer lined steel containment dike area with a location master emergency shut down electric device located at the tank battery. Also present is (4) gas processing units one each servicing the (4) existing wells on location. A low pressure recovery system is also present with a total of (2) vertical burner unit present. |

The tubing well head has 2 manually controlled valves and one emergency shut down valve. Proper well identification is posted at the well head. Also on location is (4) gas processing units one for each well head on location. Emergency shut down devices are situated at the well pad entrance and at the tank battery loading site. 4 additional cellars were set on the pad but the wells never spudded. |

Patterson Drilling rig 332 was on location with the recently cemented 13.375 casing job on 08/21/2015 witnessed by Inspector Eric Conrad to of not had good cement returns to surface. A bond log was ran showing a estimated top of cement at 350. We than ran a 1 tubing string down the annular casing side until refusal at 160. The casing was then topped off by CalFrac cement service using 90 barrels of Class A -15.6# Cement with a measured weight of 15.6# returned to surface. I also addressed with the company safety representative on site the need for corrective action on the external electrical grounding on the Drilling rig and support equipment along with the Air booster units and soap tank, it was observed and documented through photographs that the electrical grounding was attached at multiple points with beam style clamps and not direct bolt ground lugs, it was also observed that the copper braided ground wires strands were either cut down or pulled apart only giving partial wire dimensional capacity connection, some wires were also connected with improper sized connectors and were loosely fastened. General housekeeping issues were also noted. |

There are a total of 4 wells on this pad with none live at this time. |
