There are (4) producing wells, (4) gas processing units, (8) 400-barrel steel production tanks within polymer-lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 0 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 320 psi.
0890991380 2016-09-22 Preliminary Restoration No Violations Preliminary Restoration status check inspection accompanying Inspector Bill Stolarik. Addressing the restoration of the area affected by the well pad construction; upon inspection it was observed that there is erosion control and vegetation in place according to ORC 1509.072. There is 4 wells in production on the well pad with a total of 8 -400 barrel production tanks present.
0926082193 2016-01-13 Drill / Deepen / Reopen No Violations On my inspection today accompanying MRI Bill Stolarik, Patterson Drilling Rig 332 had reached total depth of 16,966 feet with the bit depth brought up to 16,945 feet in order to try to circulate out heavy lost circulation material. While at the site, the rig horn blasted to warn of higher pressure and to close the BOPs. A gas influx was then flared off before re-starting mud circulation.

There is 4 gas processing units on location one associated with each well on the pad. There is a total of 8 production tanks each with a total capacity of 800 barrels. The lease road is in good repair and a Myers water truck was on location removing production fluid from the production tanks. No standing water was observed inside the tank battery or on location.

1450736706 2015-12-21 Casing String Witnessed No Violations On site inspection to witness the surface casing cement job found Patterson Drilling rig 332 on the hole and they have drilled a 17.5 surface borehole to a measured depth of 1,329 and ran 1,329 of 13.375 -54.5lb/ft J-55 Buttress thread casing with a guide shoe and float collar ran and proper centralizer placement demonstrated. CalFrac Cement service was on location to cement the casing using 1,301 sacks of class A -15.6# - 1.18 yield cement with 50% open hole cement excess ran and 40 barrels of viscous spacer ran ahead of the cement. The casing was tested to 500 psi over lift pressure upon the landing of the plug with good cement returns witnessed to surface. The cement mix water was tested to verify good PH & Chlorides.
1451436374 2015-12-29 Drill / Deepen / Reopen No Violations On site status check inspection found Patterson Drilling rig 332 on location, the company has recently set and cemented the 9.625 1st intermediate casing string to a measured depth of 7,869. The rig then conducted a formation integrity test and began drilling the 8.5production borehole and were at a measured depth of 8,187 when the auto track directional drilling assembly failed, the rig has pulled the drill string and is currently switching out the bottom hole drilling assembly, once complete they will run in the hole with the drilling assembly and continue drilling the production borehole.
1451572562 2015-12-31 Drill / Deepen / Reopen No Violations Notification was received by the company man on site at 23:03 pm on 12/30/2015 that the company had experienced a well bore gas influx and has shut the Blow out preventer in and was flaring gas, also noted that a small amount of residual diesel fuel based drilling mud had been omitted out the flair stack and was contained onto the well location, the company reported crews were in root to location to recover the product of approximately 1 barrel total lost. 1520006479 2018-03-02 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well site meets ORC 1509.072. The well pad entry point is gated and locked with signage posted. The lease road is graveled along with the well pad, both are in good repair. Addressing the well head - The well is equipped with a B section along with a 9.65 and 5.5 casing well head section and a tubing well head. The tubing well head has 2 manually controlled valves and one emergency shut down valve. Annular pressure monitoring is in place with 350 (psi) pound per square inch indicated between the 9.65 and 13.375 and 2,000 psi between the 5.5 and 9.625. Proper well identification is posted at the well head. Also on location is (4) gas processing units one for each well head on location. At the right front corner of the well pad is the production fluid storage area consisting of a total of 8 steel construction tanks all situated inside a polymer lined steel containment dike area. Emergency shut down devices are situated at the well pad entrance and at the tank battery loading site. Antero Production Superintendent Doug Stebbins was contacted on this day and met with me on site to address the annular pressure issue.
1520946927 2018-03-13 Production Wells No Violations On site inspection to document annular pressure on the well head. The 13.375 well head section has 375 pounds per square inch pressure and the 9.625 well head section has 2000 pounds per square inch pressure. Crews with Superior Pressure Control were on site this morning to hook up flow back iron and flow test the annular pressure to begin a time pressure study of the well. Antero production supervisor Doug Stebbins was on site and the pressure was reduced on the 9.625 casing head taking 1 minute and 27 seconds to blead the pressure off to zero pounds per square inch. The last reported pressure reduction on the 9.625 from the Company was during November of 2017.
1739566816 2016-09-22 Preliminary Restoration No Violations Preliminary Restoration status check inspection accompanied by ODNR Division staff member Jimmy Wiles found the area affected by the well pad construction restored according to ORC 1509.072 with erosion control in place and vegetation established. Addressing the 4 wells on location- The well drilling and completion process is now complete with casing and tubing well heads installed and a flow back company identified as Solid Oak on site flow testing the wells. A production facility is located on the West side of the well pad consisting of 8-400 barrel production tanks all located in a poly lined steel containment dike area.
