Mr. Goodsell stated that 110 of drive pipe was drilled-in and set for this well on 2-6-16 through 2-7-16. HAD Drilling currently moving off location. Cellars are being installed today.
0870370142 2017-08-30 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well site meets ORC 1509.072 with vegetation established and erosion control in place. The well entrance has signage posted and is gated and locked with the well pad perimeter also fenced with a 8 chain link fence and locks also present on the well pad perimeter fence. There is emergency shut down devices at the well pad entrance the tank battery and gas processing units along with at the well heads. There are (2) well heads present on location with identification signage on both well heads. The wells are producing off the production tubing. There are annular pressure monitoring gauges on the 13.375 and 9.625 well head assemblies with 200 pressure on the 13.375 well head section and zero pressure on the 9.625 well head section. Also on location is the production fluid storage area consisting of (3) steel construction 400 barrel capacity production tanks located inside a steel constructed polymer lined containment dike area with proper identification posted. There is 2 gas processing units on location along with one vertical sand trap unit.
0963669862 2019-05-24 Production Wells No Violations On this day, I found the pad fenced and gated with signage at the entrance. An earthen berm surrounds the perimeter of the pad. There are (2) producing wells, (2) gas processing units (GPU), a compressor station, and (3) 500-barrel steel production tanks within polymer lined steel containment. An Emergency Shutdown (ESD) button is present at the production facility, wellheads, and entrance. Additionally, concrete barriers surround the perimeter of the wellheads. Identification and annular pressure monitoring devices are present at the wellheads. The 9.625 inch diameter intermediate casing has a pressure of 550 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 100 psi.
1456058678 2016-02-21 Casing String Witnessed No Violations I am onsite to witness the surface casing cementing operations. H & P Drilling drilled 1927 of 17.5 borehole. Then H & P Drilling installed 1897 of 13.375 fifty four pound per foot casing. CalFrac cemented the casing in place by circulating 1677 sacks of class A blend cement at a weight 15.8 pounds per sack. Circulation was lost for approximately 80 barrels during the job, but was regained. I witnessed 40 barrels of cement circulate to surface. The Casing integrity test (CIT) was performed immediately after landing the wiper plug by pressuring up 500 psi over landing pressure. The CIT was passed.

1457026872 2016-03-03 Casing String Witnessed No Violations Status check on site inspection to witness the 9.625 casing string installation. H&P rig 521 is on location and has drilled a 12.250 intermediate well bore to a measured depth of 9,070 with the total depth being reached on 02/29/2016. The company on their 1st attempt was unsuccessful in getting casing in the hole past 2,900, on this day they were currently running casing between 3,400 and 5,000 while I was on location. The well bore was loaded with 10lb/gl brine and remaining static.

1657155483 2017-08-30 Preliminary Restoration No Violations On the day of inspection I accompanied ODNR Inspector William Stolarik and found that the wellsite meets ORC 1509.072 with vegetation established and erosion control in place. Emergency shutdown down controls are located at the gate entrance, between well heads, and around the tank facility. There are (2) well heads properly identified, production equipment consisting of (3) 400 barrel steel tanks, (2) gas processing units, and (1) vertical separator.
