1483136117 2016-12-30 Casing String Witnessed No Violations On site inspection to witness the surface casing cement job at 13:30 Hours. Well Service Group rig # 1 is on location and has drilled a 26 diameter surface borehole on air with a hammer bit to a measured depth of 546 and ran 516 of 94 pound per foot 20 buttress thread casing with a double valve Halliburton float shoe casing equipment and bow string centralizers on every collar. The drill string was stung into the float shoe and upon attempting to begin circulation the Halliburton Cement Service operator indicated he was unsuccessful in being able to circulate out the bottom of the casing shoe with pressure increasing to 500 psi. The rig pulled the drill string and attempted to pull the casing off bottom unsuccessfully, they company then attempted to load the casing and work the drill string while pressuring up on the casing string. The drilling rig blew a hydraulic line at 15:15 hours while attempting to pull the casing string. I left location at 18:00 hours and directed the company man to notify me once they resumed operations. The job was pumped on the morning of 12/31/2016 after the company pulled the casing string and found the shoe track plugged up.

2138897288 2018-05-22 Production Wells No Violations On site inspection found the well drilling and completion process now complete and well in production. The well is selling off the 5.5 diameter production casing through a primary 7.187 master valve with 2 additional in line manual and 2 hydraulic shut down valves in the production flow line. The company has annular pressure monitoring in place with riser pipes and gauges on the 13.375 and 9.625 well head sections. The 13.375 casing well head section gauge showed 100 pounds per square inch (psi) and the 9.625 well head section had 0 psi. Also on site is a total of 3 gas processing units one for each well on the pad along with a production facility consisting of 2 steel construction 400 barrel production tanks situated inside a polymer lined steel containment area. With the wells in the initial flow back stages he company has an additional 8 steel 500 barrel tanks that are being used to capture the flow back fluid from the wells, the 3 wells in production is generating approximately 800 barrels of fluid total each day. The preliminary restoration is also due- no earthen slips or deficiencies was observed on this day, vegetation is present throughout the disturbed area depicted on the initial construction design. The lease road is in good repair and consists of a gravel surface. The well pad entry point is gated with signage present. I will follow back up on the restoration later in the year and re confirm proper vegetation is still present. I also called the company and asked them to address the mouse holes used in the drilling process that have not been plugged.
