Signage is posted at the well pad entrance. Crews are on site erecting a polymer lined steel containment tank battery dike area. Baker Hughes pressure pumping service is on location and has assisted in the well bore toe prep operations. US Well service is slated to mobilize in the next 48 hours onto location to begin the well stimulation operation with a 6 existing wells on location slated to be stimulated. The well head assembly consists of a Cactus well head completion stack made up of a single Hydraulic Master Valve and Manual Master Valve sitting on top of the B-Section 10,000 psi rated lower well head assembly.

1273488972 2017-11-01 Preliminary Restoration No Violations Status check inspection found the well drilling and completion process finished with the well equipped with a production tree and the production valve open. The tin horn cellars and mouse holes have been filled in. There is a total of 6 complete active wells on the pad all in production. There is also a total of 8 production tanks all of 400 barrel capacity each. There is 2 vapor recovery burner units attached to the tank battery and 6 gas processing units on location also. The location signage is present at the entry point and there is also a gate across the lease road. The well pad location and lease road is of stone construction and in good repair. The well pad has a total of 6 outlet drain catch basins that are in the closed position. Addressing the restoration- The well site meets ORC 1509.072.
1481665812 2016-12-13 Casing String Witnessed No Violations On site inspection to witness the conductor casing cement job with Express Energys drilling and cement services. A 32 diameter borehole has been drilled to 135 and 26 diameter casing ran to 135. The casing was cemented using 435 sacks of class A cement and displaced with 77 barrels of fresh water and cement returns witnessed to surface. The job was delayed due to a hydraulic hose failing on the cement truck.

1526312088 2018-05-14 Production Wells No Violations Status check inspection found a total of 6 wells on the pad all in operation and producing. There is emergency shut down devices on both the tubing and well head production outlet flow lines with manual valves also present. Annular pressure monitoring gauges are present on the 13.375 and 9.625 well head sections with flow lines attached to each annular port leading to on site vapor recovery system. The wells are still flowing through sand trap separators before the product enters the gas processing units. Also on site is a 3rd party gas processing and distribution system.
2006768436 2017-02-23 Casing String Witnessed No Violations On this day I contacted the company man on site at 13:00 hours in regards to the up and coming surface cement job with H&P Drilling rig 519 on location. He indicated they were landing their last joint of casing in the hole and I proceeded to location so I could attend the pre job safety meeting. Upon arrival at 13:15 hours I was informed the last joint of casing would not go and they were attempting to wash the casing to bottom. The company proceeded to wash the casing eventually getting the casing to bottom at 17:00 hours. The following job description was noted. H&P Drilling drilled a 17.5 diameter surface borehole to a measured depth of 1,302. A Borehole deviation survey was ran. The rig then proceeded to run the following casing. 13.375 diameter J-55 grade 54.5 pound per foot buttress thread casing was ran with a guide shoe and float collar casing equipment in place along with proper centralizer placement being demonstrated. At 18:15 pm Schlumberger Cement Service began the cement job by pressure testing their cement lines to 2000 pounds per square inch and proceeding to start the cement job by pumping 10 barrels of fresh water followed by 30 barrels of 6% bentonite gel spacer chased by 10 barrels of fresh water and then 243 barrels of class A cement consisting of 1145 sacks of cement at 5.266 gallons of water per sack. The cement weight was verified by weight of scale. Upon completion of the pumping of the cement the cement plug was deployed and chased to the float collar with 193 barrels of fresh water. Upon the landing of the cement plug the casing was pressure tested for 15 Minutes at 500 psi over cement displacement pressure or 944 psi. 68 barrels of cement returns were witnessed to surface inside the cellar.
