0489575196 2016-04-27 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction has been restored according to ORC 1509.072. Addressing the well status, the well drilling and completion stage is now complete and Warrior Energy Service is on location flowing the well back. With the following well support equipment present on location, 4 gas processing units one each for each well present on the pad, 8- 400 barrel capacity production tanks located inside a polymer lined steel containment dike area.
0539557298 2015-06-03 Casing String Witnessed No Violations I am onsite to witness the conductor cementing operations. Express Energy Services cemented 100 of 26 weld pipe in place with 230 sacks of class A cement. I witnessed 20 barrels of returns to surface.
0635927846 2017-08-28 Preliminary Restoration No Violations Preliminary Restoration accompanied by ODNR Inspector Tom Thomas found the area affected by the well pad construction meets ORC 1509.072 with erosion control in place and vegetation established. Signage is posted at the well pad entrance and the location has a gated and locked entrance. Upon inspection of the well head the well was observed to be equipped with both a casing and tubing well head with a production flow line attached to the tubing well head. Also present is pressure gauges on the 5.5 casing well head and 9.625 casing B section of the well head with 1500 psi showing on the 5.5 casing head and 550 psi showing on the 9.625 casing well head B section. At the production fluid storage facility a total of (8) tanks are present that are steel construction and 400 barrels total fluid capacity each. The tanks are situated inside a polymer lined steel containment dike area with a location master emergency shut down electric device located at the tank battery. Also present is (4) gas processing units one each servicing the (4) existing wells on location. A low pressure recovery system is also present with a total of (2) vertical burner unit present.
0873026060 2018-03-02 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well site meets ORC 1509.072. The well pad entry point is gated and locked with signage posted. The lease road is graveled along with the well pad, both are in good repair. Addressing the well head - The well is equipped with a B section along with a 9.65 and 5.5 casing well head section and a tubing well head. The tubing well head has 2 manually controlled valves and one emergency shut down valve. Annular pressure monitoring is in place with 500 (psi) pound per square inch indicated between the 9.65 and 13.375 and 1,200 psi between the 5.5 and 9.625. Proper well identification is posted at the well head. Also on location is (4) gas processing units one for each well head on location. At the right front corner of the well pad is the production fluid storage area consisting of a total of 8 steel construction tanks all situated inside a polymer lined steel containment dike area. Emergency shut down devices are situated at the well pad entrance and at the tank battery loading site. 4 additional cellars were set on the pad but the wells never spudded.
1228036448 2015-08-27 Drill / Deepen / Reopen No Violations My inspection today finds the drilling rig, Patterson 332, is drilling 12.25 hole at a depth of 6340. The rig crew is preparing to trip the drill pipe out of the hole and replace the drill bit.
1440263439 2015-08-22 Casing String Witnessed No Violations On site inspection to witness the surface casing cement job annular top out. Patterson Drilling rig 332 was on location with the recently cemented 13.375 casing job on 08/21/2015 witnessed by Inspector Eric Conrad to of not had good cement returns to surface. A bond log was ran showing a estimated top of cement at 350. We than ran a 1 tubing string down the annular casing side until refusal at 160. The casing was then topped off by CalFrac cement service using 90 barrels of Class A -15.6# Cement with a measured weight of 15.6# returned to surface. I also addressed with the company safety representative on site the need for corrective action on the external electrical grounding on the Drilling rig and support equipment along with the Air booster units and soap tank, it was observed and documented through photographs that the electrical grounding was attached at multiple points with beam style clamps and not direct bolt ground lugs, it was also observed that the copper braided ground wires strands were either cut down or pulled apart only giving partial wire dimensional capacity connection, some wires were also connected with improper sized connectors and were loosely fastened. General housekeeping issues were also noted.
1452710850 2016-01-13 Completion Testing No Violations Status Check inspection on the well stimulation process found US Well Service Pressure Pumping Service on location currently stimulating stage 8-of 51 total stages to be completed with a total of 4 wells present that have been drilled and are to be completed on this location. Bass Well Head is providing well control with 10,000# rated well heads and completion iron present. Horizontal Wireline Company is on location and has perforated the current 8th stage that is being stimulated with 60 perforations shot in 6 clusters of 10 shots utilizing 90 degree phasing between the depths of 16,100 and 15,945 with a composite completion plug set at 16,116. All equipment on location that is supporting the stimulation process is located on secondary polymer containment with the primary earthen containment berm in place with the location drain valves in the closed position.
1452766861 2019-02-07 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (4) producing wells, (4) gas processing units, (8) 400-barrel steel production tanks within polymer-lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing gauge appears inoperable, and the 13.375 inch diameter surface casing has a pressure of 50 psi.
1456589347 2017-08-28 Preliminary Restoration No Violations On this day I accompanied ODNR Inspector William Stolarik on site for a Preliminary Restoration inspection with four (4) producing wells on location. We found that the area affected by the well pad meets ORC 1509.072 with erosion control in place and vegetation established. Signage is properly posted at the well pad entrance and is gated and locked. There is an emergency shutdown switch located prior to entering the storage facility. The production equipment is made up of eight (8) 400 barrel steel tanks confined within steel dikes, four (4) gas processing units with one each per well head, two (2) low pressure vapor recovery units, and the tanks are properly identified.
1457706680 2016-03-11 Completion Testing No Violations Status check inspection found the well stimulation process is now complete and a service rig owned by Fortis is rigged up on the hole drilling out the final completion plug of 51 that were set in the well bore. Great White company is on location and rigged up on the hole with their well control tubing snubbing unit. Bass Well Head is providing the main well head BOP stack control , the BOP stack and snubbing unit consist was function tested on 03/08/2016. EFS Company is providing flow back services. The service rig and support equipment is set up on secondary polymer containment.
1503354354 2015-09-11 Drill / Deepen / Reopen No Violations My inspection today finds the drilling rig, Patterson 332, drilled an 8.75 hole to a depth of 17415. The rig is running 5.5 casing at a depth of 5050.
1891372053 2015-08-21 Casing String Witnessed No Violations I am onsite to witness the surface casing cementing operations. Upon my arrival Patterson Drilling Rig 332 ran 1210 of 13.375 casing. CalFrac Well Services rigged up and pumped 1229 sacks of class A cement. Circulation was lost while the pump truck shut down to drop the wiper plug. Circulation could not be re-established. Displacement was cut short to ensure a good shoe. The company was directed to bond log the well and remediate accordingly.
2022591285 2016-01-13 Completion Testing No Violations On my inspection today accompanying MRI Bill Stolarik, we found current well stimulation in progress. Horizontal Wireline Service on location had perforated the current stage with 60 perforations shot in six clusters of ten shots using 90 degree phasing starting at 16,100 feet to 15,945 feet. A completion plug had also been set at 16,116 feet. During our inspection, USWS was stimulating the 8th of 51 total stages through 10k rated completion iron. Bass is providing the well control with wellheads also rated to 10k psi. On the well location, all equipment supporting the stimulation is in secondary containment with primary earthen berm surrounding the pad. There are a total of 4 wells on this pad with none live at this time.
