0306170620 2019-01-14 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (8) producing wells, (8) gas processing units within steel containment, (8) 400-barrel steel production tanks within polymer lined steel containment, and an Emergency Shutdown (ESD) button is present. Additionally, concrete Jersey barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 500 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 240 psi.
1044773743 2015-07-17 Casing String Witnessed No Violations The said well was spudded on 07/16/2015 by Express Energys top hole rig wit a 32 x 100 conductor borehole being drilled. Express Energys Cement service cemented the casing using 330 sacks of class A-2% calcium .125 pound per sack cement with 10% open hole cement excess ran and circulation established to surface prior to cementing and good cement returns witnessed to surface.

1441319008 2015-09-03 Casing String Witnessed No Violations On site inspection to witness the surface casing cement job found the following- Patterson Drilling rig 343 is now on the hole and has drilled a 17.5 surface borehole to a depth of 1,343 and ran 13.375 casing to 1,343. Halliburton Cement service is in location to cement the casing using 1,275 sacks of HalCem 15.6# cement with circulation established prior to cementing and 50 barrels of 6% gel ran ahead of the cement, the casing was pressure tested to 500 psi over lift pressure upon the landing of the cement plug with 50 barrels of good cement returns witnessed to surface.
1457702003 2016-03-11 Completion Testing No Violations Status check inspection on the well completion process found US Well Service stimulation services on location with the 7th stage recently stimulated. Horizontal wireline service perforated the stage between the depths of 15,087 and 15,235 with 60 perforations in 6 clusters with a completion plug set at 15,249. Bass Well Head service is providing well control with 2 primary manual valves and one hydraulically activated valve, the well head lower base is a McClinton 10,000 psi. ROC company is providing the well head flow back services. Us Well service is operating with 15,000 psi rated completion iron and monitoring all well head annuli pressures electronically.
