0334776241 2018-07-30 Preliminary Restoration No Violations Preliminary Restoration status check inspection found vegetation established in the area affected by the well pad construction. There has been disturbance around the well pad where pipe lines have been installed that will require the vegetation to be established in those areas before the restoration can be signed off. The lease road and well pad surface does have a final grade of gravel present. Addressing the wells present on the pad. A total of 6 wells are present and to date only the drilling process has been completed on all 6 wells. The well are equipped with a lower A section and upper B section with a base plate and 2 master valves on top of the B section to allow access to the inside of the 5.5 diameter production casing. 2 diameter riser pipes are present on both the A & B well head sections with gauges present to monitor annular pressure. The pressure between the 5.5 diameter production casing and 9.625 is zero pounds per square inch (PSI) and the pressure between the 13.375 and 9.625 is zero psi. Other than the midstream valve station no production equipment has been installed on the well pad.

0983378790 2019-02-13 Completion Testing No Violations On this day, I found the stimulation process has been completed and the wells are being prepared for drill-out, which is projected to commence on 2/25/19. I spoke with Gulfport Company Representative Aaron Nero onsite, and he stated that 5 of 6 wells were stimulated. Permit number 4676 (5A) was not hydraulically fractured during the stimulation process of the additional wells on the pad.
1254915384 2019-01-15 Completion Testing No Violations On this day, I am conducting an inspection during the completion process, and found Halliburton Pressure Pumping Service onsite along with Schlumberger Wireline Service. They are zipper fracing between permit numbers 4688 (6B) and 4680 (1B). The pumping operation is currently down due to pump trucks being swapped out on location. I spoke with Gulfport Company Man Dan Staats during the inspection. They just perforated stage 18 of 33 with 40 shots, divided into 5 clusters, and are preparing to frac the stage. The frac plug is set at a measured depth of 13,764 feet. Mr. Staats stated that annular pressures for the 13.375 inch diameter casing and 9.625 inch diameter casing are monitored and logged daily.
1412576868 2017-01-25 Drill / Deepen / Reopen No Violations On the day inspection I witnessed that Lightning Energy Services had drilled a 36 inch diameter hole to the depth of 110 feet, hitting water at 80 feet and ran 110 feet of 26 inch diameter 85.6 pounds per foot conductor pipe. Stingray Pressure Pumping was onsite pumped 5 barrels of fresh water, performed a 500 pounds per square inch pressure test, gained circulation with 68 barrels of fresh water, pumped 15 barrels of gel, pumped 10 barrels of fresh water and cemented pipe with 440 sacks Class A cement with 2% CaCl and .5#/sk. of floseal weighing 15.6 pounds per gallon and a yield of 1.2 cubic foot per sack. Cement was weight was verified with pressurized mud scales and displaced to 100 feet with 62.5 barrels of fresh water with good cement returns of 20 barrels to surface. Mix water was tested revealing chlorides of 50 parts per million, pH 7 and 73 degrees. Simultaneous operations were ongoing with Pioneer Drilling Rig #73 drilling their lateral on the 34111246760000 Potter 5A. I completed walk around with Gulfport Consultant Nick Berrelsman.
1487076136 2017-02-01 Casing String Witnessed No Violations I was on location to witness O-Tex cement the 13.375 inch diameter casing into a 17.5 inch diameter bore hole. Pioneer drilled a 1266 foot 17.5 inch diameter bore hole and set 1266 feet of 13.375 inch diameter casing into the hole. O-Tex ran 50 barrels of fresh water gel ahead of the cement. O-Tex then ran 1150 sacks of class H cement, 2% CaCL, .6% C-16A, .2% X-Air and .25 parts per sack of floseal with 245.8 barrels of fresh water at 15.6 pounds and a yield of 1.18. O-Tex then circulated with 186 barrels of recycled water until we got cement to surface. O-Tex circulated about 55 barrels of cement to surface and finished cementing the casing into the bore hole.
1526734983 2018-07-30 Casing String Witnessed No Violations On this day, I accompanied ODNR Inspector William Stolarik. Please reference his comments for a detailed report. The drilling process has completed and the wellhead has been plumbed. No flowlines or production equipment are onsite. The wellhead has annular pressure monitoring devices attached. The well is awaiting completion.
