0871634448 2018-01-17 Production Wells No Violations Status check inspection on the current well status found the well drilling and completion process now complete. The well is in production and the master valve open. Annular pressure monitoring is present and in place. Well identification is present on each of the (2) wells present on location. Each well is selling into a gas processing unit which is located on location. Also present on location is a production fluid storage area consisting of (2) steel construction 400 barrel capacity tanks situated inside a steel containment barrier with a polymer liner.
0903454032 2019-04-04 Production Wells No Violations On this day, I am accompanied by ODNR Inspector Rick Brown. I found the pad gated with signage at the entrance. There are (2) producing wells, (2) gas processing units, (2) 400-barrel steel production tanks within steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 700 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 0 psi. The production records are current.
1468091451 2016-07-09 Casing String Witnessed No Violations I am onsite to witness the conductor casing cementing operations. Express Energy Services (EES) drilled 100 of 36 borehole then installed 100 of 30 weld casing .375 wall. EES then mixed 275 sacks of class A cement with .25 pounds per sack Flowseal. I witnessed 12 barrels of cement circulate to surface.
1473701883 2016-09-12 Casing String Witnessed No Violations 09/12/2016 -On site inspection to witness the 20 surface casing cement job found Orion Drilling Polaris rig # 14 on location with a 26 surface borehole drilled to 425 and 400 of 20 buttress thred surface casing ran. Halliburton cemented the casing through the 5 drill string stung into a sing in float shoe. Displacement was 5 barrels of fresh water with 20 barrels of cemetn returns witnessed to surface.
1475181270 2016-09-29 Drill / Deepen / Reopen No Violations Status check inspection on the current well drilling process found Orion Drilling rig # 14 on location currently drilling the 12.250 2nd intermediate well bore at a measured depth 8,038 with a proposed total depth of 9,800. The rig is encountering difficulty with the down hole drilling bit assembly and has pulled their drill sting and is attempting to get back to bottom.
1502811960 2017-08-15 Work Over Reconditioning No Violations Status check inspection on the well completion process found a hydraulic work over snubbing unit rigged up on the hole owned by Deep Well Services. The well bore stimulation process is complete and the rig has drilled out the completion plugs in the well bore to a measured depth of 23,566. At this depth a wireline perforating gun was lost in the hole and the tubing snubbing unit is making a recovery run to fish out the lost perforating gun and will then finish drilling out the completion plugs. After that point the company will begin the initial flow back operation process. The company man on site was directed to record and report all annular pressures. The workover rig and support equipment is situated on polymer containment. Quail Well control completion stack is on the well head. Roc Water flow back services is standing by to flow the well back.
1692785138 2016-09-19 Drill / Deepen / Reopen No Violations Status check inspection on the current well drilling process found Orion Drilling Polaris rig # 14 on location with the 17.5 1st intermediate well bore total drilling depth recently reached at 2,220 and 2,206 of 13.375 ran and cemented back to surface by Halliburton Cement service with 35 barrels of cement returns recorded. The rig is testing the BOP stack and shall begin drilling the 12.250 intermediate section.
