0122198255 2018-05-02 Production Wells No Violations Status check inspection found There is 3 wells now drilled and producing on the pad. Each well is producing off the production tree and the production valves are in the open position, the production flow line valve shows 2,000 psi. There is annular pressure monitoring in place, the 13.375 well head section has 20 psi on the gauge and 400 psi on the 9.625 well head section. The well is equipped with an emergency shut down button that is controlled from multiple point on the well pad. There is a production facility present consisting of (4) steel 400 barrel tanks situated inside a polymer lined steel containment area.
0556908771 2018-06-05 Production Wells No Violations On this day, I found the well equipped with annular pressure monitoring devices. The 13.375 inch diameter surface casing pressure gauge is reading 25 pounds per square inch, and the 9.625 inch diameter intermediate casing pressure gauge is reading 200 pounds per square inch. The facility is gated with proper signage and an electronic shutdown button at the entrance. There is a midstream facility at the front left-side of the pad, prior to the gate. (3) gas processing units are located at the front right-side. There are (3) wells in production on this pad, all with electronic shutdown buttons. All wells have concrete barriers surrounding them. The wellheads are properly identified. There are (4) 400 barrel steel tanks within containment, at the back right-side, with an electronic shutdown button.
0600453009 2016-12-08 Drill / Deepen / Reopen No Violations Status check inspection on the 9.625 diameter round casing wireline logging evaluation to determine the annular cement top. KLX Energys wireline truck is on location and has ran multiple logs on the well with the following logs noted -3 temperature logs were ran one with the casing annular shut in, one with the casing annular flowing, one with 1,800 psi on the annular side of the 9.625 and one with pressure on the casing internal, a fourth log was ran with the well shut in identified as a Gamma Ray CCL log with the cement top indicated at 7,900. The company also attempted to get the 9.625 annular to take fluid unsuccessfully with no rate achieved.

1090649937 2017-01-24 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area disturbed within the well pad limits of disturbance has been restored according to ORC 1509.072 with erosion control in place and vegetation established. The wood chip filled filter sock used around the well pad perimeter to control erosion is still in place. The outward slopes of the well pad are stable and the lease road is in good repair, the well pad entrance is gated and signage is posted. Addressing the 4 wells present on the pad, the well drilling is complete and the wells are awaiting stimulation. No production equipment is present.

On this day a retrievable bridge plug was set off the drill string at 7,815 and pressure tested to 2,700 psi at surface. KLX wireline was on location and rigging up to run a casing temperature log.
1531962708 2016-11-22 Casing String Witnessed No Violations On site inspection to witness the surface casing cement job with H&P Drilling rig 628 on the hole and Stingray cement service cementing the 13.375 surface string with a 17.5 well bore drilled to 1,354 and 54.5 lb/ft J-55 ran to 1,351 with a float shoe and collar casing equipment along with proper centralizer placement demonstrated. Stingray cemented the casing by first establishing circulation and then pumping 25 barrels of 6% gel and then 256 barrels or 1,200 sacks of class A cement with 2% calcium chloride and .250 lb/sk loss circulation material additives, the cement was displaced to 1,313 using a wiper plug chased with 203 barrels of recycled fluid and the casing pressure tested to 500 psi over cement displacement pressure upon the landing of the cement plug with cement returns witnessed to surface inside the cellar.
