0197375682 2017-02-15 Casing String Witnessed No Violations The purpose of my inspection is to witness the 13.375 inch diameter surface casing being cemented within a 17.5 inch diameter borehole that is 1270 feet in depth. A pre-job safety meeting was held with all personnel on location including Schlumberger, H&P, and Antero. Schlumberger pumped 200 barrels of fresh water, 30 barrels of gel, 10 barrels of fresh water, and 242 barrels of cement. The cement was Class A cement weighing 15.6 pounds per gallon. A cement sample was pulled and weighed in pressurized scales confirming the cement weight of 15.6 pounds per gallon. The plug was dropped and 190 barrels of fresh water was used for displacement. An estimated 33 barrels of cement returns were established.
0596434314 2017-06-06 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the affected area depicted in the pre construction drawings has vegetation established and erosion control in place meeting ORC 1509.072. Signage is posted at the well pad entrance. Crews are on site erecting a polymer lined steel containment tank battery dike area. Baker Hughes pressure pumping service is on location and has assisted in the well bore toe prep operations. US Well service is slated to mobilize in the next 48 hours onto location to begin the well stimulation operation with a 6 existing wells on location slated to be stimulated. The well head assembly consists of a Cactus well head completion stack made up of a single Hydraulic Master Valve and Manual Master Valve sitting on top of the B-Section 10,000 psi rated lower well head assembly.
1481903485 2016-12-14 Casing String Witnessed No Violations Initial call out to witness the conductor casing cement job at 7:00 pm on 12/14/2016. While enroot to the location at 8:00 PM the company man notified me that the cement contractors had not arrived and were reporting a delayed arrival of at least 4 hours. I notified my supervisor Brent Bear of the delay and the decision was made to not attempt to witness the job due to the time delays and sub zero temperatures possibly causing further delays. The cement job did not commence until 1:30 am on 12/15/16. 135 of 26 conductor was set inside a 32 hole and cemented to surface.
1485733545 2017-11-01 Production Wells No Violations Status check inspection found the well drilling and completion process finished with the well equipped with a production tree and the production valve open. The tin horn cellars and mouse holes have been filled in. There is a total of 6 complete active wells on the pad all in production. There is also a total of 8 production tanks all of 400 barrel capacity each. There is 2 vapor recovery burner units attached to the tank battery and 6 gas processing units on location also. The location signage is present at the entry point and there is also a gate across the lease road. The well pad location and lease road is of stone construction and in good repair. The well pad has a total of 6 outlet drain catch basins that are in the closed position.
1492167820 2017-02-15 Casing String Witnessed No Violations During my inspection I met with Antero Company man Tommy. Tommy and I went over the cement proposal for the 13.375 inch diameter surface casing. Tommy stated that they will be cementing the 13.375 inch diameter surface casing within a 17.5 inch diameter borehole at a depth of 1270 feet. During my inspection the HP Drilling Rig 519 was running the 13.375 inch diameter surface casing.
1691092660 2017-02-15 Drill / Deepen / Reopen No Violations 7:30 am Status check inspection on the well drilling process found H&P Drilling rig 519 on location and has drilled a 17.5 diameter surface borehole to a measured depth of 1,280. I have been on will call stand by and delayed for 24 hours awaiting the 13.375 surface cement job due to the rig having problems with the air body lower sealing leaking between the air body and conductor casing and having to be repaired. The 13.375 diameter surface casing cement job is now scheduled for 13:00 Hours on this day Inspector Austin Guthridge is on will call stand by to cover the job.
1800665871 2017-07-31 Completion Testing No Violations Status check inspection on the well completion process found Allied Horizontal Wireline Service preparing to hav the wireline gun pumped down the hole by US Well Service to perforate stage 8 of 39 total stages to be stimulated in the well. The well will be perforated with 6 clusters of 10 shots between the depths of 16,590 and 16,423 with a composite completion plug set in the well bore at 16,608. All support equipment on location is placed on polymer containment.

2096359097 2018-05-14 Production Wells No Violations Status check inspection found a total of 6 wells on the pad all in operation and producing. There is emergency shut down devices on both the tubing and well head production outlet flow lines with manual valves also present. Annular pressure monitoring gauges are present on the 13.375 and 9.625 well head sections with flow lines attached to each annular port leading to on site vapor recovery system. The wells are still flowing through sand trap separators before the product enters the gas processing units. Also on site is a 3rd party gas processing and distribution system.
