1019089980 2019-02-07 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (4) producing wells, (4) gas processing units, (8) 400-barrel steel production tanks within polymer-lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 0 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 450 psi.

1224300879 2016-04-27 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction has been restored according to ORC 1509.072. Addressing the well status, the well drilling and completion stage is now complete and Warrior Energy Service is on location flowing the well back. With the following well support equipment present on location, 4 gas processing units one each for each well present on the pad, 8- 400 barrel capacity production tanks located inside a polymer lined steel containment dike area.
1377073703 2017-08-28 Preliminary Restoration No Violations Preliminary Restoration accompanied by ODNR Inspector Tom Thomas found the area affected by the well pad construction meets ORC 1509.072 with erosion control in place and vegetation established. Signage is posted at the well pad entrance and the location has a gated and locked entrance. Upon inspection of the well head the well was observed to be equipped with both a casing and tubing well head with a production flow line attached to the tubing well head. Also present is pressure gauges on the 5.5 casing well head and 9.625 casing B section of the well head with 100 psi showing on the 5.5 casing head and 420 psi showing on the 9.625 casing well head B section. At the production fluid storage facility a total of (8) tanks are present that are steel construction and 400 barrels total fluid capacity each. The tanks are situated inside a polymer lined steel containment dike area with a location master emergency shut down electric device located at the tank battery. Also present is (4) gas processing units one each servicing the (4) existing wells on location. A low pressure recovery system is also present with a total of (2) vertical burner unit present.
1396251114 2017-08-28 Preliminary Restoration No Violations On this day I accompanied ODNR Inspector William Stolarik on site for a Preliminary Restoration inspection with four (4) producing wells on location. We found that the area affected by the well pad meets ORC 1509.072 with erosion control in place and vegetation established. Signage is properly posted at the well pad entrance and is gated and locked. There is an emergency shutdown switch located prior to entering the storage facility. The production equipment is made up of eight (8) 400 barrel steel tanks confined within steel dikes, four (4) gas processing units with one each per well head, two (2) low pressure vapor recovery units, and the tanks are properly identified.
1442371534 2015-09-15 Casing String Witnessed No Violations I am onsite to witness the surface casing installation and cementing operations. Patterson Drilling 332 drilled a 17.5 borehole to a depth of 1155. The drilling rig then ran 1154 of 13.375 J-55 casing with eight bow spring centralizers. CalFrac cemented the casing in place by circulating 258 barrels of cement into the annular space behind the casing. I witnessed

2129553249 2018-03-02 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well site meets ORC 1509.072. The well pad entry point is gated and locked with signage posted. The lease road is graveled along with the well pad, both are in good repair. Addressing the well head - The well is equipped with a B section along with a 9.65 and 5.5 casing well head section and a tubing well head. The tubing well head has 2 manually controlled valves and one emergency shut down valve. Annular pressure monitoring is in place with 0 (psi) pound per square inch indicated between the 9.65 and 13.375 and 425 psi between the 5.5 and 9.625. Proper well identification is posted at the well head. Also on location is (4) gas processing units one for each well head on location. At the right front corner of the well pad is the production fluid storage area consisting of a total of 8 steel construction tanks all situated inside a polymer lined steel containment dike area. Emergency shut down devices are situated at the well pad entrance and at the tank battery loading site. 4 additional cellars were set on the pad but the wells never spudded.
