0186201824 2017-06-06 Completion Testing No Violations On site inspection to check on the well completion process found US Well Service on location along with Horizontal Wireline Services. The company men on location reported that the well bore had construction failure and in phone conversation with Jon Black with Antero Resources it was reported that the 5.5 diameter production casing had a joint split vertically at 2,200 measure depth from surface. It was reported that a bond log was ran and the top of cement was marked at 2,400 with a cement stringer at 1,100. Initially the toe sleeve in the 5.5 casing was opened and at that point the integrity failure was discovered. A proposal is being prepared by Mr. Black to the effect of plugging the well. My direct Supervisor Brent Bear along with the primary county Inspector Austin Guthridge was notified of the defective well construction.
0306799125 2019-02-07 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (4) producing wells, (4) gas processing units, (8) 400-barrel steel production tanks within polymer-lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 300 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 0 psi.
0449070981 2017-02-21 Production Wells No Violations Production well status check inspection found the well drilling and stimulation process is complete and the well is in production. There is 4 gas processing units on location one associated with each well on the pad. There is a total of 8 production tanks each with a total capacity of 800 barrels. The lease road is in good repair and a Myers water truck was on location removing production fluid from the production tanks. No standing water was observed inside the tank battery or on location.
0812916690 2018-03-13 Production Wells No Violations On site inspection to document annular pressure on the well head. The 13.375 well head section has 0 pounds per square inch pressure and the 9.625 well head section has 325 pounds per square inch pressure. Crews were scheduled to be on site this morning to hook up flow back iron and flow test the Clara 4-H annular pressure to begin a time pressure study of the well. I spoke with Antero production supervisor Doug Stebbins who indicated they were running behind and would not be ready until the afternoon.
1224489394 2018-03-02 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well site meets ORC 1509.072. The well pad entry point is gated and locked with signage posted. The lease road is graveled along with the well pad, both are in good repair. Addressing the well head - The well is equipped with a B section along with a 9.65 and 5.5 casing well head section and a tubing well head. The tubing well head has 2 manually controlled valves and one emergency shut down valve. Annular pressure monitoring is in place with 0 (psi) pound per square inch indicated between the 9.65 and 13.375 and 350 psi between the 5.5 and 9.625. Proper well identification is posted at the well head. Also on location is (4) gas processing units one for each well head on location. At the right front side the well pad is the production fluid storage area consisting of a total of 8 steel construction tanks all situated inside a polymer lined steel containment dike area. Emergency shut down devices are situated at the well pad entrance and at the tank battery loading site. Antero Production Superintendent Doug Stebbins was contacted on this day and met with me on site to discuss annular pressure.
1260935034 2015-11-20 Drill / Deepen / Reopen No Violations I am onsite to witness the surface casing cement remediation. 1.900 internal coupler tubing was ran to 520. Calfrac circulated cement back to surface. The company was instructed to perform a casing integrity test, a bond log and a formation integrity test prior to drilling the next section.
1442609310 2015-09-18 Casing String Witnessed No Violations The said well was spudded on 09/17/2015 by Express Energys top hole rig with a 32 x 100 conductor borehole being drilled. 09/18/2015 the conductor was set and cemented by Express Energy Cement service using 330 sacks of Class A cement with circulation established prior to cementing and the cement displaced with 55 barrels of fresh water and good cement returns witnessed to surface.
1448004230 2015-11-20 Casing String Witnessed No Violations 11/20/15 Patterson Drilling rig 332 is now on location and has drilled a 17.5 surface borehole to a depth of 1,130 and ran 1,299 of 54.5# J-55 Casing with a float collar and guide shoe ran. CalFrac Cement service is on location to cement the casing using 1,301 sacks of class A-15.6# pound per gallon 1.19 yield cement with the cement mix water PH & Chlorides verified prior to cementing. The cement locked up 45 barrels into displacement of 199 total barrels, 50% cement excess was ran.

Addressing the 4 wells on location- The well drilling and completion process is now complete with casing and tubing well heads installed and a flow back company identified as Solid Oak on site flow testing the wells. A production facility is located on the West side of the well pad consisting of 8-400 barrel production tanks all located in a poly lined steel containment dike area.

Addressing the restoration of the area affected by the well pad construction; upon inspection it was observed that there is erosion control and vegetation in place according to ORC 1509.072.
