0736036439 2015-10-10 Drill / Deepen / Reopen Violations Noted At 3:45 am Patterson Drilling rig 332 experienced a failure of the managed pressure drilling system packing rubber while circulating out a kick. As a result, an estimated 24 bbl of oil based mud escaped from the well. All but 1 to 2 bbl stayed in containment. Myers Well Service and Petta have been dispatched to clean the affected area. I have requested a written procedure before proceeding. Aaron Goddard was notified of the violation for pollution and contamination.
0792640740 2015-08-18 Casing String Witnessed No Violations I am onsite to witness the surface casing cementing operations. At the time of my arrival Patterson Drilling ran 454 of 18.625 casing. The rig crew struggled to land the casing in the style of landing head. After several hours they were able to land the casing and CalFrac finished rigging up their equipment. After twelve hours on duty I called Inspector Stolarik and requested his assistance.
0813845459 2017-08-28 Preliminary Restoration No Violations On this day I accompanied ODNR Inspector William Stolarik on site for a Preliminary Restoration inspection with four (4) producing wells on location. We found that the area affected by the well pad meets ORC 1509.072 with erosion control in place and vegetation established. Signage is properly posted at the well pad entrance and is gated and locked. There is an emergency shutdown switch located prior to entering the storage facility. The production equipment is made up of eight (8) 400 barrel steel tanks confined within steel dikes, four (4) gas processing units with one each per well head, two (2) low pressure vapor recovery units, and the tanks are properly identified.

0950421322 2015-10-13 Drill / Deepen / Reopen No Violations I am onsite to witness the conductor casing cementing operations. Express Energy Services drilled a 32 borehole to a depth of 100. Express then ran 100 of 26 weld casing. Express cemented the casing in place with 330 sacks of class A cement. I witnessed 22 barrels of cement circulate to surface.
0969344001 2015-10-11 Drill / Deepen / Reopen No Violations I am accompanied by Trisha Cartmell. My inspection today finds that the area adjacent to the well pad in the hay field is has been mowed and all of the grass clippings have been bagged and are awaiting transport to a collection box that will be hauled to an approved landfill. The clean up continues on pad as well, Petta and Myers are pressure washing the drilling rig and associated equipment. All fluids that are contaminated with oil based mud are being solidified on site to be hauled to an approved landfill. The company was directed to maintain a copy of all manifests for disposal of materials related to this release.
0975850454 2018-03-02 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well site meets ORC 1509.072. The well pad entry point is gated and locked with signage posted. The lease road is graveled along with the well pad, both are in good repair. Addressing the well head - The well is equipped with a B section along with a 9.65 and 5.5 casing well head section and a tubing well head. The tubing well head has 2 manually controlled valves and one emergency shut down valve. Annular pressure monitoring is in place with 100 (psi) pound per square inch indicated between the 9.65 and 13.375 and 400 psi between the 5.5 and 9.625. Proper well identification is posted at the well head. Also on location is (4) gas processing units one for each well head on location. At the right front corner of the well pad is the production fluid storage area consisting of a total of 8 steel construction tanks all situated inside a polymer lined steel containment dike area. Emergency shut down devices are situated at the well pad entrance and at the tank battery loading site. 4 additional cellars were set on the pad but the wells never spudded.
1328244180 2015-10-14 Drill / Deepen / Reopen No Violations I am onsite to witness the blow out preventer (BOP) testing. At the time of my arrival, I witnessed KLX rigging down the wireline truck. Once the wireline truck was clear, Patterson Drilling personnel replaced the variable pipe rams on the BOP stack. I witnessed Greenes Energy Group perform the pressure test on the variable pipe ram BOP, the annular BOP, and the blind ram. The rig is preparing to retrieve the bridge plug.
1404407411 2017-08-28 Preliminary Restoration No Violations Preliminary Restoration accompanied by ODNR Inspector Tom Thomas found the area affected by the well pad construction meets ORC 1509.072 with erosion control in place and vegetation established. Signage is posted at the well pad entrance and the location has a gated and locked entrance. Upon inspection of the well head the well was observed to be equipped with both a casing and tubing well head with a production flow line attached to the tubing well head. Also present is pressure gauges on the 5.5 casing well head and 9.625 casing B section of the well head with 100 psi showing on the 5.5 casing head and 220 psi showing on the 9.625 casing well head B section. At the production fluid storage facility a total of (8) tanks are present that are steel construction and 400 barrels total fluid capacity each. The tanks are situated inside a polymer lined steel containment dike area with a location master emergency shut down electric device located at the tank battery. Also present is (4) gas processing units one each servicing the (4) existing wells on location. A low pressure recovery system is also present with a total of (2) vertical burner unit present.
1423528634 2016-04-27 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction has been restored according to ORC 1509.072. Addressing the well status, the well drilling and completion stage is now complete and Warrior Energy Service is on location flowing the well back. With the following well support equipment present on location, 4 gas processing units one each for each well present on the pad, 8- 400 barrel capacity production tanks located inside a polymer lined steel containment dike area.
1439920952 2015-08-18 Completion Testing No Violations On site inspection to witness the surface casing cement job and releive Inspector Conrad. Patterson rig 332 is on location and has drilled a 24 surface borehole to a depth of 454 with 18.625 casing then being ran into the well to a depth of 543. CalFrac cement service is on location to cement the casing using 743 sacks of class A cement with 50% open hole cement excess ran and circulation established prior to cementing. The casing was pressure tested to 500 psi over lift pressure upon the landing of the cement plug with good cement returns to surface.
1444777809 2015-10-13 Drill / Deepen / Reopen No Violations

- upon my initial visit at 8:30 am the company had successfully controlled the well to a static level using 18 pound per gallon drilling mud and was pulling the drill string out of the hole at a depth of 7,200. Myers well service was on location pressure washing the equipment affected and recovering any standing fluid on the location surface. I left location at 9:30 am to attend to other matters and arrived back on location at 14:00 pm to find the company had removed the entire drill string and was laying down the bottom hole directional drilling assembly, at 16:30 pm KLX Energy wireline was now rigged up and they proceeded to set a retrievable 10,000# rated capacity bridge plug at a depth of 7,124by electric wireline. Once the wireline plug is set the company will then re-test the blow out preventer, Inspector Eric Conrad will be on site at 23:00 pm as my relief.

They are circulating on well to recover the 18.1 pound drilling mud and to gain a consistent fluid weight. The clean up has been completed. I received copies of the manifests for the drilling mud disposal.
1648248966 2015-10-23 Casing String Witnessed No Violations My inspection today finds the drilling rig is running 5.5 casing at a depth of 12,021. I instructed the company representative Eric May to notify me one hour before the cement job begins.
1670745351 2019-02-07 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (4) producing wells, (4) gas processing units, (8) 400-barrel steel production tanks within polymer-lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 100 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 0 psi.
2016931755 2015-10-23 Casing String Witnessed No Violations I am onsite to witness the cementing operation on the production casing string. Patterson Drilling installed 17,464 of 5.5 casing. Calfrac cemented the casing in place with 768 barrels of Class H cement. I witnessed cement weight verification at 15.3 and 15.2 on pressurized mud scales.
