0512086975 2016-12-27 Casing String Witnessed No Violations Status check inspection to witness the surface casing cement job. Well service group rig # 1 is on location and has drilled a 24 surface borehole to a measured depth of 548. The rig has ran 517 of 20 x 94 lb/ft buttress thread casing with a Halliburton double float valve casing shoe and centralizers on every casing collar. Halliburton Cement Service cemented the casing through the drill string stung into the double valve float valve shoe using 870 sacks of class A cement by first establishing circulation to surface and then pumping 40 barrels of 6% gel space followed by a 10 barrel fresh water spacer and then the 192 barrel cement mix completed by a 6 barrel H2O displacement with cement returns witnessed to surface.
0763671987 2016-12-16 Casing String Witnessed No Violations I was on location today to witness 30 inch diameter conduction casing being set by Formation Cementing Inc inside a 36 inch diameter bore hole. When I arrived on location Formation was not there yet. When they did arrive on location they had forgot to bring sugar with them for the vacuum trucks. The Company man on site Vincent Dwyer ran into town to get some. Once Vincent came back Formation began to pressure test in order to pump the cement. Formation ran a freshwater gel mix and freshwater ahead of the cement. Formation used 275 sacks of class A cement with 600 pounds of accelerant and 60 pounds of cottonseed hulls. The cement weighed 15.6 parts per gallon and had a yield of 1.18. Formation circulated about 14 barrels of cement back to surface.

0898495229 2018-05-22 Production Wells No Violations On site inspection found the well drilling and completion process now complete and well in production. The well is selling off the 5.5 diameter production casing through a primary 7.187 master valve with 2 additional in line manual and 2 hydraulic shut down valves in the production flow line. The company has annular pressure monitoring in place with riser pipes and gauges on the 13.375 and 9.625 well head sections. The 13.375 casing well head section gauge showed 200 pounds per square inch (psi) and the 9.625 well head section had 0 psi. Also on site is a total of 3 gas processing units one for each well on the pad along with a production facility consisting of 2 steel construction 400 barrel production tanks situated inside a polymer lined steel containment area. With the wells in the initial flow back stages he company has an additional 8 steel 500 barrel tanks that are being used to capture the flow back fluid from the wells, the 3 wells in production is generating approximately 800 barrels of fluid total each day. The preliminary restoration is also due- no earthen slips or deficiencies was observed on this day, vegetation is present throughout the disturbed area depicted on the initial construction design. The lease road is in good repair and consists of a gravel surface. The well pad entry point is gated with signage present. I will follow back up on the restoration later in the year and re confirm proper vegetation is still present. I also called the company and asked them to address the mouse holes used in the drilling process that have not been plugged.
1481209351 2016-12-08 Casing String Witnessed No Violations On site inspection to witness the conductor casing cement job with Express Energys top hole rig on location and a 36 x 100 conductor casing being set and 100 of 30 .375 wall conductor being ran. Express Energys cement service cemented the casing using 275 sacks of class A cement displaced with 74 barrels of fresh water with cement returns witnessed to surface at 54 barrels into displacement.
