0624987400 2016-09-22 Preliminary Restoration No Violations Preliminary Restoration status check inspection accompanying Inspector Bill Stolarik with the following being observed on this day. The affected area of the well site construction has been restored according to ORC 1509.072 with erosion control and vegetation being established, all ditch lines are rock lined and the well pad construction perimeter still has super silt fence and filter sock in place to be removed at a later date by the company. There is 4 wells on the well pad now in production with more well permitted and not complete as of the date of my inspection. Inspector Stolarik signed off on the restoration on his report for this day.
0683355457 2017-12-29 Production Wells No Violations Status check inspection found the well head equipped with a casing and tubing well head with both the casing and tubing well head production valves open and feeding into a production sales line that leads to a gas processing unit on location near the well head. Annular pressure monitoring gauges are present on the well head. There is a total of 4 completed wells on the location along with 4 gas processing units and 8 -400 barrel capacity steel construction production tanks. The well pad has a earthen berm perimeter and the well pad surface is of stone construction. The lease road is gravel construction and in good repair. Also 4 additional wells were spudded on the pad and the conductor casing only was set.
0708398793 2016-05-09 Completion Testing No Violations Status check inspection on the well bore completion process found US Well service rigging up to begin the stimulation process on the 4 existing wells on the pad. Allied Horizontal Wire Line Service is on location and has initiated a wire line bond log on each of the wells, also noted is the well bore toe key preparation work has been completed. All equipment on location is located on polymer containment and the well pad perimeter drainage valves are in the closed position and the company has a vacuum truck service identified as ECM recovering any product that may accumulate in the well pad sumps or on the location surface. Bass Well Head is the well head control company. The company Representative also indicated that they are conducting weekly gas alarm and emergency shut down drill for the location in case of a on site emergency during stimulation operations.
0781089841 2017-06-27 Production Wells No Violations Status check inspection found the well equipped with a production tree casing head and the production valve in the open position. Annular pressure monitoring gauges are present. At the production fluid storage facility a total of (8) steel tanks capable of holding 400 barrels of fluid each are present inside a polymer lined steel containment dike area. A total of 4 gas processing units are present on location one each for each existing well present on the pad.

1160363052 2015-12-23 Drill / Deepen / Reopen No Violations On this day I met on site with the rig supervisor and company safety representative along with the area supervisor for Express Energy, we discussed the upcoming spudding of 4 additional wells on the pad and the setting of the conductor casing strings on each well. The rig is slated to move in and begin drilling the holes on Monday 12/28/2015. Also note was Patterson Drillings rig # 343 that is currently drilling the 8.5 production borehole at a measured depth of 9,423 in the horizontal well bore section. The simultaneous operations procedure for 2 rigs on the pad was discussed and the company (Antero) does have company operational procedures in place.
1174021553 2019-01-24 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (4) producing wells, (4) additional wells that have had only the conductor drilled and set, (4) gas processing units (GPU), (2) 400-barrel steel production tanks within polymer lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Additionally, concrete barriers surround the perimeter of the wellheads. Identification and annular pressure monitoring devices are present. The 9.625 inch diameter intermediate casing has a pressure of 1,500 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 100 psi.

1246490940 2016-09-22 Preliminary Restoration No Violations Preliminary Restoration status check inspection accompanied on this day by ODNR Division Staff member Jimmy Wiles. Upon inspection of the well it was observed that the well drilling and completion process is now complete with a casing head and tubing production head installed. Warrior Energy service is on location with their flow back equipment monitoring the well flow back process. There is 4 wells now complete on the well pad and on line with 4 gas processing units in place. There is 8 -400 barrel capacity production tanks on location located inside a poly lined steel containment dike area. Addressing the restoration- The well site meets ORC 1509.072 with vegetation established and erosion control in place. The company also still has all perimeter filter sock and super silt fence in place at this time and will be removed at their discretion at a later date.
1449968647 2015-12-12 Casing String Witnessed No Violations On site inspection to witness the surface casing cement job found Patterson Drilling rig 343 is now on the hole and has drilled a 17.5 surface hole to a measured depth of 1,337 and ran 1,325 of 54.5lb/ft J-55 casing with the following casing equipment ran-a guide shoe and float collar along with 9 centralizers. Halliburton Cement Service was on location to cement the casing using 1300 sacks of class A-50% open hole excess cement with circulation established prior to cementing and 50 barrels of 6% gel Viscous spacer ran ahead of the cement and the casing pressure tested to 500 psi over lift pressure upon the landing of the cement plug with good cement returns witnessed to surface. The cement mix water was tested for PH & Chloride proper levels prior to cementing.
1451404052 2015-12-29 Casing String Witnessed No Violations On site status check inspection to witness the 5.5 production casing being ran into the well bore at a current measured depth of 14,334. The casing weight is 23 lb/ft and a casing guide shoe and float collar was ran and proper centralizer placement is being demonstrated. Patterson rig 343 is on location and Halliburton Cement service is being mobilized to cement the casing.

1870659500 2016-01-05 Drill / Deepen / Reopen No Violations On my inspection this day, accompanying ODNR Inspector Eric Conrad, we found KLX Wireline Service working with Patterson Rig 343 to free drill string stuck in the well bore. KLX had been successful retrieving their wireline tool that had also been stuck and was, at the time of this inspection, attempting to torque up and back out the drill string.
1950881190 2015-10-29 Casing String Witnessed No Violations The said well was spudded on 10/28/2015 by Express Energys top hole rig with a 100 x 32 conductor borehole being drilled and 100 of .375 wall 26 conductor being set. Express Energys Cement service cemented the casing using 330 sacks of class A cement with the cement mix water verified chloride free and PH of 7 and circulation established prior to cementing with the cement displaced with 46.45 barrels of fresh water and good cement returns witnessed to surface.
