0011735743 | 2017-06-06 | Preliminary Restoration | No Violations | Preliminary Restoration status check inspection found the affected area depicted in the pre construction drawings has vegetation established and erosion control in place meeting ORC 1509.072. Signage is posted at the well pad entrance. Crews are on site erecting a polymer lined steel containment tank battery dike area. Baker Hughes pressure pumping service is on location and has assisted in the well bore toe prep operations. US Well service is slated to mobilize in the next 48 hours onto location to begin the well stimulation operation with a 6 existing wells on location slated to be stimulated. The well head assembly consists of a Cactus well head completion stack made up of a single Hydraulic Master Valve and Manual Master Valve sitting on top of the B-Section 10,000 psi rated lower well head assembly. |
0378944378 | 2018-05-14 | Production Wells | No Violations | Status check inspection found a total of 6 wells on the pad all in operation and producing. There is emergency shut down devices on both the tubing and well head production outlet flow lines with manual valves also present. Annular pressure monitoring gauges are present on the 13.375 and 9.625 well head sections with flow lines attached to each annular port leading to on site vapor recovery system. The wells are still flowing through sand trap separators before the product enters the gas processing units. Also on site is a 3rd party gas processing and distribution system. |

1256723082 | 2017-02-06 | Casing String Witnessed | No Violations | On site inspection to witness the surface casing cement job with H&P Drilling rig 519 on location along with Schlumberger Cement Service. H&P has drilled a 17.5 diameter surface borehole to a measured depth of 1,269 and ran 1,264 of 13.375 diameter 54.5 lb/ft J-55 Buttress thread casing with a guide shoe and float collar ran. Schlumberger Cement Service cemented the casing by first establishing circulation to surface followed by 30 barrels of 6% gel spacer then completed the job by pumping 243 barrels or 1,145 sacks of class A cement with 1% calcium additives, upon the completion of pumping the cement slurry a cement plug was deployed from the cement head and the cement was displaced using 189.7 barrels of fresh water. Upon reaching total displacement the cement plug was landed in the float collar and the casing pressure tested to 500 psi over cement displacement lift pressure. Cement returns were witnessed to surface inside the cellar. |
1349730926 | 2017-11-01 | Preliminary Restoration | No Violations | Status check inspection found the well drilling and completion process finished with the well equipped with a production tree and the production valve open. The tin horn cellars and mouse holes have been filled in. There is a total of 6 complete active wells on the pad all in production. There is also a total of 8 production tanks all of 400 barrel capacity each. There is 2 vapor recovery burner units attached to the tank battery and 6 gas processing units on location also. The location signage is present at the entry point and there is also a gate across the lease road. The well pad location and lease road is of stone construction and in good repair. The well pad has a total of 6 outlet drain catch basins that are in the closed position. Addressing the restoration- the well site meets ORC 1509.072. |
2020417237 | 2016-12-13 | Casing String Witnessed | No Violations | I arrived on location to witness Express Energy set a 26 inch diameter conductor pipe into a 38 inch diameter hole. Inspector Bill Stolarik called me and informed me that morning the job was canceled due to Antero unable to get water trucks on site. As I was going to do another inspection Bill called me again and informed me that Antero had the trucks show up on site and they began to pump the cement already. When I arrived on site they had already began pumping the cement into the casing. Prior to me arriving there Express had a hydraulic line break and disabled one of their pumps. Express was able to pump all the cement they needed to into the casing prior to that happening. Express pumped 14 barrels of freshwater ahead of the cement as a spacer. Express then pumped 435 sacks of class A cement using 54.48 barrels of water down hole. Express then displaced the cement using 77.41 barrels of freshwater. 20 barrels of cement was returned to surface during circulation. The cement had weighed 15.6 parts per gallon and had a yield of 1.18. After Express had finished pumping the cement they began repairs on their truck to pump cement for another conductor on site. Inspector Bill Stolarik came and relieved me from the site before they were able to finish the repairs. |
2125897392 | 2017-07-31 | Completion Testing | No Violations | On sitte inspection to withness the perforating of stage 2 out of 44 total stages to be completed. Allied Horizontal Wireline perforated the stage with 60 perforations in 6 clusters of 10 shots between the depths of 18,347 and 18,515. A completion plug was set at 18,531. US Well Service is beginning the stimulation of the stage. All stimulation equipment is placed on polymer containment. |
